DOCUMENTS ACCEPTABLE AS PROOF OF IDENTITY AND ADDRESS (For domestic individual Savings Account) List of Documents acceptable as both Proof of Identity and Address is as follows:
1. Passport (valid as on date) 2. Permanent Account Number Number (PAN) Card, 3. Voter’s Identity Card (Election Card) 4. Permanent Driving License which is valid with the photograph photograph affixed thereon 5. Job Card issued by by NREGA duly signed by by an officer of the State Government Government 6. Letter / Card issued by Unique Identification Authority of India (UIDIA) containing details of name, photograph, address, and Aadhaar number. Accordingly, either physical Aadhaar card/ letter issued by UIDAI received through post or the Aadhaar number validated through the e-KYC process adopted by the Bank is acceptable as an officially valid KYC document for onboarding the customer and for re-KYC purpose. Following additional documents are allowed to be accepted in case of the customer carrying out occupation mentioned in Annexure I, however these documents would not be accepted for customer who are opening Savings Accounts under Priority Banking, Wealth and Privee Segment / Products. 1. Identity Card with applicant’s photograph issued by Central /State Government Departments, Statutory / Regulatory Authorities, Public Sector Undertakings, Scheduled Commercial Banks, and Public Financial Institutions. 2. Letter issued by a Gazetted Officer containing the name, photograph and address of the individual, The photograph has to be duly attested by the Gazetted officer. Customers are advised to submit any one documentary evidence for proof of address, either current or permanent. In case proof of address furnished is not the mailing address, a declaration needs to be submitted along with the same declaration form available at br anches as per format in Annexure I I.

Annexure II (Specimen of Declaration to be obtained for proof of the mailing address)

In case the address submitted by me as proof, undergoes a change, I note to submit the fresh proof of address to the Branch for updation in the account records. I further confirm that if the copy of the proof of address is not submitted to the satisfaction of the bank, within 6 months, the Bank shall have the right to freeze / close the account. I hereby agree and undertake to hold harmless and keep Axis Bank fully indemnified against claims and damages, which may be made in respect hereof by reason of Axis Bank relying and acting on this declaration.
