Account Type: Online Saver Account Number: 33750780 Thank you for your application. To complete the account opening process please complete steps 1-3 below. Once you have completed this form please send it, together with the documentation and cheque requested, to: Freepost RSKH-AYTT-CRAB, Post Office® Savings, PO Box 520, WELWYN GARDEN CITY, AL7 9JE. 1. Confirming your identity and residential address Please: - Send us certified copies* of one document from List A to verify your identity and one document from List B to verify your address. Please note you are not able to use the same document to prove both your identity and address. - Place a tick against the documents you will send us below. * Certified copies must have “Certified Original Sighted” written/stamped on them with the certification signed and dated by a Solicitor, Accountant or Bank Official. You can send us original documents however, for security reasons, we do not recommend this. List A • Full UK/EU signed passport (not more than 10 years old) • Current UK/EU Photo card driving licence (not more than 10 years old) • Current full UK driving licence (old style) (not more than 51 years old) • Correspondence from DWP/benefits agency eg pension/Allowance benefit - dated within the last 13 months and showing your full name • EU National Identity Card (photographic) • Tax Coding notice/assessment - dated within the last 14 months List B • Utility bill – e.g. gas, electricity, water telephone (not mobile) - dated within the last 3 months • Bank/Building Society/ mortgage/ credit union/ Credit Card Statement - dated within the last 3 months • Current full UK / EU photo-card driving licence (not more than 10 years old) • Current full old-style UK driving licence (paper version) - (not more than 51 years old) • Local Authority Correspondence (e.g Rates / council tax) - dated within the last 13 months • Correspondence from the DWP / Benefits Agency - dated within the last 13 months • HM Revenue & Customs / Revenue Commissioners correspondence – e.g. Tax coding notice (P60s & P45s are not acceptable) - dated within the last 13 months Please note if we do not receive suitable documentation within the next 28 days your application will be cancelled and you will need to start the application process again.

2. Your Deposit Please send us a personal cheque for at least £1 made payable to your name(s) from your own bank account or building society account.

3. Your Signature(s) Your new Online Saver account is managed online. However, for your security, there are a small number of requests which we will require in writing e.g. change of address.
