Responsible for making sure all skin care consults were evaluated and immediate treatment initiated. 3. Performed monthly Quality Assurance of Restra int use, correct application of the device, and appropriate nursing measures pro vided and documented. 4. Assisted with implementation guidelines to encourage a "restraint free" facility. Certain circumstances warrant restraint use for th e safety of the patient and others. For behavior restraints require a face-to-f ace physical exam by physician within 1 hour of the application of the restraint and then every 4 hours thereafter. 5. New nursing standards put in place which decrea sed the number of "Code Blues" called on the Medical-Surgical areas as well as i mproved overall outcome. 6. Monthly Quality Assurance of Ventilator Acquired Pne umonia. Quality Improvement team developed an evidence-based "Ventilator Bundle" which drastically decreased rate on infection; thus, more successful outcomes w ere resulted. 7. Worked closely with the Bariatric Surgeons to develop an educat ional program to prepare patients prior to Gastric Bypass Surgery. I was respons ible for making rounds daily on all Bariatric Surgery patients, answering questi ons, making dietary suggestions, and providing encouragement. I spear-headed a m onthly support group and yearly fashion show. I served as a role model for the p rogram by actually having Gastric Bypass Surgery in 1993 and lost a total of 160 pounds. Personal Achievement: Survived an F-3 tornado on April 23, 2010. In a matter of minutes, I lost 75% of my house, 100% of my mother's, grandmothers, and father's houses. My spouse and I were unharmed, but my mother, grandmother, and father w ere blown out of their homes and missing among all the rubble. It seemed like ho urs had passed, when the three were finally found by my son and his friends, all with critical injuries. Ambulances could not get to us; therefore, we improvise d. Each of them was rolled over onto a twin size box-spring mattress using full spinal precautions. They were then placed in the back of 4 X 4 trucks and transp orted to the Emergency Room via back roads. After 7 days hospitalization, each w ent to Rehab Therapy for 19 days for exercise and strengthening. Everyone is imp roving and back in their replaced homes. As with everything, there are good and bad days. I am blessed daily with my family's love and presence. Miracles are pe rformed daily! My family and I are living proof!
