INTRODUCTION
This Manual sets out emergency response procedures for specific types of emergencies that may occur at the University of Regina, and is the property of the University of Regina. The Manual is for the exclusive use of University of Regina staff who have been assigned responsibilities in accordance with the University of Regina’s Emergency Management Policy, Emergency Closure Policy and Emergency Management Plan. (Yellow tab in manual) The University’s Emergency Response Procedures are comprised of three distinct sections: • The first section contains GENERAL PROCEDURES including the initial response and the responsibilities of Campus Security, Work Control Centre/Heating Plant, Emergency Wardens, trades personnel and the City of Regina Emergency Response Agencies. • The second section contains appendices for SPECIFIC RESPONSE PROCEDURES for use by University employees with emergency response duties and responsibilities. • The third section contains EMERGENCY CONTACT INFORMATION for University staff with emergency response duties and responsibilities. The GENERAL PROCEDURES contained in Section 1 can be accessed on the University of Regina website at: http://www.uregina.ca/hr/health_and_safety The complete Manual is only available in hard copy. To ensure confidentiality of personal information, only six copies of the manual have been printed. Persons receiving a copy of the Manual are responsible to ensure: • The manual is not copied or otherwise distributed to unauthorized persons. • The manual is kept up to date with updates provided by Health & Safety, HR • Upon leaving an emergency response position, the manual must be returned to Health & Safety, HR. “The success of any emergency response is dependent upon the experience, training, and cooperative attitude of the men and women who actively participate in the response. This, or any other Emergency Manual then, is only as good as the people who undertake these various responsibilities.”

AUTODIALER
The Regina Qu’Appelle Health Region Emergency Communications Center, located at 2755 Avonhurst Drive, Regina, provides the Autodialer service to the University of Regina under contract. The Autodialer is a high speed notification system that the University utilizes to make EMERGENCY NOTIFICATIONS (“Emergency” defined below) to specific lists of personnel and agencies. (“Emergency”: When a present or imminent event that is caused by accident, fire, explosion, technical failure or by forces of nature and requires prompt coordination and communication of action to protect the health, safety, or welfare of people or to limit damage to property.) Access to the Autodialer is restricted to the following authorized university Units: Campus Security Communications, External Relations Health & Safety, Human Resources The Autodialer contains four separate notification groups as listed here: 1. All call list: Includes President, Vice-Presidents, Associate Vice-Presidents, Deans, Directors, Unit Heads, URSU President, Federated Colleges, Regina Research Park and their identified designates. This list will also include individual positions identified by the committee. 2. Senior Executive list: Includes President, Vice-Presidents, Associate VicePresidents, Deans, Directors, and Unit Heads and their personal assistants/secretaries. 3. Emergency Management Committee: Includes all designated members of the Emergency Management committee and the UEOC support staff. 4. Building Wardens: Includes Chief Building Wardens and Assistant Chief Building Wardens for all U of R facilities.

ASBESTOS DISTURBANCE
INITIAL RESPONSE

ASBESTOS DISTURBANCE
ASBESTOS DISTURBANCE EVALUATOR WILL:
• • • • • • INSPECT and evaluate asbestos incident LOCK room or cordon off area and place signs REPORT back to WORK CONTROL CENTRE OR CENTRAL HEATING PLANT with recommendations for action ARRANGE for immediate cleanup and/or removal if necessary ENSURE that room or area remains locked and cordoned off until all cleanup and repairs are complete SCHEDULE non-emergency repairs or removal that may be necessary

HEALTH & SAFETY WILL:
• • • MONITOR cleanup and removal PERFORM air tests, if necessary NOTIFY as necessary: o Sask Ministry of Advanced Education, Employment & Labour – OH&S Division 787-4496 o Sask Ministry of Environment 1-800-667-7525 WRITE a report on the incident MAKE recommendations as necessary

NOTE: When an incident occurs after normal working hours, HEATING PLANT will notify the WORK CONTROL CENTRE first thing the next working day.

BOMB THREAT / SUSPICIOUS PACKAGE
INITIAL RESPONSE
THREAT BY PHONE The Receiver of the Bomb Threat will collect as much information as possible using the Bomb Threat Report (Appendix I). THEN: • CALL CAMPUS SECURITY: 585-4999 • REPORT the incident to your Supervisor • Provide all information to Police RECEIPT OF LETTER/CARD CONTAINING BOMB THREAT • HANDLE letter/card as little as possible to protect evidence • CALL CAMPUS SECURITY: 585-4999 • NOTIFY your Supervisor immediately SUSPICIOUS PACKAGES • DO NOT move or touch the package • NOTIFY your supervisor immediately • NOTIFY CAMPUS SECURITY: 585-4999 o TELL them exactly where the package is and why you feel it is suspicious • WRITE down everything you can remember about the package, including: o time you first noticed the package o where it came from and method of delivery, if known

CAMPUS SECURITY WILL:
• OBTAIN pertinent information, including: o Who is the person reporting the bomb threat? o What does the threat say and where is it directed? o Location of the complainant and where the Emergency Response Team can meet him or her CALL 911 and provide as much information to the Regina Police Services including location of Command Post NOTIFY Director of Campus Security: 585-4655 ATTEND the location to: o Secure the building or area as necessary o Provide crowd control or evacuation as necessary o Establish an Incident Command Post Prepare a detailed, written report of incident and forward copies to: o VP (Administration) o AVP (Facilities and Planning) o Director, Health & Safety

VICE-PRESIDENT (ADMINISTRATION) WILL:
• Based on information provided DETERMINE whether incident is a Level 1 or 2 Emergency o LEVEL 1 EMERGENCY will be managed by the Emergency Response Team o LEVEL 2 EMERGENCY will be managed by the Emergency Response Team supported by the Emergency Management Committee (REFER to Emergency Management Plan)

WORK CONTROL CENTRE / HEATING PLANT WILL:
• • • • OBTAIN pertinent information, including the location of threat DISPATCH Facilities Management personnel as necessary KEEP CAMPUS SECURITY informed ASSIST the Command Post as necessary

SEARCHING FOR SUSPICIOUS PACKAGES If the Incident Command Post/ Emergency Operations Centre determines the need to conduct and search for a suspicious package, the Director of Campus Security or the Director of Health & Safety will initiate a call through the University AUTO-DIALER to contact the Chief Building Wardens and advise them to have the Emergency Wardens work through their Zones having employees search their respective areas. NOTE: Persons who work in the area are best able to recognize things which are out of place, and are thus best suited to search that area. All such assistance will be fully voluntary. IF AN EXPLOSION OCCURS Refer to FIRE, FIRE ALARM and EXPLOSION SECTION

CHLORINE ALARM - KHS ROOM 156
INITIAL RESPONSE

CHLORINE ALARM - KHS ROOM 156
KHS STAFF TRAINED IN CHLORINE EMERGENCY RESPONSE WILL:
• • • • • • • Upon arrival at the site EVALUATE the situation for safety When there are at least two trained operators on site or one operator supported by the Regina Fire Department, TURN OFF the main chlorine cylinder valve ASSIST in evacuation and/or isolation of building as required EVALUATE situation and inform Facilities Management of additional requirements ARRANGE for ventilation of building and crawl space ARRANGE for repair of leak SCHEDULE non-emergency repairs or removal that may be necessary

HEALTH & SAFETY WILL:
• NOTIFY as required by regulations: o Sask Ministry of Advanced Education, Employment & Labour – OH&S Division 787-4496 o Sask Ministry of Environment 1-800-667-7525 ASSIST in investigation, isolation and repair of leak PREPARE a detailed written report of incident and make recommendations as necessary

ELEVATOR FAILURE TRAPPING PERSONS
INITIAL RESPONSE
PERSONS USING ELEVATOR • If trapped in an elevator you will have access to a telephone or a call button in case of any emergency • The phone will be answered by CAMPUS SECURITY or the HEATING PLANT • Provide all information as required • STAY CALM PERSONS AWARE OF AN ELEVATOR FAILURE • CALL CAMPUS SECURITY: 585-4999 • Assure trapped individuals that help is on the way • Stay to maintain communication with trapped person if possible • Direct Campus Security to the elevator site

CAMPUS SECURITY WILL:
• • • • • • OBTAIN pertinent information, including location of trapped person Establish location where caller can meet response team CALL 911 if there are injuries or medical concerns NOTIFY WORK CONTROL CENTRE/HEATING PLANT Attend the location to: o communicate with trapped person o provide assistance as necessary Provide report to the DIRECTOR, PRODUCTION

INITIAL RESPONSE
• • • • • • • • • ALERT others to the danger as you leave CLOSE the door to the area if possible PULL (RING) THE NEAREST FIRE ALARM LEAVE the building by the nearest exit DO NOT USE ELEVATORS NOTIFY 911 to report details of the emergency NOTIFY CAMPUS SECURITY: 585-4999 GO to the staging area and wait for Emergency Wardens; report details to the Wardens STAY to assist the Warden with information for the Fire Department

EMERGENCY WARDENS WILL:
• • • • INITIATE emergency evacuation procedures developed for their building CHECK, if safe to do so, their areas to ensure complete evacuation ENSURE that all evacuees move away from the building and there is an open route for arriving fire trucks MAKE SURE no one enters the building until the all clear is given by the Fire Department

CHIEF BUILDING WARDENS WILL:
• • • • • ENSURE, upon notification of fire alarm or fire emergency, that the Fire Safety Plan is implemented ENSURE that no one re-enters the building until the all clear is given by the Fire Department LIAISE with the Fire Department and Campus Security at the fire alarm panel to ensure effective communication MAINTAIN communication with Facilities Management as required Upon notification from the Fire Department that the building is safe, INFORM building occupants that they can re-enter the building

• If contacted directly, OBTAIN information, including: o Nature of the emergency o Location of the emergency o Whether injuries or loss of life occurred VERIFY that 911 has been called and given full information; if not, call 911 NOTIFY WORK CONTROL CENTRE/HEATING PLANT Upon arrival at the scene ESTABLISH an INCIDENT COMMAND POST in conjunction with the Chief Building Warden and the Fire Commander ASSIST the Incident Command Post by: o Providing radio communications o Providing crowd control as necessary o Providing assistance to the emergency response team Prepare a detailed, written report of incident and forward copies to DIRECTOR, HEALTH & SAFETY and DIRECTOR, PRODUCTION

APPENDIX II:

GENERAL FIRE AND EVACUATION PROCEDURES
EVERYONE HAS RESPONSIBILITIES

Every member of the University Community has a responsibility to be prepared for emergencies

WHAT EVERYONE SHOULD KNOW:
It is your responsibility to know, before an emergency occurs: • The location of the nearest emergency escape route/fire exit • The location of the nearest fire alarm pull station • The location of the nearest fire extinguisher • The location of the staging area (evacuation assembly point) for your building • The City of Regina emergency telephone number: 911 • The University emergency telephone number: (585)-4999 • The EVACUATION PROCEDURE and FIRE PROCEDURE

WHAT EVERYONE SHOULD DO:
• • Keep hallways, stairways and fire exits clear Report blocked hallways, stairways or fire exits to WORK CONTROL CENTRE 4039 during normal working hours or to CAMPUS SECURITY 4999 at other times

• • • • • • •

EVACUATION PROCEDURE: IF THE FIRE ALARM SOUNDS:
IMMEDIATELY PROCEED TO THE NEAREST FIRE EXIT! Proceed calmly but rapidly. CLOSE DOORS as you go if you are the last person out Do not wait for more information or to ascertain the cause of the evacuation DO NOT USE ELEVATORS! Follow the instructions of the Emergency Wardens (orange vests) or Campus Security officers Go to the designated staging area (evacuation assembly point) for your building DO NOT RE-ENTER THE BUILDING until the Fire Department gives clearance

NEVER ENTER A BUILDING WHEN THE FIRE ALARM IS SOUNDING!

IF YOU DISCOVER A FIRE:
• • • • • Alert others to the danger as you leave; CLOSE THE DOOR to the room or area involved PULL THE NEAREST FIRE ALARM Leave the building, following the EVACUATION PROCEDURE As soon as you can do so safely, CALL 911 to report details of the emergency Go to the designated staging area (evacuation assembly point) for the building; tell the Emergency Wardens details of the fire. Stay in case the Fire Department has additional questions.

APPENDIX III:

RESPONSIBILITIES OF CLASS INSTRUCTORS

The University has a responsibility to protect the health and safety of students. Anyone instructing a class, as a representative of the University, shares this responsibility and is expected to show due diligence and take all reasonable measures to maximize the safety of students.

RESPONSIBILITIES OF ALL CLASS INSTRUCTORS:
• • • • At the beginning of the semester, learn the evacuation routes from each classroom you will be teaching in and the staging area (evacuation assembly point) for the building. In the first class of the semester, call attention to the University Fire & Evacuation Procedure, the designated emergency exit route from the classroom being used, and the designated staging area for the building. Contact the Chief Building Warden of the building for more information as required. Also in the first class of the semester, request that any student who might need assistance during a building evacuation come to you and tell you what they need. This information must be reported to the Chief Building Warden, who can arrange for assistance.

IF A FIRE ALARM SOUNDS, YOU AND YOUR CLASS MUST EVACUATE THE BUILDING IMMEDIATELY.
• DO NOT USE ELEVATORS DURING AN EVACUATION. If a person with a disability cannot be evacuated through a stairway, they must be taken to a designated place of refuge until help arrives. (All inside fire escapes are designated places of refuge). Contact the Chief Building Warden of the building for further information. Direct your class to the designated staging area for the building. If any members of the class are thought to be missing, inform the Emergency Warden or Fire Department. Do not allow the class to re-enter the building until the Emergency Wardens, Campus Security, or the Fire Department give clearance.

RESPONSIBILITIES OF EVENING/WEEKEND CLASS INSTRUCTORS:
• The EMERGENCY WARDENS are only available during regular working hours. Those instructing classes on evenings and weekends therefore have the following additional responsibilities: o Guide or direct students to the nearest exit, and direct them to the staging area. o Check for stragglers and students who did not/could not leave the classroom. o Assist, or coordinate the assistance of, any student who requires help to evacuate the building. Appendix I:

Emergency Response Procedures

Assume a low oxygen atmosphere exists when approaching. Do not approach a potentially low oxygen atmosphere alone or without a toxic gas monitor. Only ITC emergency personnel with gas monitor and NMR training may investigate a low oxygen alarm. Do not approach the potentially low oxygen atmosphere if feeling unwell or unsafe. Ensure a second responder remains outside the potentially low oxygen toxic area but within view. If a second responder is not available consider having the Regina Fire Department attend to the scene.

WITH ANY INDICATION OF A MAGNET QUENCH, CALL VARIAN IMMEDIATELY: 1-800-356-4437 Quote instrument serial number: S011790

1. Make sure you have: a) ITX personal gas monitor capable of measuring oxygen (available from GG313). b) Second qualified responder. 2. Ensure gas monitor is operating properly. 3. Ensure personnel are away from the south and west doors leading into GG112, as well as the north entrance to GG112.1 and the north stairwell. 4. Open the south door to GG112 and carefully enter GG112. Slowly approach the NMR lab while monitoring oxygen levels. 5. If the oxygen level falls below 19.5% in GG112 (or you feel unwell in any way), leave the lab immediately and close the door. This signifies a release of helium and/or nitrogen gas from the NMR magnet, which displaces the oxygen. a) The ventilation system in GG112.1 will remove the helium and/or nitrogen gas, but sufficient time must be allotted for this. b) Immediately verify that all entrances to GG112 and GG112.1 are closed.

c) Post signs and barriers at all entrances to GG112, GG112.1 and the north stairwell to indicate there has been an NMR magnet quench, and that these areas are oxygen deficient and cannot be entered until further notice. Follow steps 1-5 as well when approaching the north stairwell. d) For the next hour, check these entrances frequently to ensure barriers have not been crossed. e) When an hour has passed, repeat steps 1-5. If oxygen levels are still unsafe, check again in another hour. f) When oxygen levels are safe in GG112 and the north stairwell, proceed to step 6.

6. If oxygen levels are safe in GG112 and the north stairwell, approach the south door to GG112.1 and look into the lab. If oxygen levels are safe but there are visible signs of a magnet quench (venting from the magnet, fog and condensation around the magnet and other equipment), do not enter the lab. Return to step 5. Do not attempt to enter GG112.1 until at least an hour after the magnet stops venting. If there are no signs of a quench, proceed to step 7. 7. If the amount of time specified in step 6 has passed, or there are no signs of a magnet quench, slowly enter GG112.1 while monitoring oxygen levels. a) If oxygen falls below 19.5% in GG112.1, return to step 5. b) If there was a quench but oxygen levels are safe in GG112.1, silence the alarm and survey the room to ensure all conditions are safe according to Varian’s advice. c) If oxygen levels are safe and there was no quench, the oxygen alarm was false. Silence the alarm and contact work control to arrange for recalibration or repair. Ensure the NMR lab is not used until the oxygen alarm is functioning properly. 8. When the alarm has stopped and conditions are safe, remove all signs and barricades and return the gas monitor to GG313.
