Running head: HEALTH TEACHING PLAN – WOUND CARE AND DRESSING CHANGE

Assessment
Objective: - 35 year old male - He has a broken arm and three broken ribs due to motorcycle accident. - Many severe wounds mostly on his leg such as four inches gash along the side Mr. B.‟s calf. - Daily sterile dressings are needed. - Mr. B. will be discharged today (Jan. 30, 2013). - His wife has agreed to continue with the dressings at home. - His wife is crying and when the wound is uncovered, she

Objectives
Following the teaching session, 1. Mrs. B. (the patient‟s wife) will feel comfortable to look at and touch the uncovered wound and will be able to identify the signs and symptoms of wound infection on Jan. 27, 2013.

Planning
Short Term Goal 1. Mrs. B will feel free to looking at and touch the uncovered wound and can correctly list signs and symptoms of wound infection on Jan. 27, 2013.

Implementation
First Session The nurse and Mrs. B. will start their first teaching session in quiet small conference room in the hospital from 2:00pm to 2:45 pm on Jan. 27, 2013. 1. a. The nurse will start to show Mrs. B. a piece of artificial wound, let her touch and slowly get rid of fears until finally she can look at her husband‟s uncovered wound. b. The nurse will use a video from YouTube to show infected wound appearance, and also give handout about signs and symptoms of wound infection such as the following (Potter &Perry, p1246):  Pain and tenderness at the wound site  Reddening of the surrounding tissue  Swelling  Inflammation of wound edges  Purulent discharge  Warm to touch  Fever, chills  Foul odor Second Session

Evaluation

1. Mrs. B. will verbalize that she feels comfort to look at and touch the uncovered wound, and when the nurse open her husband‟s to change dressing, she can look at it by the end of first teaching session on Jan. 27, 2013. She will also correctly describe the signs and symptoms of wound infection by the end of first teaching session on Jan. 27, 2013.

HEALTH TEACHING PLAN – WOUND CARE AND DRESSING has difficulty to look at the wound. - Mr. B.‟s follow up appointment will be next week (Feb. 4, 2013). Subjective: - His wife is anxious to learn how to care for the wound, signs and symptoms of complications, sterile technique and other related aspects, but she appears uncomfortable. Assessment Activities (one on one in a quiet room) 1. Assess Mrs. B.‟s readiness to learn, and ask “do you agree to continue with the dressings at home until next follow up?” 2. The nurse will ask what signs and The nurse and Mrs. B. will start their second teaching session in quiet small conference room in the hospital from 2:00pm to 2:45 pm on Jan. 28, 2013. 2. Mrs. B. will be able to discuss and demonstrate appropriate wound care skills after the nurse‟s demonstration of cleansing wound on a piece of artificial wound and on her husband Jan. 28, 2013. 2. The nurse will demonstrate wound cleansing on a piece of artificial wound, and then perform wound care on her husband, and also provide handout and discuss available resources on Jan. 28, 2013. The major information of wound care include Potter &Perry, p1271):  If the doctor prescribes the pain medication, let Mr. B. takes medication half hour ago before wound care and dressing change.  Wash your hands before and after each wound dressing change. Put on a pair of non-sterile gloves. Carefully remove the tape and old dressing. Check the wound for signs and symptoms of infection, and check the amount, color, consistency and odor of drainage. Cleanse the wound 2. Mrs. B. will correctly answer the questions regarding wound care and give an appropriate return demonstration of wound care by the end of second teaching session on Jan. 28, 2013.

Ask Mrs. B. some questions such signs and symptoms of wound infection, how to keep sterile technique, and etc. Let Mrs. B. perform a return demo of wound care and dressing change and ask her what next follow up date is.

Long Term Goal: The client‟s wound will be healing without signs of infection or

The client„s wound will be healing well at next follow up appointment. It will indicate that Mrs. B. will have mastered the wound –care and dressing change skills. Therefore, no future needs will be required.
