In the past decade, thanks to the clinical studies of Wilcko & Wilcko, a new chapter has developed in clinical orthodontics: the Periodontally Accelerated Osteogenic Orthodontics (PAOO). Based on the combined effects of the Regional Accelerated Phenomenon (Frost) induced by the alveolar corticotomy and hard- and soft-tissue grafting, PAOO has proved effective in reducing treatment time and expanding treatment possibilities. Segmental corticotomy, limited to the teeth to move, has also been proposed to reduce anchorage needs and facilitate complex tooth movements (i.e. upper molar distalization, lower molar mesialization, extrusion, intrusion, and so forth). This procedure is known as Periodontally Facilitated Orthodontic (PFO) and can be combined to skeletal anchorage and proper biomechanics to modulate orthodontic tooth movement. Several surgical procedures have been proposed to achieve PAOO and PFO. The author will try to elucidate the different aspects, indications, contraindications and limits of piezocision, segmental corticotomy, PAOO, and osteotomy. The different surgical procedures to produce the alveolar corticotomy and the role of grafting will be presented and discussed. The combined orthodontic treatment, appliances and proper wire sequence will be shown. Several clinical cases will be presented.
