Analysis of Patient s response The patient recognizes the nurse and is conveying indirectly that she is open up for conversation. She said she is happy which shows a good sign of a good start of effective conversation between her and the nurse. Her smile connotes a merry disposition which is a sign that she is willing to socialize.

(Orientation Phase) in this phase, as the nurse have her initial interaction with the patient, the nurse tries to build rapport with the patient. Through this that the nurse will be able to establish boundaries of relationships and also to identify the expectations. If the nurse will be able to build rapport, then he/she can assess well and give the appropriate nursing care to them.

Patient: ok naman ako ngayon. masaya naman ako.(smiles)

2.)Nurse: kumain na po kayo? Ano pong ulam niyo kanina?

Patient:ah, isda at monggo ung kinain namin. Kumain nga ako ng marami kasi masarap sya.
