Medical Home Builder  2.0 CareFirst BlueCross BlueShield and CareFirst BlueChoice, Inc., collectively CareFirst, have partnered with the American College of Physicians1 (ACP) to offer a web-based practice education and improvement tool at no cost to all CareFirst Patient-Centered Medical Home (PCMH) participants. As a PCMH participant, you and your office staff will have access to ACP’s ACP’s Medical Home Builder 2.0 (MHB 2.0). The MHB 2.0 tool is designed to help your practice improve patient care, understand workflow and improve team dynamics within your PCMH. As an online and collaborative tool, MHB 2.0 helps you transform your practice in easy steps, on your own timeline. Get ready with background information

Conduct an initial practice assessment with ACP’s Practice Biopsy ® Review your results, recommended modifications and resources Identify and implement desired changes for increased quality that may impact your Outcome Incentive Award



Share and collaborate with your fellow PCMH participants through the MHB online community.

 An Online Community tto o Share and Learn Learn With MHB 2.0, you’re part of an online community of PCMH practices, large and small, who are committed to improving patient care. You and your office staff will also have access to best practice videos and a discussion board where questions can be posed to a community of MHB colleagues.

MHB 2.0 Modules and Features: MHB 2.0 is organized into modules that address various topics related to practice.  You and your office staff can complete the modules independently, independently, or work together in small groups. Modules currently fall into two general categories: Medical Home, and Clinical Topics. A third category (Practice Management) will be built beginning in 2012. Each module



The Practice Biopsy ® — a self-assessment included in each module that covers National Committee for Quality Assurance (NCQA), Utilization Review Accreditation Commission (URAC), and draft Joint Commission standards. The Practice Biopsy® will help identify your practice’s opportunities for improvement  – or validate the efforts your practice has already undertaken in this area.



A Case Study — “How It Works in Practice” is included to describe the efforts of one practice as it works through the findings generated through each module.



The Resource Library — a virtual bookshelf, which includes relevant references and informative guides in a variety of formats including downloadable guides and policy templates.



Reporting functions to help track progress over-time, identify opportunities for improvement and compare your practice with others.
