BASED ON A FACT FIND ING SESSION OF THE EXECUTIVE B OA RD OF DIRECTORS HELD ON MO NDAY DECEM BER 26, 20 11, 900AM AT ANSONIA RESCUE M EDIC AL SERVICES HQ, THE BOARD FINDS THE FOLLOWING SEQUENCE O F EVENTS

T U E S DAY, D E C E M B E R 2 7 , 2 0 1 1 T I M E L I N E :

1. On or about Friday December 16, 2011 Jerry contacted Bob to ask for the new truck to drive. Bob acknowledged and stated he would have the truck at Oxford EMS for Monday December 19, 2011. 2. Sometime after that Madalene saw Bob in passing and told him she would be using the truck to move some items. Bob acknowledged. 3. On Tuesday December 20, 2011, Madalene took the truck to the Washington DC area to get items belonging to her son’s girlfriend’s deceased father. On the return trip to CT on Wednesday December 21, 2011 in the evening Madalene was involved in a MVA in Bel Aire Maryland on I-95 NB at mile marker 75.5. Madalene had her son Travis and his girlfriend Shelby in the vehicle with her. Madalene contacted Jerry immediately via phone and Travis sent Jerry pictures of the truck. Maryland State Police responded and are completing an accident investigation and report. Jerry attempted to contact Bob via phone and ultimately left him a message and got a call back a few hours later. Jerry asked Bob to keep the matter handled internally. Bob informed Jerry and Madalene that he would not lie if someone asked him about the incident. 4. On Thursday December 22, 2011 Jerry contacted Frank Pellacia from Auto Repairs Unlimited to make arrangements for repair of the vehicle. Frank stated he would make arrangements with Gabby’s to repair the vehicle. Jerry asked Frank to make sure that the vehicle was not visible in an effort to avoid the involvement of the press. Madalene drove the vehicle back to CT to Oxford Ambulance and then to Gabby’s for repair. Vehicle arrived at Gabby’s in the late afternoon on December 22, 2011. Jerry told Gabby’s to fix it quick and not worry about who was paying for it as the other vehicles insurance would be covering it. 5. On Friday December 23, 2011 Bob was informed that another Board member had seen damage to the new truck. Bob told the member what had transpired. Bob contacted Jerry and let him know that others were aware of the incident. Jerry then called Jared and Bob also texted Jared to call him. Jerry called Jared and informed him of incident. Jared called Bob back who also informed him of the event. Jared contacted Shannon to inform her. Jared and Shannon attempted to see the truck at Gabby’s however they were closed. Jared then contacted Noreen and informed her. Jared asked Noreen and Shannon to ask for a meeting and have an investigation conducted by the non-involved Executive Board members. Jared sent an email to all involved to meet. 6. Multiple emails were sent over the Christmas Holiday weekend.

Was the Executive Director notified that the vehicle would be used out of state? Was information purposely withheld from the Board in an attempt to fix the vehicle before anyone knew? Did the President contact the Vice President only after he was notified that another Board member knew about the incident? Did the Supervisors and Executive Director utilize the new vehicle out of state as well for non VEMS related functions?
POSSIBLE CORRECTIVE ACTIONS

1. Develop a New Vehicle Policy and a Vehicle Usage Policy. 2. Develop an Accident Reporting Policy/Protocol. 3. Develop a notification policy for Board Members. 4. Develop a policy when there is a conflict who should assume responsibility for handling the issue. 5. Present a brief report to the Full Board at the next meeting outlining events. 6. Jared Heon to handle anything else pertaining to this event as Jerry Schwab is recused due to personal involvement. 7. Bob Pettinella to inform VFIS of the accident and obtain a detailed Accident Report from Maryland State Police when available. 8. Notify Katherine Matthies Foundation of the accident with the vehicle (purchased from grant money).
F OL L OW U P

1. Jared Heon will complete detailed report as well as report to Full Board of Directors. Jared Heon will present the incident to the Full Board at the next Regular Meeting. 2. Jared Heon will oversee repair and outfitting of new truck in cooperation with Bob Pettinella and Supervisors. 3. Status of new policies pertaining to accidents and vehicle usage.
