BIRTH
TIMELY REGISTRATION OF CERTIFICATE OF LIVE BIRTH
(Within 30 days from birth)

REQUIREMENTS:
1. Accomplished Certificate of Live Birth prepared by hospital/clinic administrators/nurse/midwife. 2. Attendant at birth should appear in person to sign the Certificate of Live Birth, if the child was delivered at home. 3. Registered Marriage Contract of parents, (NSO or LCRO copy; or Civil Registry Form 3A – original should be presented), to determine legitimacy. 4. If illegitimate child, father should appear in person to acknowledge the child and bring the following: a. Valid ID with signature b. Official Receipt – Legal Instrument Registration Php100.00 (Affidavit of Acknowledgement/Admission of Paternity)

DELAYED REGISTRATION OF CERTIFICATE OF LIVE BIRTH
(beyond 30 days from birth)

REQUIREMENTS:
1. Certificate of Live Birth prepared by hospital/clinic/ nurse/midwife 2. CRS Form No. 1-Negative Certification of Birth (original) from National Statistics Office 3. Verification Result – from the Local Civil Registrar 4. Posting of application for 10 consecutive days 5. Affidavit of 2 disinterested persons who had witnessed or known the true facts of birth of the child stating among others, as the case may be, the: a. b. c. d. e. f. name, date and place of birth of the child complete name of parents child was born out of wedlock, if illegitimate whereabouts of the mother & father, if the person registering is not the parent, name and address of the attendant at birth child is baptized and immunized

6. Registered Marriage Contract of parents, (NSO or LCRO copy; or Civil Registry Form 3A - original should be presented) to determine the legitimacy. 7. Registered Marriage Contract of person to be registered, if 18 years old & above, if married. (NSO or LCRO copy; or Civil Registry Form 3A - original should be presented) 8. Any two of the following documentary evidences which show the facts of birth specifically the date and place of birth: a. b. c. d. e. f. g. h. i. j. Baptismal Certificate or Certificate of Akeca for Muslims School Records or Transcript of Records Income Tax Return Insurance Policy Medical or Immunization Record with Dr. or Midwife’s signature over printed name PhilHeath Membership Data Record Voter Certification Passport SSS or GSIS Record Employment Record

9. Valid ID with signature of affiant

10. Attendant at birth should appear in person to sign the Certificate of Live Birth, if the child was delivered at home. 11. Official Receipt from the City Treasurer’s Office for authorized fees:
6 months – 1 year old ₱ 50.00

1 year old - 5 years old 100.00 5 years old – 10 years old 200.00 10 years old and up 300.00 Certified machine copy 50.00/document Legal instruments registration 100.00 (Affidavit of Acknowledgement/Admission of Paternity) 12.If illegitimate child, father should appear in person to acknowledge the child and bring the following: a. Valid ID with signature b. Official Receipt-Legal Instrument Registration
