To collect an adequate sample of sputum for laboratory analysis and culture. To minimize contamination of the sample with oral or other secretions.
Assessment
y Verify the type of test to be performed (cytology studies should be collected into a cup containing a preservative solution; cultures must be collected into a sterile container) y Assess the client·s level of consciousness and ability to follow instruction. y Assess the client·s breath sounds; coarse rales indicate the presence of sputum in the airways.
Interventions:
y Obtain correct specimen container y Wash hands and don clean gloves y Asssist the client to rinse the mouth with water (not mouthwash) y Instruct client to raise sputum from the lungs not the throat of the nose. y Instruct client to expectorate sputum into the cup without touching the inside of the container
Interventions:
y Replace cap on container as soon as sample is obtained. Label container and wash the outside if indicated. y Place in a bag with biohazard label for transport y Provide mouth care for the client and assist to a comfortable position y Remove gloves and wash hands. Send specimen to lab immediately.
Documentation:
y Document amount, color, character and odor of sputum obtained and time the specimen was sent to the lab.
