VERIFICATION
Definition: It is an averment by the party making a pleading that he is prepared to establish the truth of the facts which he has pleaded. A pleading is verified by an affidavit stating that the person verifying has read the pleading and that the allegations thereof are true of his own knowledge. Verifications based on “information and belief,” or upon “knowledge, information, and belief” shall be deemed insufficient. (Rule 7, Sec. 6, Revised Rules of Court)

SAMPLE:

I, __(Name of affiant)__, after being sworn in accordance with law, deposes and says: “That he (her) is the petitioner in the above-entitled case; that he has caused the (above) foregoing petition (complaint) to be prepared and has read the contents thereof; that the allegations therein are true and correct of his (her) own knowledge.
