2011 Scholarship Application

The purpose of the PTA Scholarship Awards is to develop student creativity and interest in a wide range of subject areas, to help students build lifelong skills, and to foster student growth as well-rounded individuals. We encourage all students to take advantage of this opportunity. Two recipients per grade will receive scholarship awards. Students may apply in any subject area of interest to them, BUT only one application per student will be accepted. All students in grades 5- are eligible to apply. Applications consist of a Student Application form and !arental Approval form. Answers must be typed double spaced and may not e!ceed "## words. "ack of !arent Approval form dis#ualifies the applicant from further consideration. The awards may be used for instruction, enrichment, materials or e#uipment re#uired to promote growth in the subject area of the award. The !TA Award $ommittee is composed of three teachers four parents and the principal. The committee selects award recipients. %ecipients are chosen on the basis of the enthusiasm and interest in the subject area e&pressed by the student on the application. 'nly the $ommittee $hairperson knows the identity of the applicant until the selection process is completed. $ompleted and signed applications should be returned to the school office in an envelope marked PTA S$%&'A(S%)P APP')$AT)&* by the end of the school day on (riday, )ay *+, ,-**.
