2012 Veterinary Economics Hospital Design Competition
Official Entry Form Judges will choose one Hospital of the Year and 10 Merit Award Winners. Follow these guidelines to prepare your entry: • The hospital must have been completed between Jan. 1, 2007, and Nov. 20, 2011. • All types of practices are encouraged to enter (small animal, equine, specialty). • The hospital must be owned by a private practitioner(s) or privately owned corporation. Hospitals funded by foundations, government agencies, nonprofit organizations, or publicly traded corporations are ineligible. • Each entry must include a complete color photo “tour” of the hospital’s interior and exterior. Judges rely heavily on photos that show the features you consider unique and important and how the hospital enhances the efficiency of the team and the clients’ experience. See Section IV of the entry form for details. • Each entry must include a site plan and a presentation drawing of the hospital’s floor plan. See Section II of the entry form for details. To prepare your entry: 1. Fill out the Official Entry Form from dvm360.com/hd using Microsoft Word or a compatible software application. 2. Use a standard-size, three-ring notebook with pockets on the inside covers, preferably no thicker than 2 inches. 3. Insert each entry item into a clear, nonfrosted sheet protector or photo-album page; bind each sheet protector in the notebook. 4. Place items in this order: 1) Entry form; 2) Presentation drawing/site plan; 3) Photos (placed in clear, nonfrosted plastic sleeves with a label and caption for each photo); 4) Letters, newspaper articles, or other supporting material. 5. All entries must be postmarked by Nov. 25, 2011.

Section II Architectural and design information (to be completed by the architect and/or designer) Include a summary for Section II that outlines your overall design objectives and any specific design goals for different areas of the practice. Please explain any obstacles you overcame. Then answer the numbered items below. Answer each numbered item on a separate sheet, and limit each to one page or less. (You may need to write only a few sentences.) Place each individual sheet in a nonfrosted sheet protector. 2.1 Site plan and site use Include a sketch of the lot, showing the building, parking lot, landscaping, and signage. 2.2 Floor plan Please provide a labeled presentation drawing of each floor, drawn to scale (with scale shown), including square-foot dimensions. Your architect or contractor can tell you how to get a presentation drawing made. No blueprints or specification booklets, please. Also include a digital copy of your presentation drawing on a CD-ROM. Digital floor plan images should be 8-by-11-inch scanned line art at 600 dpi or higher in TIF or JPG file format. If your project was a conversion or involved additions and/or alterations, please include a rough but clearly labeled sketch of the pre-existing floor plan. 2.3 Floor plan, traffic flow On a separate copy of the floor plan, please indicate the traffic patterns for veterinarians, staff, and clients using different arrows or colored markers for each group. 2.4 Finish materials Give an overview of materials used in each category, noting manufacturer or brand name. Describe the following: • flooring and flooring base • walls • ceiling • counters or work surfaces • special features 2.5 Environmental systems Give an overview of materials used in each category, noting variations from room to room, if applicable. Include the following: • heating and cooling (on a separate copy of the floor plan, please indicate isolated heating and cooling zones) • odor control • provisions for ventilating animal quarters • controlled drug storage • noise control • radiology • provisions for cleaning animal quarters (for flush drains, show run drainage on floor plan)

Section III Practice information (to be completed by the owner) Please use a separate sheet of paper for each numbered item below, and limit your answers to one page or less. (You may need to write only a few sentences.) In each answer, please explain any objectives you had and any obstacles you overcame. Place each sheet in a nonfrosted sheet protector. 3.1 Financing Briefly explain how you financed the building and describe any problems you encountered. 3.2 Practice philosophy Discuss your practice philosophy and explain how it affected design and construction decisions. 3.3 Site selection and utilization How did you determine your site’s suitability? Describe the neighborhood where your hospital is located. Did you experience any zoning problems, and if so, how did you resolve them? Did you face any special climatic or environmental problems? 3.4 Summary Describe any additional unusual features. Include ideas borrowed from other veterinary hospitals as well as your own design innovations. If you could change one thing about your facility, what would it be? What provisions did you make for future expansion? What trends (environmental, pet superstores, grooming or boarding business, etc.) influenced your design decisions? Did you make any special provisions for communication systems like computers, practice software, telephones, security cameras? What advice would you share with others who are considering building a hospital? Have you attended the Veterinary Economics Hospital Design Conference at CVC Kansas City? If yes, what did you learn at this event? What resources did you use as you were planning to build your hospital?

Section IV Photographs A printed set of labeled interior and exterior color photographs must accompany your entry. Photos must be previously unpublished. Also, send all photo images on a CD in JPG format and at least 5-by-7 inches at 300 DPI. Name all photo files according to the area of the hospital they represent (reception.jpg, treatment.jpg, etc.). Insert the CD on which the photos are saved into a plastic sleeve in the notebook. Please double-check the files on the CD before you send in your entry. Insert the CD into a computer to make sure the files load correctly. Please don’t send more than 60 photos. Incorrect photo formatting may result in the disqualification of your entry. Photo quality is essential, as is the presentation of your entry. Judges must see each room’s layout, furnishings, and equipment clearly. Send the best quality photos possible. We strongly recommend using a professional photographer and including staff members or clients in pictures. Please include one 8-by-10-inch photo of the facility’s exterior and the reception area. Other photos should be 5-by-7 or larger. Place professional quality photos in nonfrosted plastic sleeves. Label each photo clearly and write a caption describing the room and its features. You may also want to include a “before” photo of your previous facility or your current facility before remodel or addition. At least one photo of each of the following (if applicable) is required. Unless a size is otherwise specified, please provide 5-by-7-inch photos or larger. 1. Each side of the facility exterior (one 8-by-10 required) 2. Reception area (one 8-by-10 required) 3. VETERINARY ECONOMICS HOSPITAL DESIGN COMPETITION OFFICIAL RULES NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. ELIGIBILITY: VETERINARY ECONOMICS HOSPITAL DESIGN COMPETITION (“Contest”) is open to owners of veterinary hospitals that are privately owned and legally organized under the laws of the United States or veterinarians who are owners of hospitals as private practitioners of legal age of majority in their jurisdictions of residence (and at least 21) who are also legal residents of the 50 United States and the District of Columbia (excluding Puerto Rico and all other territories). Failure to comply by providing incorrect information will invalidate your Contest entry, which will result in the loss of potential prizewinning entry. Employees, officers, and directors of Advanstar Communications Inc. All entries must be postmarked by November 25, 2011. Incomplete entries will not be accepted. Limit one (1) entry per hospital per year. All entries become the sole property of the Sponsor and will not be returned. The entry must be the veterinary hospital owned by entrant, the entrant’s own original work, be in English, true and verifiable, be in keeping with the Sponsor’s image, and may not have been submitted in any other contest or won any type of award. The Sponsor reserves the right to disqualify any entry it deems to be offensive, inappropriate, not in keeping with Sponsor’s image, or otherwise not in compliance with these Official Rules, in its sole discretion. Released Parties (as defined below) are not responsible for any damage, theft, or loss to any entrant’s property resulting in whole or in part, directly or indirectly, from participation in the Contest. Following the contest judging, potential winners will be notified by telephone, mail, and/or e-mail and are required to execute and return an Affidavit of Eligibility and Liability/Publicity Release (where permitted) within 14 days. Failure to comply within the time period specified, the return of any notification as undeliverable, or noncompliance with these Official Rules by a potential winner will result in disqualification and, at Sponsor’s discretion, a runner-up may be notified. Winners will also be required to sign Sponsor’s Standard License Agreement granting Sponsor and its affiliated entities the nonexclusive worldwide perpetual license to edit, use, reuse, republish, reproduce, translate or otherwise distribute or disseminate the Material in any form or media (including digital electronic form and via the internet), whether now known or hereafter devised (hereinafter, “Reuse” or “Reusing”); provided, and the worldwide perpetual right to exploit, edit, modify, and distribute such photographs as well as the entry (including photographs submitted as part of entry) and all elements of both in any and all media now known or not currently known (including in current and future advertising and promotions throughout the world in perpetuity without compensation, permission, or notification to winner or any third party. JUDGING: All eligible entries will be judged by a panel of qualified judges in each of the following categories and criteria: originality of design (10 points), site plan (10 points), mechanical features (10 points), outpatient areas (10 points), plumbing features (10 points), inpatient areas (10 points), electrical features (10 points), quality of finish materials (10 points), and overall features (20 points). In the event of a tie in any part of the judging or in any category, an additional “tie-breaking” judge will determine the applicable winner based on the applicable criteria listed herein. Judges’ and Sponsor’s decisions are final and binding on all matters relating to this Contest. Sponsor reserves the right to not award any prize for any category if, in its sole discretion, it does not receive a sufficient number of eligible and qualified entries for such category. Winners will be announced on or about March 15, 2012. PRIZES: Grand Prize: One potential winner’s hospital as described on the Official Entry Form will be named the Hospital of the Year Grand Prize Winner and will be featured in an article created by Sponsor that will appear in an issue of Veterinary Economics.. Winner’s representative(s) authorized by winning hospital will receive complimentary registration to 2012 CVC Kansas City, Aug. 25 to 28 in Kansas City, Mo., held in conjunction with the Hospital Design Conference, for two representatives from the practice. Grand Prize also includes roundtrip coach class airline transportation for the two hospital representatives between the major U.S. airport nearest winner’s residence and/or winning entry’s location and CVC Kansas City, and hotel accommodations for each representative (standard occupancy). Travel details are subject to provider terms and conditions. Winners must make travel arrangements through Sponsor’s designated travel agent. All details of the trip not specifically listed herein will be determined by Sponsor. Winner’s are solely responsible for any expenses incurred in connection with the prize not expressly described above as being awarded including those expenses as may be incurred by winner’s representatives, and including without limitation, applicable federal, state or local taxes. Winner’s representatives must travel together on same itinerary,

and each such representative must be at least 21 years of age and each must sign a liability/publicity release prior to travel. Approximate retail value for the Grand Prize: $2,000. Actual retail value depends on dates of travel and city of departure. First Prize: Eleven Merit Award Winners, including the Hospital of the Year winner, will earn plaques of an Approximate Retail Value (“ARV”) of $100 each. No substitution, transfer, or cash equivalent of prize is permitted except by Sponsor, which reserves the right to substitute a prize of comparable or greater value at its sole discretion. Any and all taxes on the prize, and any other costs and expenses associated with prize acceptance and use not specified herein as being awarded are the sole responsibility of the winning hospital. All prize details will be at the sole discretion of the Sponsor. GENERAL: By participating, each entrant agrees: (a) to abide by these rules and decisions of Sponsor and judges, which shall be final in all respects relating to this Contest; (b) to release, discharge, and hold harmless Sponsor and its respective parents, affiliates, subsidiaries, and advertising and promotion agencies, and all of the officers, directors, shareholders, employees, agents, and representatives of the foregoing (collectively, “Released Parties”) from any and all injuries, liability, losses, and damages of any kind to persons, including death, or property resulting in whole or in part, directly or indirectly, from entrant’s participation in the Contest or any Contest-related activity (including any shipping, travel, or activity related thereto), the acceptance, possession, use, or misuse of the awarded prize or the use by Released Parties of any rights granted herein; and (c) to the use by Sponsor and its designees of his/her Veterinary Economics 2012 Hospital Design entry, name, company name, trademarks, logos, entry, biographical information, voice, performance, photograph, image, and/or likeness for trade, advertising, publicity, and promotional purposes in any and all media, now or hereafter known, worldwide and on the Internet and in perpetuity without compensation, notification, or permission unless prohibited by law and to execute specific consent to such use if asked to do so. Released Parties are not responsible for late, lost, damaged, delayed, inaccurate, misdirected, incomplete, illegible, undeliverable, destroyed, mutilated, stolen, or postage-due entries or mail or for errors or problems of any kind, whether electronic, network, technical, typographical, printing, human, or otherwise relating to or in connection with this Contest, including, without limitation, errors that may occur in connection with the administration of the Contest, the processing or judging of entries, the announcement of the prizes, or in any Contest-related materials. Released Parties are not responsible for injury or damage to participants' or to any other person's computer related to or resulting from participating in this Contest or downloading materials from or use of the web site. Sponsor may disqualify anyone from participating in the Contest or winning a prize if, in its sole discretion, it determines that such person is attempting to undermine the legitimate operation of the Contest by cheating, deception, or other unfair playing practices or intending to annoy, abuse, threaten, or harass any other entrant or Sponsor’s representatives or who are otherwise in violation of these Official Rules.
