R-TECH INCIDENT REPORT A. DESCRIPTION OF THE INCIDENT: Date : 19 September 2011 Time : 11.30 a.m Location : At T-Junction, Bandar Baru Selayang, Selangor Equipment Involved : Two Car Injury : Syafiqah: Head Bleeding and Broken Leg Adib : Light Injury
Details: Syafiqah was drive a car but the car must service first. The car brake is not grip. He need to drive his car (not service yet) because of pack timetable. When he drive back from work, he get a call. At the junction in the factory, Syafiqah suppose to stop the car. However the break not functions well. At the same time, Adib who drive another car was cross the junction. Because of that, Adib car crash with Syafiqah car. Syafiqah got head bleeding and he faint. Adib only get light injury. Adam who was working nearby heard the accident and ran to the accident area. On seeing Adib and Syafiqah injury, Adam quickly call the ambulance. After 15 minute, the ambulance arrived at that place. Those two victims were taken to the Hospital Kuala Lumpur (HKL).
Dr Anwar gives emergency treatment to both victims. After the medical checkup, Dr Anwar found that both Syafiqah leg was broken and he lost his memory. Adib only get light injury. Syafiqah treated at hospital until his condition stable and Adib can check out next morning. C. CAUSES After an initial investigation, I discovered that Syafiqah break the road law because use hand phone during drive the car and his car not in good condition. He also drive faster in the factory. A further investigation revealed that Syafiqah was guilty under AKTA SEKSYEN 119 (2) AKTA PENGANGKUTAN JALAN 1997 because of dangerous driving. D. RECOMMENDATION To prevent similar occurrences, I suggest the following: 1. Make sure the car in good condition before drive such as be alert for slick tire and else. 2. Don’t use hand phone during driving. If get call, please use hand free or stop the car at the side of the road for a while. 3. Consider equipping a car with air bags. Studies show that air bags alone can reduce the chance of a fatal crash by 20 to 40 percent and when air bag protection is combining with seat belt the risk of a fatal crash reduced from 45 to 55 percent. 4. Don’t drive faster in the factory and recommends driving safely.
