Pada tanggal 18.06.2014 saya
menejemput crew snubbing pada jam
05.10 WITA dirumah Herry T. dan
melanjutkan menjemput crew di kantor
Elnusa Balikpapan : Yusman (Senipah),
Ardiono (Handil), Hendi (Handil). Kami
melanjutkan perjalanan menuju Handil.
Kami tiba di Handil pukul 07.30 WITA.
Pada tanggal 18.06.2014 pukul 08.05
kami menuju Balikpapan bersama crew
snubbing (Syahransyah, Charles, M.
Siko, Syamsudin (crew Slichline). Kami
melaui KM 38 Samboja. Pada jam 09.30
kami diserempet dengan mobil box
truck dengan plat nomor KT 8709 KZ
dari arah berlawanan dan
mengakibatkan laka lantas Kilometer
12 Balikpapan-Samarinda. Mobil
mengalami kerusakan pada bagian
sebelah kanan depan. Kami dalam
keadaan sehat dan aman
On July 24, 2014 At 04.00 PM, During
disconnect dual joint rig’s ladder at
window basket to substructure by
mobile crane, the crew was released
the iron breaching underneath lower
section of the ladder then signal man
gave the order to the crane operator to
continue lifting up the ladder. In the
meantime the lock bolts on the top of

the ladder not yet in free while the
ladder continued to be lifted. Someone
that was standby on top upper section
of ladder is shouting to stop lifting it
but no person that heard, suddenly one
of bolt broken. The Bolt’s part was
thrown and hit a rear view mirror crane.
Resulted the rear view mirror broke out.
No person injury.
Pada tanggal 08 Agustus 2014 IP dari
Balikpapan ke Tanjung Santan ke lokasi
EHR 11 (Tirta Rajawali Barge). Sekitar
pukul 22.00 IP mengeluh pusing dan
memeriksakan dirinya ke dokter yang
ada di lokasi. Keesokan harinya IP
beraktifitas seperti biasa. Tanggal 10
Agustus IP kembali sakit dan meminta
untuk di evakuasi. Dan IP di observasi
di rumah sakit Samarinda. Senin 11
Agustus 2014 IP dirujuk kerumah sakit
Siloam Balikpapan dan diberi
penanganan lanjut. Senin, 18 Agustus
2014 IP meninggal dunia pada pukul
03.30
Pada tanggal 25 September 2014 telah
terjadi kecelakaan kerja yang dialami
oleh Herman
(Mechanic) pada jam 09.00 WITA di
Galangan PT. Masa Kampung Baru
Balikpapan (Project Docking Elsa 1).
Ketika korban akan membuat baut
swivel Hydrolic Hose di bawah pedestal
crane yang berada di atas LCT ELSA-1
yang di bantu oleh Fendi (Crane
Operator), karena tempatnya sempit
maka digunakanlah manhole crane.
Korban menggunakan mainholr yang
berada disebelah kiri crane sedangkan
rekannya menggunakan mainhole
disebelah kanan, untuk mengjangkau
dan naik digunakan tangga lipat. Untuk
membuka baut swivel korban
menggunakan dua kunci satu pas dan
satu kunci pipa karena ada connection

yang terbuat dari pipa. Dikarenakan
posisi korban sangat sempit untuk
menggunkan dua kunci maka korban
dibantu oleh rekannya untuk mebuka
baut, korban memegang kunci pas dan
sementara rekannya memegang kunci
pipa, ketika baut tidak berhasil dibuka
secara tidak sengaja kunci pipa
terjatuh dan mengenai jari tengah
sebelah kanan korban. Untuk kasus ini
kedua pekerja menggunakan PPE yang
lengkap. Dugaan sementara penyebab
terjatuhnya kunci pipa ini dikarenakan
posisi kerja tidak tepat karena
keterbatasan ruang kerja sehingga
kunci pipa terjatuh
On 4 November 2014 at approx. 08.20
pm, during lifting operations for pick up
the Blind Sheap BOP on”KITIWAKE”
AHTS, the load (BOP) hit to the 2”
Quick Connection Hydraulic Return line
of Power Pack Hyd Pump cause Approx.
3 liters of oil sprayed to “KITIWAKE”
AHTS deck. No personal injury nor spill
overboard.

At 05.30 AM, During disconnect drive
head of PCP on wellhead. The IP
position on top deck as signalman.
When attempt to pick up the drive
head and back to slack off. He saw
lifting wire was twisted and then he
attempt to reposition the sling by hand,
without communicate to signalman on
wellhead. Suddenly, signalman on
welldeck gave a signal to crane
operator to pick up the object. At the
time the IP was grabbed the lifiting wire
and his left ring finger pinched between
the lifting wire. Immediately the IP gave
instruction to crane operator for the
slack off the lifting wire and he went to
clinic for medical treatment, then the IP

The IP had ¼ amputation on tip left ring
finger and back to work location.
On 02.00 crew prepare the SSD for RIH
& make up the assembly on top deck
floor before running in hole with
completion string. Worker squatting for
install thread protector onto the SSD
thread & he didn’t notify about oil drum
behind his back/bottom placed on deck
floor. Unintentionally small part & sharp
edge the oil drum caught his bottom &
torned his working FRC/coverall. No
serious injury/wound on his body parts
contributed by this event, near miss
event only
IP ketika hendak memakai Coverall
untuk bagian lengan kiri IP langsung
spontan merasa bahu kirnya ada
sesuatu/bunyi dan langsung merasakan
sakitpada bahu kirinya atau dislokasi
lengan kriri atas. Setelah pergantian
Night crew IP segera dibawa ke Mess
dan menuju RS. Haji Darjad samarinda.
On march, 6 2013 at 16.00 PM, Caing
crew that work on flat top barge beside
Elsa 2 barge has finished the job to
POOH tubing 5 ½ “ at well HWN-1. They
mean to disconnect the tubing that laid
on flat top barge using crane and
elevator and next install the tubing
connection to hydraulic power tong on
well head. Main block crane using to lift
the tubing and then elevator was install
about 70cm from stopper at the end of
tubing using auxiliary block. Main block
then released, after that tubing has
lifted but when on 3m high on top of
the deck and move swing to the right,
elevator was sliding and hit the
stopper, the affected has elevator
opened and tubing falling on the tool
basket. The consequences is the door
of the tool box was bend but no person

Pada tanggal, 11 Juni 2013 hari kamis
pada jam 10.00 di Area Yard/Workshop
Balikpapan, terjadi kecelakaan ketika
salah satu crew melakukan pekerjaan
pengecatan tiang pagar di daerah
Asset Storage #03. Kecelakaan
tersebut terjadi ketika IP turun dari
bangku (1 meter) untuk berpindah
kebagian sebelahnya, tiba-tiba kaki
kanan terpeleset lalu terjatuh
kebelakang dan kepala IP mengenai 41/6” Raiser (2 Ft) yang berada di
belakang sebelah kiri. Terdapat luka
terbuka dibagian kepala belakang (+/1) dan IP mendapatkan 2 jahitan diluka
terbuka tersebut. Setelah itu IP dapat
kembali bekerja kembali seperti semula
Sekitar jam 09.30 pagi, IP bersama
rekan kerja sedang memasang radiator
baru ke unit PE-25 dengan bantuan
forklift. Pada saat radiator sedang di
turunkan oleh forklift namun posisi
radiator tidak pas dengan lubang baut
sehingga IP berusaha mendorong
radiator agar posisinya sejajar dengan
lubang baut. Tiba-tiba radiator
bergerak dan bergeser kearah frame
PE-25 yang mengakibatkan ibu jari
tangan sebelah kanan terjepit diantara
siku radiator dan frame PE-25. IP
langsung menghentikan pekerjaan dan
melaporkan kejadian tersebut ke HSE.
IP dirujuk ke UGD rumah sakit
Pertamina Balikpapan. Luka sobek pada
ibu jari tangan sebelah kanan yang
mendapatkan jahitan dan IP kembali ke
lokasi pekerjaan.
On October 18th, 2013. 20.00 WITA, a
crew in the work basket remove the
shackle type 5/8 Inc-4 ¼ tons from
lifting Sub. While he remove the bolt,
the safety pin and the lifting sub,

suddenly the counter balance swing to
the middle of the basket and touch the
tubing that stick the rotary. The “BOW”
side of the shackle that still attach to
the sling fell down through the jack and
windows and fell down to the deck floor
while people still measure pipe length
for space out job. The”BOW” side of
shackle fell from window to the deck
floor as high as +/- 18.20 ft with weight
200 grams. Fortunately no casualties
and equipments damage caused by the
fall of the shackle.

Pada hari sabtu tanggal, 30 Nop. 13
pukul 07.00 tiba dibarge Elsa 3 job di
TN-I 146, setelah selesai hand over
pekerjaan dengan crew malam, pada
pukul 07.30 IP (Maulana Enginer
Training) mau sarapan, pada saat
sarapan IP hendak mengambil minum
dan pada saat IP mau berdiri tiba-tiba
dia hilang keseimbangan dan terjatuh,
pada saat hendak terjatuh dengan
reflex dia menggunakan tangan kanan
sebagai penahan, karena tangan tidak
kuat menahan beban tubuh dan pernah
ada riwayat Dislokasi sebelumnya,
sehingga terjadi Dislokasi pada tangan
kanan

At the time 03.15 pm had been an
accident of vehicle while Elnusa car
from Client office back to Elnusa office,
at the Sepinggan street Elnusa car (KT
1730 KH) want to “U” turn position &
the Elnusa car was stopped but
suddenly other car from opposite
direction hit to Elnusa car cause front
side (head lamp, Fog lamp, & Bumper)
had been damage
At the time 00.30 pm there was traffic
incident for Elnusa car transportation
for crew change (KT 1729 KH), while
traveling from Sei Meriam to
Balikpapan include with 4 person &
driver at Soekarno-Hatta street km.21,
the car stopped by traffic officer
because the road is damage suddenly
heavy truck (120 ps-KT 8896 BS) from
back side hit or impacted to Elnusa car
at right site cause the Elnusa car got
damage at glass back side & right side
condition is dent
At 07.00 PM, During rig up snubbing
unit on Victim platform, a worker felt
some dust in his right eye while he
handling guy wire to install on the
deck. He then tried to rub it, causing
his eye redden. He went to clinic and
back to work after the doctor clean his
eye with cleaner water
Pre-tour safety meeting/JSA Meeting
prior at 11.30 PM. When RIH mule shoe
with 11/4” Mac Pipe, until 42 ft working
pipe stuck between top tree with raiser
adaptor and then pull out of the work
string to surface and got mark on tip of
mule shoe, immediately report to WSM
and Wireline RIH IB to ensure indication

unsuccessful and then RIH wire grab
two prong to fishing the object and

the dies falling into the well??? As per
information from the crew when
changed old dies size 11/2” to 11/4” on
the upper slip, probability position of
dies not install properly on the reel of
slip, and while RIH test joint for
pressure test of BOP, lower slip and
snub in open position, probability when
running test joint, tip of the test joint
push the dies and the dies falling into
the well. When running test joint not
person stand by at jack basket
On 10th October 2012 at approximately
05.45 HRS on Sea Haven-3 Barge, PK-CI
Platform, a personal injury occurred. IP
(Injured Person) found there is leak
occur on the power pack. IP have
initiative to check and clean the oil
droplets on bottom of body engine.
Unfortunately due to limited space and
wrong position, IP back body touch
hand rail causing his body pushed
forward and his finger touch the fan
blade, cause IP got open wounded
below finger 1,2 3,4 and metacarpal. IP
got treatment from medic barge and
medivac to SNP Clinic. IP then went to
Pertamina Hospital Balikpapan to check
the wounded by rontgen, and no
fracture was found
At 11.30, on November 17, 2012 he
visit to clinic. Illnes person is complain
pain the stomach since noon day. Also
felt bellyful and puke. Immediately
doctor take action for helping him.

causing losss of balance and IP cannot
control his motorcycle it so falls rail on
the road. IP get minor injuries (Scratch)
at his elbow of right hand and elbow of
left foot. Immediately IP continues the
journey to workshop and than report to
Elnusa Safety Dept and sent to AURI
hospital to get first aid and assessment
by doctor
Pada tanggal 30 Juli 2011 hari sabtu
jam 10.00 di area Workshop dan Yard
Balikpapan, terjadi accident ketika
salah satu crew melakukan pekerjaan
six Monthly pada unit wire line.
Accident itu terjadi ketika crew
memasukkan (positioning) 41/2” pipa
lubricator kedalam rak pipa dengan
menggunakan alat bantu Loader. Saat
itu operator loader membantu
mengangkat pipa lubricator untuk
dimasukkan kedalam rak pipa yang
sekaligus menempel pada sisi toolbox.
Ketika korban megangkat pipa
lubricator dengan maksud untuk
diposisikan kedalam paritnya rack,
tanpa disadari dan tidak ada yang
memberikan kode ataupun isyarat
terhadap si operator, tiba-tiba si
operator loader menurunkan beban
sementara posisi tangan si korban
belum siap (tangan korban masih
berada memegang pipa tepat diatas
parit raknya pipa)
Akibatnya jari korban terjepit di rak
pipa lubricator, persis diantara parit
dengan sisi luar pipa lubricator. Korban
kemudian di kirim ke RS Pertamina
Balikpapan.
Pada pukul 17.15 wita, akan dilakukan
high pressure test 41/2” Wireline Raiser
dengan Stripper Bowl, sebelum
pengetesan telah dipasang safety
chain/sling pada bagian atas (control
line) dan pada bagian bawah

(connection antara wireline raiser
adaptor ke stripper bowl), kemudian
pukul 17.20 wita mulai dilakukan
pengetesan dari kondisi pressure 0psi2500psi, holding selama 5 menit dan
aman, kemudian dinaikkan dari
2500psi-5000psi holding selama 5
menit hasilnya aman. Namun pada
pukul 17.30 wita, pada saat pressure
dinaikkan dari 5000psi-7200psi,
connection antara wireline adaptor
dengan stripper bowl tiba-tiba terlepas.
Setelah dilakukan pengecekan,
ditemukan kerusakan dari ulir drat
antara wireline raiser adaptor dengan
stripper bowl mengalami kerusakan.

position was found not in properly set,
then one crew member attempted to
reposition the ladder. Suddenly the
ladder slipped toward inadvertently
and result in the IP right middle finger
tip pinched by the ladder hook. And IP
sent to Attaka Medic for further
treatment to Bontang PKT Hospital
Escorted by Attaka medic and Elnusa
crew.
The incident occurred when the
persons (asset department crew
member) was removed the pressurized
CO2 Bottle (45Kg, 2500 Psi) from top of
the truck that came from Handil
Operation to the bottle rack in the
Balikpapan Yard Area. Crew members
(2 persons) tried to handle the bottle
by working together. One crew member
lifted the lower part of the bottle while
the others lifted at the uuper one ( by
putting his hands at approx. 60cm from
the bottle neck where the sensitive and
unproctected valve is located). When
the crew at the upper one handling the
bottle, he did not realize that his
body/hand was touching the
unproctected valve that causing the
bottle’s pressure was released and
causing the bottle moving around
continuously and resulting in high level
of noise. Fortunately, the wild moving
botltle was then hooked, trapped and
stopped by the ram of the yard’s fence
until it’s pressure release to be zero.
The persons who realized that they are
in danger was escaping before. The
crew member was shock as the impact
of this incident.
At 2.15 am, during r/d 150 unit at
kerindingan p/f (chevron premises) 2
person working at jack assembly. IP sat
and working at jack assembly. IP
disconnect nut (1-1/16” nut) from bolt

and hios team mate on jack assembly
unloose the other nut. The bolt and nut
connect to work basket and jack
assembly. IP using adjustable wrench to
loose the bolt. He was using proper
minimum ppe and safety harness, but
suddenly rain came and IP lowering
downs his safety glasses, he cannot
see clear the object that he wanted to
be loosen. IP grab the adjustable
wrench and nut haven’t bite enough
and he got slipped off. The right hands
of IP punch himself and hot face below
the right eyes. Immediately IP- sent to
attaka quarter to get further medical
treatment and assessment by doctor.

On Thursday January 22th 2009 at
08.00 am, one crew was maintenance
chicksan loop at vice rip (in front of
dog house#2) after finish maintenance
chicksan loop, the IP asked to another
crew to transfer chicksan loop to
lowery & will put on rack in front of
Sclumberger engine. They picked up
chicksan loop to lowery & bring to
chicksan rack. IP side (chicksan) was in
rack while another side tried to put it
down, little finger of IP was hit
between rack & chicksan before put it
down to lower position
A worker was lifting a joint of 3 ½” PH6 tubing so that a pipe sling could be
installed for backloading the tubing.
After the sling was in place, he lowered
the joint and pinched the tip of his ring
finger, on his left hand, in between the
joint and the pipe rack. Tip of finger
lacetared.
LCT. Elsa-1 in positioning mode from
safe area to Tn Q 32/44. While
approaching the captain of LCT didn’t
realized the sea truck still alongside on
portside without driver. Captain of LCT
will move. While approaching to
wellhead the captain realized there is
no driver n sea truck and pull back the
LCT and sea truck hit between LCT and
pilling.
We got material basket to be lift from
vessel to ss1 barge. All lifting job from
vessel to barge or otherwise was taken
by ss1 crew and vessel crew. There is
four oil drum inside the basket need to
be lift and transfer to ss1 main deck.
Suddenly, we found oil basket already
sat on ss1 main deck, but TIW valve

tied off and stand up with rope along
with oil drum basket.
The incident occurred when IP was
changing slip bowls in Work Basket of
Elnusa Snubbing Rig # 225K. slip Bowl
was opened and lifted by two counter
balanced cables as per the SOP to allw
removal of the completion packed
from the well. The IP put is hand in
apinch point so when the slips were
set down on the floor and closed, they
pinched the skin between the thumb
and forefinger. The IP came to the SS-1
clinic to get injury looked by Doctor.
The SS-1 doctor communicated with
Dr. Bayu in the Pasir Ridge Clinic. a
bandage was put on the injury. the IP
has returned to work and is able to
perform his normal duties.
Crew all ready completed POOH
macaroni workstring to surface. After
that crew are preparing rig for slickline
job and assisted by crane. The wire
rope of telescopic ginpole was
attached the crane main block. When
section no 2 of telescopic ginpole is
ready to slack down, IP was squatting
on base jack unit. IP grabbed the wire
rope close to pulley by his left hand
due to unstable position. Suddenly by
intentionally crane operator slack
down the wire tension and telescovic
ginpole was move downward. His left
hand glove was caught and pulled into
bottom of pulley and his left middle
finger wa cut off and tear 1 cm on left
ring finger. After the accident, IP got
first aid treatment on Barge and
brought him to SPU clinic for further
treatment. SPU medic gave wound
dessing to IP and recommended him to
send to RSPB Balikpapan for medical
surgery.

While snubbing crew (ELNUSA) has
already come to mess at 06.30 pm,
suddenly fire occurred from laundry
room beside of mess, fire came from
short connection between electrical
cable above washing machine, after 2
minutes fire could be extinguish by
crew with portable fire extinguisher.

While snubbing crew (ELNUSA) goes to
continues the job, IP found the pantry
was dirty and many croakroach, then
IP go to take insecticide spray type. IP
come to pantry room, spray the
croakroach from inside with exhause
fanturned off and door closed by
himself. After 5 minutes indside,
suddenly he feel sick at is head due his
face

On Saturday, Februari 16,2008 at
09.00 OM, after finished disconnect
and lift by crane of pumping hose on
out rigger p/f, he going back to across
to bottom deck of platform. When he
bow for entering bottom deck, his
lifejacket hook at deck and he pulled
back and falling to the sea. And get
the injury on head, and get the
medical treatment.
In day light we performed “hazard
hunt” to find any substandard tool,
especially home made tools. Along
with WSM and Elnusa safety officer we
looking inside tool box belong to rig
10. We found many home made tools
inside box likes, metal pipe protector
was welded, wrench had improper
modification, etc. after hazard hunt, all
home made tools was junk into metal
waste basket.
On Friday, April 11, 2008, at 02.00 PM,
doing pull oout skid plate at below skid

Tuesday May 6, 2008, at 07.10 AM
Rizal Hamdani and Sugeng Dermawan
were standing on BOP c/w safety
harness have been taken out the bolt,
and Yon Fauzi as Supervisor staying on
deck with radio for communication to
counter balance operator. One bolt
only will taken out. Position left hand
Rizal Hamdani was holding bolt.
Counter balance position on standing
by on lifting plate in up position, but
no pressure. Suddenly the lifting plate
picked up due the counter balance
pressure increase by it self. The little
finger of Rizal Hamdani has impacted
between bolt and lifting plate.
Midnight 00.30 AM, May 7 2008, the
Running Tool Tubing Hanger was
running counter balance. The Running
Tool Tubing Hanger position was in Jack
Basket and attempted to slack through
the hole at lower jack base plate. Due
the space between Lower Jack base
plate and Upper Jack plate too tight,
sometimes Lower Jack base plate and
upper Jack base plate. Yuliansyah as
operator and M Taufik as Roughneck
were on Jack Basket to ensure running
tool can pass throughthe hole at mid
of Lowrr Jack base palte. Yuliansyah
was handling radio at right hand and
his left hand put on top of running tool
where as M Tauifik hold the running
tool at side body. Duie the running tool
sitting on lower Jack Plate, Yuliasnyah
command to counter balance operator
by radio to pick up the counter balace,

but he forget that his right hand was
put on running tool so his left hand
impacted between running tool and
Upper Jack Plate.
On the Wednesday September 3, 2008
at 09.10 AM IP found the counter
balance wire was broken and not fit to
use IP attempted to cut the wire by
using slip dies and brass hammer. IP
use the sharp edges of slip dies as
landing base and put the wire on it and
then he hit with brass hammer.
Suddenly on 3 hit, the hammer was
miss and hit his left hand. He got and
open wounded approximately 1 cm
length.
On Sunday November 9, 2008 at 08.00
am when slickline job in progress,
Elnusa Crew trying to solve the
counter balance problem after finish
solving at 10.30 am Elnusa crew start
the job RIH macaroni tubing, during
perform RIH job from he 1 joint till 48
joints there was no problem. While the
next joint (49) will picked up to work
basket, suddenly pressure increase
from 600 psi to 1500 psi & pipe
uncontrol by counter balance operator
though he trying to release the
pressure. Pipe obstruct the V-Door &
through to gin pole than fall down ht
platform handrail than to the the river.
Wire of counter balance brake off at
lifting sub because the wire hit top of
gin pole.

generator kemudian dia pergi dan
mematikan percikan api tersebut
dengan fire extinguisher.
Saat bekerja di atas Basker BOP untuk
mengganti Ram Block No#1, korban
menekuk tubuhnya selama menahan
dan mengangkat beban untuk
memperbaiki silinder ke BOP tiba-tiba
korban merasa nyeri punggungnya.
Saat korban memperbaiki RAM BLOCK
BOP sambil tersebut seberat 56kg,
tiba-tiba pinggang terasa sakit.

Saat mengangkut barang pump unit
dari truck dengan bobot 4 ton, tibatiba skit baut yang ada pada bahan
crane untuk swing rusak. Baut
tersebut rusak sebanyak 2 ulir yang
menyebabkan crane tidak stabil
Saat truck parker didepan kantor asset
untuk mengantar barang, sebuah
mobil supplier parker dibelakang truck
untuk menyupply barang ke asset.
Ketika truck akan mundur tiba-tiba
menabrak mobil supplier yang
mengakibatkan kaca lampu depan
pecah. Tidak ada korban jiwa dalam
kecelakaan tersebut.
