Demonstration of Understanding
In an effort to demonstrate my current understanding of the health care system, along with my capacity to provide clients with invaluable resources and information, I am providing an outline of some of the issues that I am aware are likely to be of concern to IMDs looking to find work in Canada. This document should not in any way be interpreted as presuming that World Skills lacks an already established, professional, and much more comprehensive set of considerations for intake, assessment, and client-support; it merely aims to serve as an indication of the types of concerns and information I would expect to consider, were I to enter into the position of acting Client Support Worker/Job Developer today, without any direction whatsoever.

Potential Considerations: Preliminary Questions For New Client
*I am aware that depending on French/English language ability, the following would have to be rephrased, simplified, or translated for comprehension. 1. What is the nature of your medical training including: medical school attended, duration of program, date of graduation, area(s) of focus, and, clinical/work experience? 2. Given your background and prior knowledge of the process of obtaining employment in Canada’s healthcare sector, what are your current objectives for employment in terms of occupation, location, further education, and start date? 3. What is your current status as a resident and/or citizen of Canada, and/or what is the status of any application you currently have with Citizenship and Immigration Canada? 4. How are you supporting yourself financially? What sort of funds will you have available for further training, testing, and licensing? 5. What is your current level of competency in English and French. Do you possess any officially recognized certificates? *accompanied by individual language assessment.

Potential Considerations: Licensure and Memberships

Registration with Provincial Regulatory Authority (CPSO, CMQ, CPSNB etc.) Eligibility: • Completion of a medical degree from an accredited Canadian, US, or FAIMER (IMED) recognized school • An acceptable residency (PGT) • LMCC (usually consisting of MCCEE, MCCEQ1, and MCCEQ2) • Certification by the RCPSC or CFPC • One of the alternate “pathways” for IMGs (ie. CPSO’s pathways 2-4) Time Considerations: Approximately two weeks application processing time. See time considerations for MCC, CaRMS, Medical Residencies, etc. Financial Considerations: College Application fee (varies) and annual membership fees once licensed. Other Considerations: Registration with this body his is generally the final major objective of IMDs looking to practice in Canada. CFPC Membership Eligibility: To write the CFPC accreditation examination, one must complete a residency program in one of the 17 recognized Canadian medical schools, or an approved program in the US, Australia, Ireland, or the UK. Time Considerations: Exams are held two times per year, once in fall in once in spring, and require four days to complete. Financial Considerations: General examination Fee is $1,990.

RCPSC Membership Eligibility: Participation in an accredited specialty residency program and completion of specialty-specific training requirements. Time Considerations: Exams are held twice per year, and eligibility assessments take 68 months to process. Financial Considerations: $550 preliminary assessment fee.

Potential Considerations: Post Graduate Training
CaRMS Eligibility: • Degree from FAIMER or LCME recognized school, or school of osteopathic medicine by July 1 of match year • Have written and passed the MCCEE or be scheduled to write the September OR November MCEE for first iteration participation • IMGs must be Canadian Citizens or Permanent Residents • Variety of documents including personal letter and letters of reference (preferably form North American clinical placement) • TOEFL and TSE test scores Time Considerations: Entire process takes about seven months (First iteration: August – March), with about three weeks National Interview Period (late January to mid February). See MCC, TOEFL and TSE, time considerations. Financial Considerations: $92.50 verification fee of, $157.50 Registration fee:. $25.25 for each program application. Travel expenses during interview process. Possible costs associated with obtaining/translating/notarizing documents Other Considerations:





In many provinces, IMGs will compete in a unique stream and/or are required to complete a Return-of-Service Agreement (requiring postings in under-serviced area for period of time) Some subspecialties have their own matching iterations. Medical Residencies

Eligibility: See CaRMS Time Considerations: Medical Residencies or Post Graduate Training (PGT) usually require an average of five years to complete, though it may be shorter or longer depending on degree of focus or specialization. Financial Considerations: While yearly tuition and instrument/course materials costs can range from between $12,000 and $20,000 (outside of Quebec which has much lower fees for residents), Medical Residency Salaries range from $40,000 to $65,000 per year, depending on province and years of residency completed. IMG Programs by Province Usually associated with a province’s medical schools, their main aim is to offer support to IMGs in obtaining and completing residencies within the province. The number of positions available varies. In Ontario, CEHPEA is an arms length organization which offers evaluation and training of IMGs and other healthcare professionals to help them build their portfolio and work towards a residency or employment.

Potential Considerations: Medical Council of Canada Exams
MCCEE Eligibility: participants should have completed a medical degree in order to have sufficient knowledge to complete tests. Time Considerations: Offered during five different 1-2 week periods throughout year. Computer-based exam that takes four hours to complete. Financial Considerations: $1,550 first time applicant fees. Preparation materials or courses for those requiring review or supplemental training. A practice Self Administered Exam is available for $62. Other Considerations: Success in this exam is often the first objective for IMGs

looking to transition into the Canadian healthcare system as a physician. It tests basic medical knowledge including Child Health, Maternal Health, Adult Health, Mental Health, and Population Health and Ethics. MCCQE1 Eligibility: Completion of MCCEE Time Considerations: offered over a number of days in the fall and spring. Duration of exam is one day Financial Considerations: $780 exam fee. Client may require prep course or materials. Other Considerations: May be completed during residency.

MCCQE2 Eligibility: • Successful completion of MCCQE1 • Completion of a minimum 12 months’ postgraduate clinical medical training Time Considerations: Held twice per year (fall and spring) during one or two days. Examination duration is three hours Financial Considerations: $1,950 examination fee. Client may require prep course or materials Other Considerations: completed well into residency period. The test involves a series of clinical station simulations.

Potential Considerations: Internal Programs, Other Applications and Tests
Medical License Bridging Program Eligibility: as outlined on World Skills website and www.imd-ottawa.ca Time Considerations: Approximately 120 hours, including a 60 hour Observership.

Workshops run about four weeks, most weekday afternoons or evenings for three hours. Next two cohorts begin April 4th and August 3rd. Financial Considerations: no fee Further Career Transition Considerations Some IMDs may wish to consider permanent or temporary employment in other areas of healthcare and health education (i.e. clinician scientists and clinician educators, university professors, nursing, pharmacy, PSW or DSW, counselors, researchers) Many options are explored through the Career Transitions Program and on the Ontario Network for Internationally Trained Professionals (ONIP) website. Career Transitions for International Medical Doctors Eligibility: as outlined on World Skills website and www.imd-ottawa.ca Time Considerations: Four month long programs, approximately twice per week, evenings 6-9 pm. Next cohort begins May 2. Financial Considerations: no fee. Language Testing Eligibility: N/A Time Considerations: • TOEFL iBT: few times per year. Next tests in Ottawa are on April 2, 15, 30 • TSE: offered once per month. Financial Considerations: • TOEFL iBT: $150-200 depending on location • TSE: $125
