INVESTIGATIVE REPORT
TITLE: INTERVIEW WITH TRISTAN ROTH
SUMMARY:
On September 30, 2013, Special Agent Matthew Collins conducted an interview with Tristan
Roth at Woodmore High School in Elmore, Ohio. Roth is a sophomore on the Woodmore High
School football team. Roth plays the positions of strong safety and running back. During the
interview, Roth was asked if he had been present and participated in practice on Tuesday
September 10, 2013. He was asked about normal practice drills and drills conducted on
September 10. Roth was asked ifhe had witnessed anyone become injured or sick that day. He
was also asked if there were any hits that day which stuck out in his mind. Roth indicated they
had plenty of water breaks. Roth did not see any hits which stuck out in his mind and did not see
get hit. Roth did see come off the field and Roth asked him ifhe was
okay. told him he was sick.
DETAILS:
On September 30, 2013, Special Agent Matthew Collins conducted an interview with Tristan
Roth at Woodmore High School in Elmore, Ohio. Roth is a sophomore on the Woodmore High
School football team. Roth plays the positions of strong safety and running back. Roth was
asked if he had been at practice on Tuesday, September 10 and he stated yes. Roth stated he was
not participating in practice due to a prior injury. Roth was asked how long practice was that
day. He said it was from 3: 15 p.m. until around 6:00 p.m. When asked how often water breaks
were, Roth said they were just like regular, after every drill.
Roth was asked to describe the type of drills normally done at football practice. Roth stated they
would stretch and then go to NOOSE. Roth described Noose as when QBs would do their three-
step drops and receivers would do routes. Roth, being a running back, said the backs would run
routes with the receivers. After Noose they would go to Pat & Go again, described as route
running. From Pat & Go they would do Individual Offense. During this running backs would
work on agility drills. Then they would go to a 7-on-7 drill with receivers, running backs and
quarterbacks versus cornerbacks, safeties and linebackers. After that they wouid go to Team
Offense (first team offense vs. second team defense). Then they would switch into defense and

Interview with Tristan Roth
start doing team tackle and team takeaway. Roth was asked to describe team takeaway. Roth
said it was working on getting turnovers (tip drill and strip drill). Roth described the strip drill to
be nothing more than a player holding the ball and a defender wrapping him up with a second
defender trying to pry/pull the ball loose from the ball carrier. Later, Roth described team
tackling drills as having a ball carrier and a defender and they would walk at an angle for pursuit
tackling. Roth described another tackling drill where they would stand about five yards. apart
and the defender would tackle them onto a large black pad (does not know what the name of the
drill is). After that they would go to Individual defense and then onto team defense (first team
defense vs. scout team offense).
Roth was asked how the drills went on the September 10. Roth said he was not there for the first
hour of practice because he was inside icing and had gone outside to run then returned to the ·
locker room to ice again. When Roth went out to practice it looked like they were doing first
team offense versus scout team defense. Roth was asked what they did after team offense and he
said he did not know because he went into the locker room with Roth said he heard
Maliki hit him but when asked if he actually saw the hit, Roth stated no and that he had only
heard that Maliki hit him really hard. Roth was asked if he recalled who had said this and he
stated no. Roth said came over and was standing next to him. Roth asked if he
was alright and said he was sick. Roth knew was sick the day before because he
was not at school. then lied down and Roth told him he might get in trouble.
said, "No, it's fine they know I'm sick." Roth said came over and made sure
was alright. Roth heard say his head and neck hurt. told him to
take off his helmet and his pads. Roth said Mason Beachler took inside. They told him
to go into the trainer's room and put a cold towel on his head. Roth said Mason was the only one
with as they walked to the locker room. Roth said he was not there for any of the
conversations between the trainer and
Roth said he did not see any hits in practice that stuck out in his mind. Roth was not present
during the drill. Immediately after the drill, players were sent in to get changed and Roth was
either icing his injury or running on the track. Roth said he saw prior to practice and he
appeared to be normal.

SUMMARY:
On September 30,2013, Special Agent Matthew Collins conducted an interview with Evan
Ulinski at Woodmore High School in Elmore, Ohio. Ulinski is a senior on the Woodmore High
School football team. Ulinski plays the positions of cornerback and wide receiver. During the

asked ifhe had been at practice on Tuesday, September 10,2013 and he stated yes. He further
stated that he participated in practice as well. Ulinski was asked how long practice was that day
and he stated it was two and a half hours. He said he knew they started at 3: 15 p.m. but does not
know when they finished.
Ulinski said they probably got five to six water breaks and thinks they were about every 20
minutes. Ulinski was asked to describe the types of normal drills that are conducted at practice.
Ulinski said they start off with NOOSE or catching drills, form tackling drills (shedding blocks),
individual drills (offense/defense), team defense where they scrimmage against scout offense.
Ulinski was asked to describe form tackling drills. Ulinski described how they were on a knee
and take a step up and "throw your bowling balls," or wrap up locking your arms behind them
and drive them back. He described a drill where they would get rid of or shed a defender and
tackle the ball carrier. There is also a drill where a ball carrier just stands there and a defender,
who is five yards away tackles him onto a pad.

Interview with Evan Ulinski
Ulinski was asked to run SA Collins through the events on Tuesday, September 1 0, 2013.
Ulinski said he was not going to be able to give SA Collins the exact order of events but did
know they started off with NOOSE. SA Collins asked him to describe what happened on that
day the best he could. Ulinski said they were doing team offense and the coaches were not happy
with their level of intensity. He said they were not in full pads that day. had them
go inside the locker room and put on full pads. He said the kids who were on time were the ones
who hit in the third drill that SA Collins had previously written down with the exception of there
not being a pad used. When SA Collins asked him to describe the distance, Ulinski said he just
knows it was five yards from the lines on the field. Ulinski was asked about there being lines on
the practice field and he claimed there were lines on the field that day.
Ulinski said he was one of the late ones. Ulinski was asked what instructions he was given once
he came out of the locker room. He said he was told to get on the line. Ulinski said because he
was late, the players who were on time were going to hit him or tackle him. Ulinski said he was
not told anything else. He further stated he was really late so he did not hear what they were
supposed to be doing other than getting hit. Ulinski said he just stood there and got hit like any
other time. Ulinski was asked ifhe had been told he had done it incorrectly and he stated no.
Ulinski said he could not remember who tackled him. Ulinski was asked if it was a hard hit
since he cannot remember and he said no, he just cannot remember. Ulinski was asked if there
were any hits during practice that stand out in his mind and he stated no. Ulinski was asked if he
had seen take any hits and he said no. Ulinski said he saw lying on the ground
later in practice. Ulinski said they were at a water break when he saw him lying down. Ulinski
did not hear say anything or anything being said to him. Ulinski said one of the coaches
was attending to him but Ulinski went back to practicing. Ulinski said he did not see
after practice either. Ulinski was asked if he had seen prior to practice or in school that
day and he said yes, in school. Ulinski said when he saw him in the hallway during school he
seemed fine to him.
Ulinski was asked was guarding him while Ulinski was on Team Offense and he stated
no. Ulinski was asked if he saw anyone else at practice who appeared to be sick or injured and
he said he did not think so.
Later, after the interview with Ulinski, SA Collins reviewed what Ulinski had stated to him
versus what Ulinski had stated to Principle Kieper. There was one significant difference as it
pertained to the distance the kids were at during New Woodmore/Old Woodmore. In Kieper's
notes it indicates Ulinski said they were ten yards apart, while during the interview with SA
Collins, Ulinski was confident it was five yards due to there being lines on the field (which was
also the first time SA Collins had heard there were lines on the field and no other player
indicated this to SA Collins).

On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
John Williams. The interview was conducted at Woodmore High Schoo1located at 633 Fremont

On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
John Williams. The interview was conducted at Woodmore High School located at 633 Fremont
Street, Elmore, Ohio.
Williams stated he was present at football practice on September 1 0, 2013. Williams stated
practice started around 1515 hours and practice lasted for approximately two hours.
Due to the fact that September 10, 2013, was a hot day, the team was allowed more water breaks
than usual. Williams did not know the exact number of water breaks.
Williams explained that typical drills conducted at practice consist of individual offense, team
pass under pressure, team run game, individual defense, team takeaway, team tackle, and team
defense.
S/ A Momchilov asked Williams what kind of tackling drills are usually performed during
practice. Williams stated a position tackling drill called bowling balls is typically performed.
Williams advised that bowling ball is not run at full speed, but there is a drill that is similar to
bowling balls that is run at full speed. Williams stated that the bowling ball full speed drill has a
pad behind the person getting tackled and is usually set up about five feet apart. The person
getting tackled is holding a ball with the other arm down to the side.

INTERVIEW OF JOHN WILLIAMS
S/ A Momchilov asked Williams if the bowling ball tackling drill was performed on September
10, 2013. Williams stated no, but a drill similar to the bowling ball drill was conducted.
Williams explained that because it was so hot out, the team was dressed in "thud" which consists
of shoulder pads, helmet, and a girdle. During practice the team was instructed to go and change
into full pads. Williams stated the team was given eight minutes to get changed into full gear.
Williams stated on eight or nine players arrived back within the eight minute time frame. The
players who were late were instructed to line up and the players who arrived on time were
instructed by to hit them.
Williams stated this was not a typical drill that was performed during practice, but stated it was
similar to the bowling ball drill without the pad. Williams could not state exactly what was said,
but stated that the players who were late were classified as "old" Woodmore and the players who
were on time were classified as "new" W oodmore. Williams stated that the coach said teach the
"old" Woodmore something. According to Williams, they were not instructed to go full speed,
only to tackle the other player. After the hit, the players were instructed to run to the cornfield.
Williams stated he did not see get hit during this drill.
S/A Momchilov asked Williams if he observed any hits during practice that stood out in his
mind. Williams stated, "Maybe during scout team, but I can't remember."
S/A Momchilov asked Williams ifhe remembered anyone getting hurt during practice. Williams
stated that he remembers seeing hurt during scout team. Williams then advised that he
did not think was hurt because was lying down with his helmet off and felt that
he was only overheated. Williams stated he also thought maybe got hit and was dizzy
due to the heat.
S/A Momchilov asked if anyone asked about welfare. Williams stated that other
players were standing by and talking to him. Williams stated that walked
by and asked if was okay. Someone told that they thought
overheated.
Williams stated he thought went to the trainer, but could not remember exactly.
Williams did state that was not checked out on the field because he thought
walked back to the trainer's room on his own. S/ A Momchilov recorded the interview with
Williams. A CD of the interview was made and placed into the exhibit portion of the case file
labeled as Exhibit 3.

SUMMARY:
On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
Evan Sandwisch. The interview was conducted at Woodmore High School located at 633
Fremont Street, Elmore, Ohio.
DETAILS:
On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
Evan Sandwisch. Sandwisch stated he was at practice on September 10, 2013 and practice started
at 1515 hours and lasted until 1830 hours.
Sandwisch stated that the team had a water break after every drill. Typical football practice drills
consist of practicing catching, releases, and pat and go. Typical tackling drills consist of form
tackling drills called bowling balls. There are two types of bowling ball drills, one onto a pad and
the other where a pad is not present. When bowling ball is conducted without a pad the tackler
only lifts the other player up and does not take them to the ground. The bowling ball drill onto
the pad is run at full speed and the tackler is approximately six or seven yards away from the
player that is being tackled. The person being tackled keeps their arms to the side and is
defenseless.
On September 1 0, 2013, Sandwisch stated that the team was dressed in three quarter pads which
consisted of a helmet, shoulder pads, and a girdle. Drills performed during practice consisted of

ball drill, but the pad was not placed on the ground behind the person being tackled.
Sandwisch stated he did not notice any big hits during this drill.

On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
Bradley Bringman. The interview was conducted at Woodmore High School located at 633
Fremont Street, Elmore, Ohio.
DETAILS:
On September 30, 2013, S/A Momchilow spoke with Woodmore High School football player
Bradley Bringman. Bringman was present for football practice on September 10, 2013.
Bringman stated practice started at 1515 hours and ended around 1830 hours.
Bringman stated when it is hot outside the team gets more water breaks than usual. Bringman
advised it seemed as if they were getting water breaks every five to ten minutes.
Bringman explained that typical practice drills consist of wide receiver drills. In regard to
tackling drills, typical drills are fit tackling, shed drills, shuffle drills, and bowling balls.
Bringman described the bowling ball drill as one player standing and the other player is on one
knee. When the coach blows the whistle, the player on one knee tackles the player who is
standing. The drill is not run at full speed.
There is also a variation of the bowling ball drill in which one player is standing in front of a pad
and the other player is approximately four or five yards away. The player that is four or five
yards away will tackle the player who is standing in front of the pad. Bringman stated this drill is
performed at half to three quarters speed.
Bringman explained on September 10, the football team was dressed in helmets, shoulder pads
and maybe a girdle underneath their shorts. All the normal drills were being conducted. During
practice the team was being lazy and instructed the team to go put their full gear
on. allowed the team eight minutes to go and get dressed.

INTERVIEW OF BRADLEY BRINGMAN
Bringman stated that when the team returned to the practice field, the team did a quick, intense
version of the bowling ball drill. Bringman explained that the players who arrived back within
the eight minutes were called "new" Woodmore and the players who did not return back in time
were called "old" Woodmore. Bringman stated that "olds" punishment was to run after the drill.
"Old" Woodmore and "new" Woodmore were separated during the bowling ball drill. "Old" was
lined up approximately five to six yards away from "new." "New" was then instructed to tackle
the "old" Woodmore.
Bringman stated he did not remember seeing any big hits during practice nor did he see anyone
sick or injured during practice. However, Bringman did state he observed lay
down during the very end of practice during team defense. was lying down behind the
scout team offense. S/ A Momchilov asked Bringman if anyone checked on as he was
lying down. Bringman explained that he thought the trainer was out there next to S/ A

SA Emahiser asked Williams ifhe was present for practice on September 10, 2013, and he
advised that he was. Williams advised that practice started at 3:15p.m. and ended around 6:30
p.m. Williams advised September 10 was a very hot day and the players received regular water
breaks throughout practice. Williams said that practice began with simple warm up drills to start
the day. Williams stated that he felt was out of control and was similar to a
drill instructor when he ran practice.
Williams said that on September 10, practice was not going the way coaches wanted.

that he took his time changing and he and a few other players came out much later than the

Interview with player Todd Williams
Williams said that upon arriving to the practice field, was upset because the
quarterback, Jake, had his game pants on and was not supposed to. Williams said that
grabbed him, Tony Rozzi, and Jake Matwiezak by the jersey, in the shoulder pad area
and shoved them. Williams stated that then told them that they were "old
Woodmore" and that they were going to get what the other players got. Williams stated that
told them to just stand there and gave them no instruction on how to stand.
Williams said that blew the whistle and John Morris tackled him. Williams
advised that as soon as he got up tackled him. Williams said that during the
tackling, Tony Rozzi and Dustin Haar started to scuffle, and the two players were broken apart
by Williams said that scolded Tony Rozzi for fighting with a
teammate. Williams said after that they ran to the cornfield. Williams said all of the coaches
were standing in the immediate area of the tackling drill. Williams stated the drill ended and
they moved on to the "bull in the ring" drill.
Williams said that during the bull in the ring drill, there are no rules when trying to get past the
defender to the ball carrier. Williams said that the bull in the ring is a no-hold, barred type of
drill, and that coaches have allowed wrestling moves. In the past, Williams has seen punches
thrown in the drill. Williams said that he did not see anyone take any serious hits or appear to be
injured. Williams said that practice went on, and he never saw anyone injured or acting ill at
practice. Williams also specified that he never saw until the end of practice
when Zach Sandwich was walking to the trainer's office. After seeing being
escorted to the trainer's room, Williams said that he did not see the rest of practice.
Williams said that he did see the athletic director talking to during practice, but he
was not near them to hear the conversation.

On 9/30/2013, at approximately 1030 hours, Special Agent Najmulski spoke with Jeremy Cuevas
to ascertain what he witnessed at Woodmore High School's football practice on September 10,
2013 when was injured. The interview was recorded. The following is a
summary of the interview.
DETAILS:
Jeremy advised he plays football for the Woodmore High School football team. He said he was
at practice on September 10,2013. Jeremy said practice started around 1515 hours and lasted
until 1830-1900 hours. Jeremy said he felt that the players were given enough water breaks due
the hot weather.
Jeremy said the players were told to put on full pads because they had a really bad practice.
Everyone was hot and not taking practice seriously. He said they had eight minutes to dress. The
kids who took too long were the "old Woodmore" and the kids out on time were the "new
Woodmore." Jeremy said he was "old Woodmore." Jeremy said they were separated into two
different lines. He estimated an average of seven yards between each line. Jeremy said some of
the line was closer and some of the line was farther apart. He thought the farthest player was ten
yards apart at the most. He said they were told to just stand there and not move when they got
hit. He said he never heard to put his hands behind his back, but some of the kids were telling
him that.
Jeremy said no one appeared to be hurt after that. He said they went into a "scout drill" and that
is when came out because he did not feel good. He said he did not know which drill had
injured because they had done multiple drills after the new/old Woodmore drill. Jeremy
never talked to He saw a group of kids around while he was drinking water. He
continued to play after that. He said everyone thought he was dehydrated because of the heat.

Interview with Jeremy Cuevas
Jeremy thought was the first coach to check on but soon after some
other coaches came over as well. Jeremy said he saw the trainer come out to the field and check
on however in a later interview with the told Najmulski
he never came out to the field. No other witnesses told Najmulski they saw the trainer come out
to the field. Jeremy said he could not hear what said to the first time (on the field),
but continued to play after getting a drink of water.
Later when went to the training room, Jeremy said he was there and heard say
should go to the hospital. He said said he might have a concussion. Jeremy said he
was standing in the doorway of the training room when he heard say this. Jeremy thought it
was at least half an hour from the time sat down on the field to when he went to the
trainer's room.
Jeremy said he saw the athletic director talking to but he did not hear the
conversation. Jeremy said later said the athletic director wanted to make sure the
kids were getting plenty of water breaks.
Jeremy said some of the players, not coaches, were talking about how they were not going to talk
about it and leave details amongst themselves. Jeremy said someone told Mr. Keiper, the
principal, and then everyone started talking. He said that was something the majority of the
players had all decided while in the locker room.

following is a summary of the interview.
DETAILS:
Connor advised he plays on the junior varsity football team for Woodmore High School. He said
he was at practice on September 10, 2013. Connor said practice started around 1515 hours and
lasted until 1800-1830 hours.
Connor said on September 1 0, the team started out in what they call a shell. This consists of

the linemen were not doing tasks and routes were not run well. Skyler said he participated in all
the normal drills, but he did not do the one "bad drill." Najmulski asked him to explain.
Skyler said the players were told to go get full pads on. He said that said they had
eight minutes to get their full pads on. Skyler said he was really late getting his on. By the time
he got out to the field, the other players were already lined up. The kids who were late all had
their hands behind their back. He said this was not normal procedure for drills. The kids that got
there on time were hitting the other kids. Skyler said he was so late he did not get in the line to
get hit and only had to run to the cornfield.
Skyler said as he was running out to the field, he saw get hit. He said head
backlashed. He described this as s head hitting the ground and then bouncing on the
ground again. Skyler said got up slowly, but continued practice.

Interview with Skyler Brown
Later in practice, Skyler saw lay down and hold onto his head. Skyler had not seen what
was doing before that. Multiple players came to see if was ok. One of the players,
Zach Sandwisch, yelled out to the coaches that a player was down. Skyler said several of the
coaches came over to check on told some of the players to help him to the
locker room. Joe O'Connor and Tristan Roth helped to go see the trainer. Skyler did not
see after that.
Najmulski asked if there had ever been a discussion about loyalty to the team being mentioned in
reference to incident. Skyler said yes. He said it came from both players and coaches.
He said he heard this "here and there." When Najmulski inquired about more detail, Skyler said
every Thursday the coaches put out papers and talk about being family to everyone. He said that
is the only time loyalty is talked about.

TITLE: Interview with Ross Wolford
SUMMARY:
On October 1, 2013, SA Ryan Emahiser met with Ross Wolford, junior at Woodmore High
School and member of the Woodmore High School football team. SA Emahiser spoke with
Wolford at Penta Career Center. The interview was in regard to the allegations occurring at
football practice on September 10, 2013.
DETAILS:
On October 1, 2013, SA Ryan Emahiser met with Ross Wolford, junior at Woodmore High
School and member of the Woodmore High School football team. SA Emahiser spoke with
Wolford at Penta Career Center. The interview was in regard to the allegations occurring at
football practice on September 10, 2013. Wolford was asked the same battery of questions that
were asked to all other football players. Wolford's mother, Beth Wolford, was present for the
interview.
SA Emahiser asked Wolford ifhe was present for practice on September 10, 2013, and he
advised that he was present. Wolford said that practice started around 3:15 p.m., and due to his
attending Penta Career Center, he arrived around 3:30p.m. Wolford said practice that day lasted
until around 6:30 p.m., and that the players had regular water breaks throughout the practice.
Wolford explained to SA Emahiser the usual practice drills that are conducted at practice on a
normal basis.
SA Emahiser asked Wolford about practice specifically on September 10, and asked what events
occurred after warm up drills. Wolford stated that the coaches were not happy with the way
practice was going, and players were ordered by to go in to the locker room and
change into full gear. Wolford said players were only given eight minutes to change and be back
on the practice field. Wolford said he was only thirty seconds late arriving back to the practice
field, and when he arrived the players who were late were separated from the on time players.
Wolford stated the players who were late were called "old Woodmore" and the players that
arrived on time were called "new Woodmore." Wolford estimated that the "new Woodmore"

Interview with player Ross Wolford
consisted of approximately fifteen players, and "old Woodmore" was the rest of the team, which
was about thirty players.
Wolford stated that separated the players into two groups, and told the players
to line up. Wolford said that pushed a "new Woodmore" player toward him
and said "hit 'em." Wolford said that his hands were at his sides, and that he never received any
instructions on how to stand in the "old Woodmore" line. Wolford stated that after they were hit
one time, the "old Woodmore" players ran to the cornfield. Wolford said that there did not
appear to be any excessive tackling, aside from two players that were going at it, tackling each
other multiple times.
Wolford said next the team did the "bull in the ring drill." Wolford played the defensive player
who tried to get the ball carrier. Wolford stated that he was up against Tony Rozzi, and that
Rozzi knocked his helmet off. Wolford stated that play was stopped during the drill, and he was
allowed to put his helmet back on. Wolford said that when his helmet came off a second time,
the drill was ended. Wolford said that he was not injured, and the climate of the drill was not
abnormal from any other practice.
SA Emahiser asked Wolford if anyone took any serious hits, and he advised that nothing stood
out to him. Wolford said that he never observed taking any serious hits.
Wolford said that he saw throughout practice talking and he seemed normal. Wolford
said he never saw lying on the ground or become injured in any way. SA Emahiser
asked Wolford ifhe believed that the tackling for practice on September 10 was out ofhand and
he advised that it was a little out of hand because ofhow hard everyone was hitting. SA
Emahiser asked Wolford if he believed any coaches would intentionally cause any harm to the
players and he did not believe that they would.

Penta Career Center. The interview was in regard to the allegations occurring at football practice
on September 1 0, 2013. Morris was asked the same battery of questions that were asked to all
other football players. Morris' mother Jennifer Anderson-Morris was present for the interview.
Morris stated that he was present for practice on September 1 0 and that practice was from around
3:15 p.m. to 6:30 p.m. Morris said that day was very hot and players were given regular water
breaks. Morris said he plays offensive tackle and defensive end on the football team, and that
Tuesdays are normally reserved for tackling drills. Morris said that on September 10,
told the players to go inside and change into full pads, and that the players had eight
minutes to get back out on the practice field. Morris stated that he went in to change and made it
back out to the practice field within the eight minutes.
Morris said told the players who came out from changing on time to get in a
line and the players arriving late to form a separate line. Morris said that there were
approximately six players that were on time, and the remainder of the team was late. Morris
stated that said, "This is what happens when you let your teammates down and
you're not doing what you're supposed to be doing." Morris said that grabbed
a player and physically pointed him toward one of the late players and said "hit 'em." Morris
said that did not give any instructions to the players that were late, also known
as the "old Woodmore" players. Morris said that he was the second player to tackle one of the

Interview with player John Morris
late players, and he advised that tapped him on the shoulder and told him "hit
'em." Morris said the tackling was a wrap up tackle that followed through to the ground.
Following the tackle, the tacklees would run to the cornfield. Morris said that he first tackled
Ben Wilt, a freshman, and tackled a total of about five other players. Morris said that they had
never done the drill in this manner. Usually the drill was done in a circuit, with fewer players and
the players were tackled into a pad. Morris said the tackling portion was the exact same format
used in the tackling drill during circuit training.
Morris stated that while the late players were only tackled once, the on time players went down
the line and continued to make multiple tackles. Morris said that all of the late players were
tackled to their backs in a wrap up tackle, except for some of the bigger players. Morris advised
that the drill began with players standing about five yards apart and eventually increased to about
ten yards. Morris said that he did not recall any serious hits sustained by any players that could
have caused an injury. Morris stated t h   ~ there were some players that put their hands behind
their backs, some players had their hands at their sides, and other players took a defensive stance.
Morris said at no time was anyone ever instructed to put their hands behind their backs, and the
players that did put their hands behind their backs most likely did it out of confusion.
Morris said that the only players who were excessive in the tackling were Tony Rozzi and Dustin
Haar, who were not present for the directions of the drill. Morris stated at one point, Rozzi and
Haar were separated by the coaches because their tackling was so excessive. Morris said that
after the tackling drill, they did the "bull in the ring drill," and he did not recall any hits that
stood out to him as being excessive or having caused an injury.
Morris said that after the two drills, they did team offense and team defense. Morris stated he
heard someone yell that was lying on the ground. SA Emahiser asked Morris if
he ever saw take any hard hits and he advised that he did not. Morris said that
was lying on the ground behind some players and , who was running the offense.
Morris said that he did not see until the end of practice when he walked by the trainer's
room. Morris stated that he went into the trainer's room to see and he was lying on the
table and he looked dehydrated. Morris said that the trainer was not in the room at the time, and
he may have been trying to call parents or talking to the coaches about Morris
said that had a towel over his face and did not say anything to him. Morris advised that
he did not have any other interactions with the rest of the evening.

interview.
Rozzi said that on September 10 the players did a hitting drill to get them fired up. Rozzi said
prior to that, was frustrated with the performance Of players, and told them they
were being lazy. Rozzi stated that gave players around ten minutes to go into the
locker room to change into full pads. Rozzi said that only around ten players made it back out
onto the practice field on time, and the remaining players were late. Rozzi said that he was later
than most players getting out to the field. By the time he got to the field, the hitting drill was
already in progress and most of the players had already run after being hit. Rozzi said that when
he arrived to the drill, he and Dustin Haar hit each other between two to three times and they ran
to the cornfield like everyone else had. Rozzi said that Dustin Haar was supposed to tackle him,
but he retained a defensive position and "laid him out."
Rozzi said that upon arriving at the drill, he heard no instructions because he was late and he and
Dustin Haar just hit each other. Rozzi stated that when he did the drill there were no coaches
running it, because the drill was basically over. Rozzi said that after the tackling drill, the whole

Interview with player Antonio Rozzi
team did the "bull in the ring drill." Rozzi said that they had done both drills for years, including
this year.
Rozzi stated that throughout practice he did not see anyone take any significant hits. Rozzi said
that the only player that he saw who may have been hurt or injured was who he
saw lying on the ground. Rozzi said that he saw check on Rozzi said that
it looked like one of the coaches pointed toward water and a player brought something to
drink. Rozzi said that the rest of the team went on with practice and he did not see again.
SA Emahiser asked Rozzi if he knew when appeared to be injured and he advised that it
was toward the end of practice, approximately around 6:00 p.m.
Rozzi said that he saw make and receive tackles during practice, but nothing stood out to
him as being the cause for injury.

TITLE: INTERVIEW OF CLARK JIVIDEN
SUMMARY:
On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
Clark Jividen. The interview was conducted at Woodmore High School located at 633 Fremont
Street, Elmore, Ohio.
DETAILS:
On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player
Clark Jividen. Jividen stated he was present at football practice on September 10, 2013. Jividen
stated that football practice started around 1515 hours. Jividen stated he left practice early on
September 10, 2013 to attend an Eagle Scout meeting. Jividen stated that he was present for at
least half of practice.
Jividen stated that the football team had more water breaks than normal, but could not give an
exact number due to the fact that he was not present for the whole practice.
Practice drills usually consist of tip drills, knuckles in the grass, and "practice on throwing
bowling balls." S/A Momchilov asked Jividen to elaborate more on the bowling ball drills.
Jividen explained that one person will be on offense and one person will be on defense. The
person on offense will push the defender out and then kind of slide and throw their arms up.
Jividen advised the purpose of the drill is to practice on tackling form. Jividen stated that this
drill is not run at full speed but when players are wearing full pads they are supposed to take the
offensive player to the ground.
S/ A Momchilov asked if there is usually a pad on the ground during the bowling ball drill.
Jividen explained that there is not. S/A Momchilov asked ifthere is a drill that uses a pad that is
placed on the ground. Jividen explained that there is a drill that uses a pad. During that drill, a
player is standing in front of the pad and a tackler runs at full speed and tackles the player onto
the pad.

INTERVIEW OF CLARK JIVIDEN
On September 10, 2013, Jividen stated that typical drills were being conducted that day. S/A
Momchilov then asked Jividen about "new" and ''old." Jividen stated that asked
the team if they wanted to be the "old" Woodmore or the "new" Woodmore. Jividen explained
that stressed that "old" Woodmore did not work as hard and did not have a
good season. To be the "new" Woodmore, stated that the team needs to
improve and discipline themselves more.
During practice, gave the team eight minutes to go back into the locker room to
get changed into full practice gear. Jividen stated that only about half of the team made it back to
the practice field within the eight minute time frame. Jividen advised that is what started the
"old" and "new" Woodmore.
then instructed the players who did not make it back in time to stand in line.
The players who did make it back in time were instructed to stand in a different line. Jividen
stated that players were not matched up with each other according to size and it was completely
random. was yelling to go full on the players who arrived back within the eight
minutes and tackle the players who did not arrive back within the eight minutes. Jividen
explained the players who did not make it back in time were to stand there with their hands
behind their back or to their side and not move. Jividen explained that the tackler was
approximately six to seven yards away from the player being tackled.
Jividen explained that this was not a typical drill and has never been conducted before. Jividen
felt it was done for disciplinary reasons.
S/ A Momchilov asked Jividen if he remembered any players getting hit hard. Jividen stated that
he got hit pretty hard and remembers telling "new" Woodmore to go as hard as
they could.
Jividen stated he did not remember getting hurt during this drill.
After this drill the players were instructed to run to the cornfield. Jividen left practice early and
did not see hurt or lying on the field during practice. S/ A Momchilov recorded the
interview with Jividen. A CD of the interview was made and placed into the exhibit portion of
the case file labeled as Exhibit 3.

TITLE: Interview of Henry Flores
SUMMARY:
On September 30, 2013, SA Jon Lieber interviewed Henry Flores. The purpose of the interview
was to gather information regarding Henry's knowledge of the treatment ofWoodmore football
players and the injury
DETAILS:
On September 30,2013, at approximately 1034 hours, SA Jon Lieber interviewed Henry Flores
at the Woodmore High School located at 633 Fremont Street, Elmore, Ohio 43416.
Henry stated that he was present at the football practice held on September 10. The practice
began at approximately 1515 hours and ended at approximately 1800 hours. Henry stated that it
was hot and humid that day and in his opinion the players were given more than an average
number of water breaks. SA Lieber asked Henry what types of drills are normally conducted
during football practice. These drills are described below.
• Team Tackle- Consists of four separate stages.
1. Two players. One player stands in front of a pad that is lying on the ground. The
second player tackles the first player onto the pad.
2. Two players. Player one starts on his knees and goes through the tackling motions on
player two, but does not take player two to the ground.
3. Three players. Player one is the ball carrier, player two is an offensive linemen,
player three is a defensemen. The object of the drill is for player three (defensemen)
to tackle player one (ball carrier).
4. Tackling sled. The tackling sled consists of tackling dummies that the players hit in
an effort to move the sled in a backward direction.
Henry stated that only the three player drill described above was conducted during the September
10 practice. Henry further stated that the players were told to change into full pads within eight
minutes. Only about eight players made it back to the practice field in the time allotted. Coach

then told the players that were late that they were "old Woodmore" and that they
would stand in a line while "new Woodmore," the players that made it back in time, would hit
them. The distance between the players was estimated to be five feet. did not
tell the players that they could not defend themselves. Henry stated that he "fought back." The
old Woodmore players were hit one time each and then told to "run the com." Running the com
requires the players to run to an adjoining cornfield and back.
Aft en running the com, the team conducted a team tackle three player drill (drill #3 on page one
of this report). After this drill Henry noticed that was injured and lying on the ground.
Other players checked on and then notified one of the coaches. The coach then had
taken to the trainer's room.
The next day, September 11, Henry heard from brother, that
could barely walk to the car after practice.

TITLE: INTERVIEW- JACOB M. MATWIEJCZWK
SUMMARY:
At 1055 hours, Special Agent Thomas Brokamp met with Jacob M. Matwiejczwk at the
Woodmore High School in Elmore, Ohio. The purpose of the meeting was to discuss with
Matwiejczwk events that had occurred in football practice on September 10,2013.
DETAILS:
Brokamp explained the nature of the inteniiew to Matwiejczwk. He said that he is a senior in
high school and is the starting quarterback for the Woodmore High School football team. He said
that he was at practice that day. He said that it started out as a normal practice. He said that they
were sloppy and were not having a good practice. He said that the coaches told them that they
had eight (8) minutes to get in and change into their practice pants and get back on the field.
Jacob said that it took him about thirteen (13) minutes to get back because he could not find his
practice pants.
Jacob said that he and a couple other guys were walking out and saw the lines of people getting
hit by other players. He said that he had to get hit once. Brokamp asked Jacob ifhe felt that there
was anything that was out of bounds. Jacob said that in most drills you are allowed to protect
yourself. In that drill they told the players not to protect themselves. When asked about
protecting themselves, Jacob said that the coaches told them that they were not to defend
themselves, but just to stand there. He did not see anyone get hit really hard.
The normal practice followed and they went over offense and defense. They had plenty of water
during the practice. Jacob had no idea that was hurt until after the practice. He did not see
him wandering around during practice. The interview was concluded at 11 05 hours.

separated into the late players and players that came out on time. said that
called the players that returned to the practice field on time "new Woodmore" and the late
players "old Woodmore." "Old Woodmore" did not meet expectations.
said that instructions to the on time players were to tackle the late
players. stated that he was not sure of exact instructions, but it was
something to the effect of"put a hit on 'em,"' or "let's hit." also said that did
tell the players to "throw bowling balls," which referred to a tackling drill that they normally
performed in practice. said the players were approximately four to five yards apart from
each other. said that no one was ever told to put their hands behind their back, or stand
defenseless.

said that there were at least three to four coaches present when was
running the first drill after the players changed. advised that he never saw anything that
would be considered malice, or extracurricular to football technique. said that during this
drill, they were still performing fundamentals of football. said that
drill was very short lived, and that only one side did the tackling.
said after the tackling drill, players went to the "bull in the ring" drill. stated that
he did not see anyone take any serious hits, or appear to be injured throughout the drills.
said that after both tackling drills, did not show any signs of being ill or injured
that he observed. stated that he did not have any interactions with for the rest of
practice. never went into the trainer's room to see when he was supposedly
injured.
said that he did see appearing to be injured or sick almost at the end of
practice. was sitting on the ground, near the water with his pads off. said that he
saw in the area checking on Mason Beachler. He saw being walked into
the school, he assumed to the training room, by two other players. said that he was
unsure who the players were that walked into the school.

On Monday, September 30,2013, SA Ryan Emahiser met with Woodmore High School Athletic

On Monday, September 30,2013, SA Ryan Emahiser met with Woodmore High School Athletic
Director Stephen Barr at Woodmore High School. The purpose of meeting with Barr was to
discuss allegations of physical abuse that occurred on September 10, 2013, at a Woodmore High
School football practice.
SA Emahiser asked Barr ifhe was on the school grounds September 10, 2013, and he advised
that he was. Barr said that day was especially hot, and he and several of the coaches had been
monitoring the heat index for the day. Barr advised SA Emahiser that he had been in contact
with throughout the day about the heat. Barr stated that has a
guide that directs them on what the recommendations are for different temperatures. The
recommendation was extended and frequent water breaks were to be given. Barr said that there
was never any concern for safety that day, and he was only recommending to coaches that they
provide extra water breaks and rest to the students.
Barr stated that only one coach, the boys' soccer coach, Carlo Puccino, cancelled practice due to
the heat at his own discretion. Barr said he did not tell any coaches to cancel practice due to the
heat. Barr stated that he went outside around 4:15p.m. and 5:00p.m. and visited the different
teams outside around the school. He was checking on them and ensuring that everything was
going well. Barr said he was also checking to make sure the teams had sufficient water supplies.
Barr said that when he visited the football team, he did not observe the team conducting any
particular drills, and he did not see any ill or injured players.

Emahiser spoke with Kutchenriter at Woodmore High School. The interview was in regard to the
allegations occurring at football practice on September 10, 2013.
DETAILS:
On September 30, 2013, SA Ryan Emahiser met with Chase Kutchenriter, sophomore at
Woodmore High School and member of the Woodmore High School football team. SA
Emahiser spoke with Kutchenriter at Woodmore High School. The interview was in regard to the
allegations occurring at football practice on September 10,2013. Kutchenriter was asked the
same battery of questions that were asked to all other football players.
Kutchenriter advised that he was present on September 1 0 for practice. When the coaches sent
the team in to change into full pads, he had headed into the trainer's office to have his wrist
looked at. Kutchenriter said the coaches were upset with how practice was going and the players
were given a time limit to change into full pads. Kutchenriter said that later, he carne back out to
practice in regular clothes because he received an injury to his wrist.
Kutchenriter said that by the time he arrived out to the practice field, the rest of the team was just
finishing the drill that was running. Kutchenriter said that players were just
standing in line taking a hit, and that the tackling players were hitting the tacklees at full bore.
Kutchenriter said that most ofthe players being tackled just stood there with their hands at their
sides. Kutchenriter stated that he did not recall the team ever doing this drill in practice. When
asked if anyone took any hard hits, or if anyone appeared to be injured, Kutchenriter said that he
did not see anything extraordinary. Kutchenriter said that he knew had been
feeling sick that day, but he did not see take any hard hits, until running the scout team
offense. Kutchenriter said that he saw take a couple helmet to helmet hits while playing
scout offense.

Interview of player Chase Kutchenriter
Kutchenriter said that at some point came out of team offense and lay down by his
helmet. Kutchenriter advised that was on the ground and told him that he did not feel
well. Within a minute, walked over to check on Kutchenriter said that
asked how he was feeling, and responded with, "I feel sick." Kutchenriter
said that then instructed him to walk to the trainer's office to get looked at.
Kutchenriter stated that he picked up helmet, helped up, and walked him to the
trainer's office. Kutchenriter said that walked on his own power to the trainer's room,
and was telling him that he felt sick and that he had thrown up on the practice field.
Kutchenriter said that upon arriving at the trainer's room, he left sitting down and went
to get the trainer. The was at the volleyball game in the main gym.
Kutchenriter stated that when came into the trainer's room, he began evaluating
and was asking questions about how he felt. Kutchenriter said that told the
trainer that his head hurt and he had taken a couple of hard hits. The trainer was concerned that
may have suffered a concussion. Kutchenriter advised that wanted to close his
eyes and sleep, and they did not want him to sleep. Kutchenriter said that he was in the parking
lot and saw the trainer and put in the car. Kutchenriter said that
appeared to be out of it when they placed in the car, and the trainer called
parents.
Kutchenriter stated that he was never told to be dishonest about the practice on September 10.

DETAILS:
On September 30, 2013, SA Ryan Emahiser met with Benjamin Wilt, freshman at Woodmore
High School and member of the Woodmore High School football team. SA Emahiser spoke
with Wilt at Woodmore High School. The interview was in regard to the allegations occurring at
football practice on September l 0, 2013. Wilt was asked the basic battery of questions that were
posed to all of the W oodmore football players.
Wilt advised that he was present at practice on September 1 0, 2013, and that practice began
around 3:15 p.m. that day and ended around 6:15 p.m. Wilt stated that all grade levels practice
together, and that day they had regular water breaks throughout practice. Wilt said on September
10, the practice was not going well according to the coaches, and the coaches made them go
inside and change into full pads. The players were given eight minutes to go into the locker
room and change, and the players were separated into those that returned on time and those that
did not. Wilt said that the players that were on time were called "new Woodmore" and the
players that were late were called "old Woodmore." Wilt said that they began a drill in which
the players that were late would get hit and then run to the cornfield. Wilt said they had
previously never done this drill, and that he was in the group oflate players. Wilt said that
told the "new Woodmore" players to hit the "old Woodmore" players and said,
"Just hit 'em."
SA Emahiser asked Wilt how many players were in the on time, "new Woodmore" line and he
said around ten, no more than fifteen. Wilt said that the "old W oodmore" line consisted of about
75 percent of the rest of the team, around thirty players. Wilt said told the players to

SUMMARY:
On September 30, 2013, SA Ryan Emahiser met with Joseph Flores, junior at Woodmore High
School and member of the Woodmore High School football team. SA Emahiser spoke with
Flores at W oodmore High School. The interview was in regard to the allegations occurring at
football practice on September 10, 2013.
DETAILS:
On September 30, 2013, SA Ryan Emahiser met with Joseph Flores, junior atWoodmore High
School and member of the Woodmore High School football team. SA Emahiser spoke with
Flores at Woodmore High School. The interview was in regard to the allegations occurring at
football practice on September 10, 2013.
Flores stated that he was present for practice on September 10 and that practice began around
3:15 p.m. and lasted until 6:30 p.m. Flores stated that he remembered that day well because of
the excessive heat, and said that he recalls the team had around five to six water breaks during
practice. Flores said that because some players were not practicing hard enough, the coaches
made the players go back into the locker room and change into full pads. Flores said that the
coach gave them eight minutes to get back from changing.
Flores said that when they came out from changing, most players were late and only six to eight
players were on time. Flores stated that when they came back onto the practice field the players
that were on time were called "good Woodmore," and the players that were late were called "bad
Woodmore." Flores said that told them to line up across from the "good
Woodmore" players. Flores stated that told the "good Woodmore" players to, "just go
full go and hit them." Flores said that after the first hit, the "good Woodmore" players did not hit
anyone that hard because they felt that it was not good sportsmanship.
Flores said that during the initial drill, there were a couple players who were mismatched by size
that were tackled very hard. Flores said that Parker Lowe and Evan Sandwich both put hard hits

Interview of player Joseph Flores
on some smaller players. Flores advised that during the drill ran by each player
on "bad Woodmore" was only tackled one time. Flores said that some of the players on "bad
Woodmore" were upset because they did not feel like they deserved to be tackled. Flores said
that he read in the newspaper that players were instructed to put their hands behind their back,
however this was not the case. Flores said that the only instructions they received for the drill
were to just stand there, and that the "good Woodmore" players were told "just lay 'em out" by
Flores stated that he just crossed his hands in a defensive stance and took the
hit. Flores went on to say that after drill they did the "bull in the ring drill"
and that there are usually good hits in that drill. Flores said after that drill they went on with
practice.
Flores stated the only person that he saw take a good hit was during scout
offense. Flores said that hit occurred much later in practice, and he saw
running with the ball and being tackled hard. Flores said he does not remember seeing
after the hit, but thought he remembered seeing that was dizzy. Flores said that he did
not see again until he saw brother, driving him home. He only saw
in the car as they drove away and his eyes were open.
SA Emahiser asked Flores if anyone told him not to be honest about the events on September I 0,
and Flores said no and that his coaches told him to be honest about what happened. SA
Emahiser asked Flores if there was anything else and he advised there was not.

Beachler advised that practice was not going well and players were not practicing well. Beachler
said that players were given eight minutes to go into the locker room and change their uniform to
full pads. Beachler said that when they returned back to the filed, he was late and he and other
players were told to get in a line. Beachler said that he was standing in the line of players that
were to be hit, and he was next to Beachler advised that he was tackled by
another player and went down to the ground. Beachler said that he saw take his
hit and the hit appeared to be normal. had even lowered his shoulder into the tackling
player. Beachler said that took a normal football practice hit.
Beachler said that the initial instructions for the drill were given by to get into
lines. Beachler stated that then took over the drill and gave them instructions.
Beachler said that told the players "new Woodmore" was going to hit "old
Woodmore," and then go down the line. Beachler said that he did not see anyone take any
serious hits during this drill. Beachler advised that the team had normal water breaks throughout
practice. Beachler said that after the first drill the whole team did the "bull in the ring," and at

Interview with player Mason Beachler
was sick, but that had been telling him throughout the school day that his head hurt badly
and he was sick to his stomach.
Beachler said that he never saw take any hard hits that day, and that he initially noticed
was having issues when he sat behind the rest of the team on the practice field. Beachler
said that he was on his way to see the trainer himself, when he noticed lying on the
ground on the practice field. Beachler said that he himself was not feeling good so he went to
the trainer's office. Beachler said that about thirty seconds after arriving in the trainer's room
came in to the trainer's room, escorted by another player, Chase Kutchenriter. Beachler
said that he had given an ice pack and a bottle of water, as the trainer had given it to him.
Beachler said that was not feeling any better so he went to get the trainer from the
volleyball game, and as he left for the trainer said jokingly, "Tell him I'm dying."
Beachler said that when he brought the trainer back, asked him if he told the trainer he
was dying and was laughing still and in good spirits.
Beachler said that the trainer told that he may have a concussion and that he had to fill
out paperwork for a concussion, but told him to be quiet and that he just had a headache.
Beachler said the trainer told no, and that he had a concussion and filled the paperwork
out. Beachler advised that the trainer then went back to the volleyball game and other players
came into the locker room. Beachler said that after the trainer left, started getting worse
and he started closing his eyes, and at times his eyes were rolling back in his head. Beachler
stated that there were multiple people including the trainer in the trainer's room attending to
Beachler said at that time, he left the trainer's room and left the premises.

TITLE: INTERVIEW- CONNER HAAS
SUMMARY:
At 0900 hours, Special Agent Thomas Brokamp met with Conner Haas at the Woodmore High
School in Elmore, Ohio. The purpose of the meeting was to discuss with Haas events that had
occurred in football practice on September 10, 2013.
DETAILS:
Haas said that he was a freshman at Woodmore High School and is on the Woodmore High
School football team. He said that on September 10, he was on the practice field with the rest of
the team. They were initially dressed in shoulder pads and had girdles on. He said that practice
was not going well. sent the team into the locker room to change into full gear and
gave the team eight (8) minutes to return to the field.
Haas said that when he returned to the field was there. Haas said that he was
not there at the time they went into the locker room. He said that whoever made it out in time
stood in one line and whoever did not stood in the other line. said that the people that
made it out in time were the "New Woodmore" and those who did not were the "Old
Woodmore." The New Woodmore got to hit the Old Woodmore. After the hitting drill the whole
team had to run to the com field and back. Haas said that he was late getting out.
Haas said that was the only time that the drill had ever been done before. They were told to hit
the guy across from them if they were on the New Woodmore. He said that the hit he took was
not that bad.
Haas said that the freshman game was played on Monday and the junior varsity game was played
on Saturday. He said that there were nine (9) freshmen on the team so they dressed some
sophomores for the freshman games.
Brokamp asked Haas if he had seen anyone that had gotten hurt at the practice on September 10.
Haas said that had been hurt at the practice. He said that was not hurt

during the drill but rather about a half hour after the drill. Haas said that the "Scout" team was
facing the varsity. He was standing on the sideline near at the time. Haas said that
laid down and was holding his head and came over and asked him what
was wrong. told that he was dizzy and was nauseous. At that time,
another teammate took into the school to the trainer's room. Haas said that he did not
hear say that he had been hit hard on the drill nor did he see in the drill.
Brokamp asked Haas if or anyone else had spoken to the team about the
practice on September 1 0. Haas said that about ten ( 1 0) days after got taken out
of practice. told the team that would be back after things got cleared up.
Brokamp asked Haas if was "fiery'' when he was coaching. He said that normally if
was there the team knew that there would be more conditioning that day. Haas said
that is the defensive coordinator and also coaches linebackers.
Brokamp asked Haas if he could think of anything else that may help. Haas said that he felt that
must have lowered his head when he got hit. He said that he (Haas) just took the hit on
his chest. He said that he just felt that was the case and did not see get hit. The interview

SUMMARY:
At 0950 hours, Special Agent Thomas Brokamp met with Joshua R. Cowell at the Woodmore
High School in Elmore, Ohio. The purpose of the meeting was to discuss with Cowell events that
had occurred in football practice on September 10, 2013.
DETAILS:
Brokamp explained the nature of the interview to Cowell. Cowell said that he was a defensive
back playing varsity football and plays wide receiver on the junior varsity squad. He was late
coming out from changing to full pads and was on the "old" Woodmore side. He said that he did
not take a big hit, that the guy that hit him was kind of small. He did not see anyone else take a
big hit either. He said that most of the players felt the drill that they were doing was "dumb," so
they did not hit each other very hard. He said that he was near the last to go through the drill and
did not see anyone get hit very hard. They had done the drill before, but with a pad to fall on in
the past.
Brokamp asked Cowell if he had seen anyone get hurt or if anyone had been sick. He said that he
knew that had been hurt but that he did not find that out until after practice. He said that
when he got into the locker room was laying in the training room. Brokamp asked if he
was sick or hurt. Cowell said that he looked like he was sick but he heard that he was hurt.
Cowell said that during the drills scout against varsity, he believed that was not feeling
well and did not leave the sidelines to participate in the drills. The interview concluded at 1055
hours.

Weirich said that he plays receiver and cornerback for the junior varsity football team at
Woodmore High School. Brokamp told Weirich that our purpose was to speak about the practice
on September 10, 2013 and was asked to explain what he remembered from that practice.
Weirich said that the team went out without practice pants and were having kind of a normal
practice. He said that they were not having a good practice and were told to go in and put their
practice pants on and that they had eight (8) minutes to be back on the field. Weirich said that he
made it back in time and was on the "good" Woodmore team.
Brokamp asked Weirich how many hits he had to make for the good team. Weirich said that he
had to make two hits. He said that he hit Evan Ulinski and Jeremy Cuevas in the drill. He said
that he felt that they hit him harder than he hit them. He said that there was no name for the drill
and that they were told to tackle the other player. He said that he did not really see any other hits
and he was just doing what he had to do. He said that the "new" team did not have to run to the
com.
Brokamp asked if he saw anyone get hurt or if there was anyone sick. He said that
was sick at the practice. told Weirich that he had diarrhea and that he had been
sick. He did not know if was on the "new" or "old" team. told him that he was
sick near the end of practice. He said that he was a good friend of He said that
was still to be with the team and would still stand on the sidelines, although in street clothes. The

SUMMARY:
At 0930 hours, Special Agent Thomas Brokamp met with Colin M. Sandrock at the Woodmore
High School in Elmore, Ohio. The purpose of the meeting was to discuss with Sandrock events
that had occurred in football practice on September 10,2013.
DETAILS:
Sandrock said that he was a sophomore at Woodmore High School and is the starting center for
the Woodmore High School football team. Brokamp asked Sandrock ifhe was on the "old" or
"new" Woodmore squad for the drill after they came back from the locker room with full pads.
Sandrock said that he was on the "old" because he had come out about thirty (30) seconds too
late from putting pads on. He said that had set up the drill for the new and old
teams. Brokamp asked Sandrock to explain how the drill was to be conducted. He said that
told them to stand about three (3) to four (4) yards apart and then just to get tackled by
the "new" player.
Sandrock said that he did not get tackled very hard and that he defended himself in the drill. He
said that they then ran to the com and back and the drill was over. He said that after they ran to
the com they got a water break and then started doing offensive plays with the Scout team
against the varsity.
Brokamp asked if Sandrock noticed any player that had been injured or sick that day. He said
that he knew that had been sick that day and that he had missed school the prior day
with a sinus infection. He said that he did not see who hit in the drill. He said that no
one really hit too hard because they felt bad for the other guys. He said that there were only six
(6) or seven (7) players on the "new" Woodmore side and that they had to do more than one hit.
Sandrock said that the only contact that he had gotten about this investigation was the letter from
the school. The interview concluded at 0940 hours.

At 1025 hours, Special Agent Thomas Brokamp met with Thomas C. Schnitker at the Woodmore
High School in Elmore, Ohio. The purpose of the meeting was to discuss with Schnitker events
that had occurred in football practice on September IO, 20I3.
DETAILS:
Schnitker is a junior at the Woodmore High School. He is a junior varsity center and linebacker
on the Woodmore High School football team.
Brokamp explained that the interview was related to the practice that occurred on September I 0,
20 I3 and asked if Schnitker was on the "new" or the "old" Woodmore team. He said that he was
on the "old" Woodmore.
Schnitker said that the linemen did not feel responsible for making the head coach mad that day
and he was not in a big hurry to get dressed and come back to the field prior to the drill. He said
that the people who were on one side and the people who were late were on the other side. The
coach told the "old" Woodmore players that the "new" Woodmore players were going to hit
them and then they were to run to the cornfield and back after they got up. Schnitker said that he
did not get hit hard but rather just got picked up and laid down. He did not remember who had
hit him. No one was injured during the drill that he knew of. He said that he was probably in the
first third of the players who were hit. He said that no one was injured or complaining of injury
at that time.
About two (2) hours later when the scout defense was running plays against the varsity offense
he felt that had gotten hurt. Schnitker said that would be coverage for a wide
receiver on the defense. Sometime during the drills, told the coach that he needed a
break and sat down. The interview was concluded at I 035 hours.

TITLE: INTERVIEW OF JT ATKIN
SUMMARY:
On September 30, 2013, S/A Momchilov spoke with Woodmore High School football player JT
Atkin. The interview was conducted at Woodmore High School located at 633 Fremont Street,
Elmore, Ohio.
DETAILS:
On September 30, 2013, S/A Momchi1ov spoke with Woodmore High School football player J.T
Atkin. Atkin was present during football practice on September 10, 2013.
Atkin believes practice started at 1515 hours and was over by 1800 hours. Atkin stated on hot
days they have a lot of water breaks. Atkin stated on September I 0, 2013, the football team had a
water break every 1 0 minutes.
Atkin stated that normal practice defense drills consist of backpedaling drills, swarm pick drills,
and pursuit drills. S/ A Momchilov asked Atkin what kind of tackle drills are done during
defense. Atkin stated that a shed drill is often conducted which consists of a ball carrier, blocker
and tackler. The blocker attempts to block the tackler. The goal of the drill is to have the tackler
shed the blocker and make the tackle on the ball carrier.
Atkin advised that another tackling drill is called bowling balls. During this drill a football player
is standing straight up with a pad behind them on the ground. The tackler is five or six feet away
and the drill is not run at full speed. The tackler then tackles the football player who is standing
straight up and tackles them to the ground.
Atkin advised that on September 10, 2013, because it was so hot out, they were in "thud." Atkin
described "thud" as being dressed in shoulder pads, helmet, girdle, and shorts. Atkin stated that
the team was having a bad practice so coach sent the team in to get there pants on. Atkin stated
coach allowed the team eight minutes to get to the locker room, put their pants on and be back on
the practice field.

INTERVIEW OF IT ATKIN
Atkin stated the players who arrived back on the practice field on time were called the "New
Woodmore" and the players who did not return in time were called the "Old Woodmore." Atkin
stated that about 15 players made it back to the football field on time.
Atkin stated that was mad and had the "new" Woodmore hit the "old"
Woodmore. Atkin further explained that "new" Woodmore hitting the "old" Woodmore was a
variation of the bowling ball drill. Atkin stated the distance was farther than normal and stated it
was approximately 16 feet away.
S/ A Momchilov asked Atkin if any players were hurt during this drill or if there were any big
hits. Atkin stated no players were hurt during this drill and he does not remember any big hits.
S/A Momchilov asked if Atkin ifhe remembered seeing getting hit during this
drill. Atkin advised it was approximately an hour and a half after this drill when started
to collapse.
Atkin stated during team defense, started to get light headed and jogged off the field.

explained that never spoke up and said he was injured. instructed
to go to the trainer's office. Zach Sandwisch walked back to the trainer's

On September 30, 2013, SA Jon Lieber interviewed Malachi Brown. The purpose ofthe

On September 30,2013, at approximately 1034 hours, SA Jon Lieber interviewed Malachi
Brown at the Woodmere High School located at 633 Fremont Street, Elmore, Ohio 43416.
Malachi stated that he was present at the football practice held on September 10, 2013. The
practice began at approximately 1530 hours and ended between 1830 and 1900 hours. Malachi
stated that it was hot and humid that day and in his opinion the players were given an average to
a little more than average number of water breaks. SA Lieber asked Malachi what types of drills
are normally conducted during football practice. These drills are described below.
• Team Tackle- Consists of four separate stages.
1. Two players. One player stands in front of a pad that is lying on the ground. The
second player tackles the first player onto the pad.
2. Two players. Player one starts on his knees and goes through the tackling motions on
player two, but does not take player two to the ground.
3. Three players. Player one is the ball carrier, player two is an offensive linemen,
player three is a defensemen. The object of the drill is for player three (defensemen)
to tackle player one (ball carrier).
4. Tackling sled. The tackling sled consists of tackling dummies that the players hit in
an effort to move the sled in a backward direction.
• Team Takeaway- Two players. One player acts as the ball carrier and the second player
tries to strip the ball from the ball carrier.

Interview of Malachi Brown
Malachi stated that the players were told to change into full pads within eight minutes. A small
number of players made it back to the practice field in the time allotted.
then told the players that were late that they were "Old Woodmore" and that they would stand in
a line while "New Woodmore," the players that made it back in time, would hit them. The
distance between the players was estimated to be seven to ten yards. told the
old Woodmore players to take the hit. The old Woodmore players were hit one time each and
then told to "run the com." Running the com requires the players to run to an adjoining
cornfield and back.
After running the com the team began to practice scout defense. At this time
told Malachi that his head hurt and that he felt like he was going to throw up. drank
some water and continued to practice. About an hour later Malachi noticed lying on the
ground off to the side of the field. Some of the players and checked on
was then sent to the trainer's room.
The next day, September 11, 2013, Malachi heard from brother that
went to the hospital.

practice began sometime after 1440 hours and lasted approximately three hours. Jarod stated
that it was hot that day and in his opinion there were not enough water breaks. SA Lieber asked
Jarod what types of drills are normally conducted during football practice. These drills are
described below.
• Team Tackle- Consists of three separate stages.

not changing into full pads within eight minutes. Jarod stated that none of the players made it
back on time. Jarod and five other players were considerably later than others. When he arrived
at the field, the remainder of the team members were already participating in the punishment
drill. This drill was described as a team tackle drill (#1 on page one of this report) but without
the use of a pad for the players to fall on. The distance between the players was estimated to be
5 yards. J arod further stated that the players were lined up in two equal lines. Line one would
hit line two and after recovering from the tackle, line two would hit line one. Jarod stated that
was running this drill.
Jarod did not notice any players with injuries on September 10, 2013. On September 11, 2013,
Jarod heard about receiving injuries during practice the previous day. The
injuries included a concussion, brain trauma, paralysis, and whiplash.

On September 30, 2013 at approximately 0921 hours, SA Jon Lieber interviewed DJ Wellons at
the Woodmore High School located at 633 Fremont Street, Elmore, OH 43416.
DJ stated that he was not present at the football practice held on September 10, 2013. SA Lieber
asked DJ about drills that are normally held during football practice. DJ stated that there is a
drill where one player stands in front of a pad that is lying on the ground. A second player
tackles the first player onto the pad. The distance between the two players at the start of the drill
was estimated to be 7 yards. DJ did not know of a specific name for this drill.
DJ further stated that he heard that was injured during a tackling drill. DJ
described the tackling drill as being the same drill described above only without the use of a pad
for the players to fall on. injuries included whiplash and trauma to the head when his
head hit the ground. DJ also heard that vomited in the school parking lot and passed out
after running to the cornfield during practice.

On September 30, 2013, at approximately 0909 hours, SA Jon Lieber interviewed Derek Wank
at the Woodmore High School located at 633 Fremont Street, Elmore, OH 43416.
Derek stated that he was not present at the football practice held on September 10, 2013. Derek
then stated that he heard another player refer to a drill that had occurred on the September 10,
2013 practice as "murder." This drill consisted of allowing players that changed into full pads
and made it back to the practice field within the time allotted hit the players that did not make it
back to the field within the time allotted. Derek could not remember who he heard this from.
Derek further stated that he had heard that was injured during the "murder" drill.
However, when other players and the coaches asked if he was ok, stated that he
was fine.
Derek stated that seems to have a reduced ability to carry on a conversation.

TITLE: INTERVIEW OF TYLER MULLIGAN
SUMMARY:
On September 30, 2013, SA Jon Lieber interviewed Tyler Mulligan. The purpose of the
interview was to gather information regarding Tyler's knowledge ofthe treatment ofWoodmore
football players and the injury of
DETAILS:
On September 30, 2013, at approximately 0935 hours, SA Jon Lieber interviewed Tyler
Mulligan at the Woodmore High School located at 633 Fremont Street, Elmore, OH 43416.
Tyler stated that he was present at football practice on September 10, 2013; however, he acted as
the water boy due to a neck injury he had received the previous Friday during a football game.
Tyler stated that the practice on September 10, 2013, began at approximately 1515 hours, and
ended at approximately 1830 hours. The following drills were conveyed to be normal drills held
during practice; however, none of these drills were conducted on September 1 0, 2013.
• Team Tackle- Two players. One player stands in front of a pad that is lying on the
ground. The second player tackles the first player onto the pad.
• Bowling Ball - Two players. Player one goes through the tackling motions on player two
but does not take player two to the ground.
• Team Takeaway- Two players. One player acts as the ball carrier and the second player
tries to strip the ball from the ball carrier.
• Angle Tackle - Three players. Player one is the ball carrier, player two is an offensive
linemen, player three is a defensemen. The object of the drill is for player three
( defensemen) to tackle player one (ball carrier).
SA Lieber asked Tyler if there were any hits that stood out during the September 10, 2013
practice. Tyler stated that there was a drill that made the players do after they
took too long to change into full pads. A small number of players did make it back to the
practice field in the time allotted. These players were referred to as "new Woodmore" while the

players that did not make it back in the time allotted were referred to as "old Woodmore." Tyler
stated that the "old Woodmore" players were instructed to line up on one line while the "new
Woodmore" players were told to line up on a line across from the "old Woodmore" players. The
distance between the "old" and "new" Woodmore players was estimated to be 15 feet or 5 yards.
The "old" Woodmore players were told that they had to take a full hit and could not protect
themselves. They were tackled multiple times by the "new Woodmore" players and then told to
"run to the com." Running to the com required the players to run to the edge of an adjoining
cornfield and back.
Tyler stated that after the "old Woodmore" players ran to the com he saw
walking toward the school building and he assumed that was going to the trainer's office.
However, a short time later came back to the practice field.
After practice, Tyler saw lying on a cot in the trainer's room. Tyler tried talking to
but found him unresponsive. Tyler then tapped and still did not receive a
response. At this time Tyler notified condition. checked on
and then called parent's advising them that had signs of a concussion.
Sometime later again called parents and advised them to take to see a
doctor.
Tyler later went to visit at St. Charles Hospital. Tyler thought he heard something about
having paralysis. was then transported to another hospital.

if any players were ever instructed to put their hands behind their backs, and he said he did not
ever hear that. said that would not serve any purpose in football, so it would not have
been instructed to players. also stated that he never saw any cruel or unusual
punishment that day. said that he has known for quite some time,
and he would never put any players in danger.
described the drill that was running as wrapping players up and
taking them to the ground. said that he did not believe there were any straight hard
tackles. The only difference in the drill that day was that there was not a pad behind the players
because there is only one pad and the whole team was doing the drill. said after
ran his drill, they went to the "bull in the ring" drill, and he did not recall
participating in that drill. said he did not run this drill either, but that normal
football rules applied to the players. said they do not allow any extracurricular moves
during tackling drills.
advised agents that he is position coach and he was around
most of practice. He advised that said he had a headache earlier in practice, and he told
if he did not feel up to practicing to take himself out. said that sat
down behind him around 6:00 p.m., and prior to that he had been rotating in as a sub on team
defense. stated he saw walking over to to check on him.

office. said after the players started walking with into the school he went back
to coaching practice. said at no time did he ever see take any hits that he
believed could have caused an injury. said that he could have taken a hard hit in any
of the individual tackling drills that they did earlier in practice.
said he did not see the rest of the day, and that he never went into the
trainer's office or locker room area after practice.

SUMMARY:
On Thursday, September 26,2013, SA Ryan Emahiser made contact with Woodmore High
School Principal James Kieper. According to the Elmore Police Department, Woodmore
Schools had conducted their own investigation into the alleged incident. SA Emahiser met with
Kieper at Kieper's Woodmore High School office to obtain information gathered on the alleged
incident, as well as any additional information obtained.
DETAILS:
On Thursday, September 26,2013, SA Ryan Emahiser made contact with Woodmore High
School Principal James Kieper. According to the Elmore Police Department, Woodmore
Schools had conducted their own investigation into the alleged incident. SA Emahiser met with
Kieper in the Woodmore High School principal's office to obtain information gathered on the
alleged incident, as well as any additional information obtained. SA Emahiser explained to
Kieper that an investigation into the alleged incidents that occurred on Woodmore School's
grounds on September 10,2013, was being conducted by the Ohio Attorney General's Office
Bureau of Criminal Investigation.
SA Emahiser inquired of Kieper ifhe had conducted a preliminary investigation into the alleged
incidents and he advised that he had. Kieper stated that he had questioned all of the players and
coaches on the Woodmore football team about the events that occurred on September 10. Kieper
said that he had compiled a general questionnaire for each person that he interviewed, and the
notes on each questionnaire are his notes of the person's interview. A copy of these
questionnaires, as well as several other written statements obtained by Kieper, are included
within the administrative portion of this report.
Kieper advised SA Emahiser that he had initially heard of the injury to on
September 11, 2013, when his parents called him in sick for school, advising that he had been in
the hospital the previous night. Kieper said that he then received word from Superintendent
Linda Ruiz-Bringman that grandmother called her and advised her that had to

be drug off of the field, that he was vomiting all over, and that he was unconscious while at
practice on September 10. Kieper said that he made contact with the to meet with them
about the incident. A meeting took place with on Monday, September 16.
Kieper said that Superintendent Bringman was alsopresent for the meeting. Kieper said that
addressed concerns about what happened to his son, as well as some questionable
drills conducted during practice. Kieper advised Mr. that an internal investigation was
going to be conducted by the school into the alleged incident.
Kieper explained to SA Emahiser what he had gathered from his interviews with the players and
coaches. Kieper said that it appeared that Woodmore was
conducting practice on September 10, and was unhappy with the effort being shown by the
players earlier in practice that day. Kieper said that sent the players back into the locker
room to change into full pads, and had advised the players that they only had eight minutes to
change and be back on the practice field. Kieper said from what he learned, some of the kids
arrived later than eight minutes back out to the practice field. Kieper said that
took over a drill when the players arrived back out to the practice field, and that
admitted to him that he called the players that made it out in eight minutes the "new Woodmore,"
and the players that were later than eight minutes were referred to as "old Woodmore." Kieper
said that based on his investigation, he estimates that approximately six to eight players made it
out on time, while approximately 24 to 25 players did not make it out on time.
Kieper advised that out of all of the players that he interviewed only three to four of the players
said that they were told to stand on the line and put their hands behind their back, as was alleged.
Kieper said that the remaining players said that they were told to keep their hands at their sides.
Kieper said at that point, the "new Woodmore" kids would tackle the "old Woodmore," and after
the "old Woodmore" players were tackled they would run to the edge of the cornfield in the
practice field. Kieper said that there were players he interviewed that said they were so late
getting out that they never heard the directions for the drill, and told him that there was no way
they were keeping their hands behind their back because they were being tackled. Kieper also
added those players who said they never heard the directions, said they did not get in trouble for
not having their hands behind their backs. Kieper also said that there were some players that
came out so late that the drill was over by the time they arrived to the practice field. Kieper
estimated that the drill only lasted for a couple minutes.
SA Emahiser asked Principal Kieper about the injuries that received. Kieper
said after his investigation he believed that did not show any signs of being hurt or ill
until near the end of practice around 6:20p.m. Kieper stated that during the initial drill ran by
Mason Beachler said that he was standing next to when he was
tackled, and he appeared fine afterwards running to the edge of the cornfield with the rest of the
players. Kieper said that he learned that was playing scout defense and the coaches saw
make a good play and tackle another player. Kieper said that Zach Sandwich saw
vomit while on the practice field around 6:15 p.m. Kieper said that around the same time
frame walked off of the field and sat down, and saw sitting on the
ground. Kieper said that Zach Sandwich was talking to and that said he was
dizzy and light-headed so Zach took his helmet off. asked him ifhe was okay and
asked him if he could get up. Kieper said at that time had Zack Sandwich and
another player walk to see the who was at a volleyball game inside
of the school.

Kieper said upon their arriving into the school, evaluated and said he had
signs of a concussion with sensitive eyes and a headache. Kieper said that called
mother, telling her that may have had a concussion and to keep an eye on him, and if he
gets worse to take him to the hospital. Kieper stated that left with some ice and
returned to the volleyball game for some time. When he returned to check on his
condition had worsened. Kieper said that then called parents again and told them
that they needed to take him to the emergency room and get him checked out. Kieper said that
even helped into his brother's car, and took home.
Kieper advised that initial call to mother was at 6:32p.m. and the second call
advising her to take him to the emergency room was at 6:49 p.m. Kieper said that
went to the residence to check on after learning that his condition had
deteriorated after he had seen him last.
Kieper said that the superintendent made the decision to place on
suspension pending a full investigation into the allegations. Kieper advised that due to BCI
conducting an investigation into the allegations, he would not be continuing any further internal
investigation until BCI's investigation is complete. SA Emahiser also spoke briefly with
Superintendent Linda Ruiz-Bringman who advised that,
she has remained out of the investigation thus far conducted by Principal Kieper.
Linda Bringman advised that Woodmore School would cooperate with any requests made by
BCI for any further assistance. SA Emahiser advised Superintendent Bringman that agents
would be at Woodmore Schools on September 30, 2013 to conduct interviews with all of the
players on the football team, as well as any coaches that are available. Principal Kieper advised
SA Emahiser that some football players attend Penta Career Center in Perrysburg, so
arrangements will need to be made separately to meet with those students.
A letter was drafted by Principal Kieper advising parents that agents from BCI would be
interviewing their children in regard to the allegations that occurred on September 10, and the
letter was sent home with players on Friday, September 27, 2013.

On Wednesday, September 25, 2013, SA Ryan Emahiser spoke with Elmore Police Department
Chief George Hayes in regard to their status on their investigation into the alleged incidents that
occurred at Woodmore High School's football practice on September 10, 2013. ChiefHayes
gave SA Emahiser a copy of their investigative report, and Chief Hayes advised that the
investigation is still in progress.
DETAILS:
On Wednesday, September 25, 2013, SA Emahiser spoke with Elmore Police Department Chief
George Hayes in regard to the status on their investigation into the alleged incidents that
occurred at Woodmore High School's football practice on September 10,2013. ChiefHayes
gave SA Emahiser a copy of their investigative report, and Chief Hayes advised that the
investigation is still in progress. A copy of Elmore Police Department's investigative report has
been placed into the administrative portion of this report. SA Emahiser advised Chief Hayes that
the Ottawa County Prosecutor had requested the assistance of Ohio BCI in the investigation into
the alleged incidents. Chief Hayes advised that his office would suspend their investigation
allowing BCI to conduct the investigation. Chief Hayes also advised that Elmore PD would
provide any assistance required by BCI.
Chief Hayes advised SA Emahiser that his department received an initial report from the Ottawa
County Job and Family Services of physical abuse inflicted upon players at a Woodmore High
School football practice on September 1 0, 2013. Chief Hayes said that upon receiving the report
he directed 'Elmore Police Sergeant Jeff Harrison to conduct an investigation into the allegations.
SA Emahiser made contact with Sgt. Harrison who advised that he had just begun the
investigation, and at this point he had only made contact with Principal Jim Kieper. Sgt.
Harrison said that he had a meeting with Kieper and Superintendent Linda Bringman to briefly
discuss the alleged incident. Sgt. Harrison said that Kieper advised him that he had begun an
internal investigation into the incident and was in the process of interviewing all of the players

Sgt. Harrison advised that at this point he had not yet received Kieper's documentation ofhis
interviews. SA Emahiser advised Sgt. Harrison that he would follow up with Kieper and the
superintendent in regard to the school's investigation. Sgt. Harrison also advised SA Emahiser
that he had retrieved Woodmore football helmet from his parents, and that the
helmet was placed into evidence. SA Emahiser took pictures of the Woodmore football helmet
and the photos are attached to this report. (See Exhibit# 7)

request for assistance by Prosecuting Attorney Mark Mulligan of the Ottawa County
Prosecutor's Office. SAS Cook advised that the request was for the investigation into allegations
of injuries suffered by Woodmore High School students at a football practice on September 10,
2013.
DETAILS:
On Wednesday, September 25,2013, SA Ryan Emahiser was advised by SAS Jeff Cook of a
request for assistance by Prosecuting Attorney Mark Mulligan of the Ottawa County
Prosecutor's Office. SAS Cook advised that the request was for the investigation into allegations
of injuries suffered by Woodmore High School students at a football practice on September 10,
2013. SAS Cook advised that per the request for assistance the Ottawa County Department of
Job and Family Services had already begun a preliminary investigation of the incident, after
receiving a report of alleged physical abuse inflicted upon
student .
SA Emahiser made contact with Social Worker Betsy Gordon of the Ottawa County Job and

Betsy Gordon advised SA Emahiser that upon receiving the reported abuse she made initial
contact with the Woodmore School Board advising them of the situation. Gordon stated that the
school board advised that they would be conducting an internal investigation of the alleged
incident involving and Betsy Gordon
stated that after contacting W oodmore Schools she made contact with the victim
and his family, and made arrangements to meet with them. Betsy advised that she made a home
visit where she interviewed and Attached to the
administrative portion of this report is a copy of Betsy Gordon's report as well as the audio of the
interview with the family. Also attached are the medical records that Betsy Gordon
retrieved from St. Charles Hospital and St. Vincent Hospital.
Having received the request for assistance, interviews of the entire Woodmore Football team will
need to be conducted, including coaches and any other support staff who may have been
involved with the football practice on September 10, 2013. Contact will be made with
Woodmore Schools Superintendent, High School Principal, Athletic Director, and Athletic
Trainer, as follow up to the allegations. Contact will also be made with the Elmore Police
Department, which Betsy Gordon advised that she had initially made contact with their
department to investigate allegations of abuse at Woodmore High School. Follow up will also be
needed with the family, supplementing interviews already conducted by Betsy Gordon
of the Ottawa County Job and Family Services.
