Citibank Supplementary Card Application Instructions
Reminders:
Please don’t forget to fill-out and check details with RED Asterisk (*) on the form:
1. YOUR FULL NAME and your CITIBANK Card Details, in RED BOXES and with Asterisk (*).
2. Complete Supplementary Card Details, in RED BOX and with Asterisk (*).
3. Your SIGNATURE and DATE in the RED Box and with Asterisk (*).
4. Signature of Supplementary in the form is required for *Non- Relative Applicant –
- Fiancee / Fiance / Partner
- Boyfriend / Girlfriend
- house helper
- friend
- employee,
- driver
- officemate
*Company information is needed as well; TIN #, SSS #, Company and Industry in the form
5. For Newly-Married and Spouse is the applicant: Please provide Married Certificate if;
- Married name is not yet updated in Citibank (For Female Principal Card Holder)
- Wife doesn’t have an ID bearing Family name of Husband (For Male Principal Card Holder)
*Otherwise, it will be considered as Non-Relative applicant, please see #4
6. Please attach a Clear Valid Acceptable Government issued ID of your supplementary
- Passport (Not expired)
- Driver’s License (Not expired)
- SSS ID
- TIN ID
- Unified Multi-Purpose ID (UMID)
- GSIS E-Card
- Postal ID (Not expired)
- PRC ID (Front and Back; and Not expired)
- School ID with Birth Certificate (For Students, in absence of Passport)
- Senior Citizen ID
7. Please fill-out form in BOLD / CAPITAL Letters using Black pen
8. For changes in information, please don’t forget to counter sign it.
9. Put N/A for areas not applicable
Please send/email the clear scanned copy of the Application Form and the Supplementary Primary ID to
[email protected] with subject > Citibank : Supplementary Card for (Name of Supplementary).
NOTE:
Processing of your supplementary card request will take about 5-10 working
days upon receipt of your completely filled-out application form and Supplementary Valid
IDs. Please note also that a Service Officer will call you to confirm and verify authenticity of
your application for your security between 9:00am – 6:00pm.
