ask from your high office to retrieve patient’s data at the time when we had our OR
exposure here in DRH. The patients are the following:
First is, Patient C.D.G who undergone Right Frontal Tube Ventriculostomy last
July 29, 2012 @ 10:49 PM with case number of 402353.Under the supervision of our
C.I, Ms. Rowena Abundo RN.MN.The OR nurse assigned to the patient was Mr. Robert
Joy Manlunas RN.
Second is, Patient G. E who undergone Cesarean Section II last July 31, 2012
@ 1:00 am with case number of 410907.The OR nurse assigned to the patient was
Mr. Karl Lorenz Balili, RN.
Pertinent data of the Patient will surely be kept confidential. Hoping For your kind
