QUICK START GUIDE
WELCOME TO THE QWEST 2008 DEMOCRATIC NATIONAL CONVENTION PRODUCT AND RATE GUIDE
This Product and Rate Guide is for ordering Qwest services at the Convention Complex, which is the Pepsi Center building, as well as the adjacent parking areas the Democratic National Convention Committee has designated for the media to be located . Visit Qwest’s website at www.qwest.com/DNC for updates and information. Orders may be placed beginning April 1, 2008. Please see page 6 for ordering deadlines. This guide shows you how to order the products and services you will need during the convention.

SELECTING THE PRODUCTS YOU NEED USING THIS GUIDE
The voice products in this guide fall into two classifications: • Bundles – phone line, long distance and telephone, beginning on page 11 • A la carte – stand-alone voice services, beginning on page 17 If you are looking for an easy way to order a phone line, long distance and telephone, go to the “Voice Bundles” section on page 11. If you need only the voice phone line without other products, go to the “Voice” section on page 17 in this guide.

REQUESTING PRODUCTS AND SERVICES
Go to the “Service Request Form” found on pages 52, 54 and 56 of this guide. This form is your tool to take notes, capture questions and present to Qwest for answers and ordering. At the time of ordering, make sure you have the following: • Contact phone number and e-mail address. • Method of payment. • Location where services need to be installed. Send the completed Service Request Form via one of the options listed on the form (online, e-mail, phone or fax).

Qwest will send your order confirmation, including order number and due date, to you via e-mail.

Monday through Friday 6:00 a.m. – 5:00 p.m. (Mountain Time). Service hours will be extended effective July 1, 2008.

Additionally, voice mail will be available 24 hours a day, 7 days a week. Messages will be returned during normal hours of operation.

SERVICE AVAILABILITY
Please place all orders for products and service by the deadlines set forth below. Some products and services are only available while supplies last or capacity exists. Order dates to know: • • • First day to submit order/s – Tuesday, April 1, 2008. Orders submitted prior to this date will not be accepted. First possible day for standard installation of products and services ordered – Friday, August 8, 2008. Last day to submit order requests for video services----Thursday, June 12, 2008. After June 12, 2008, order requests for video services will be confirmed only if equipment and facilities are available. Any orders confirmed after August 1, 2008 will be subject to expedite charges. Requests for installation prior to August 8, 2008 will be handled on an individual case basis, subject to availability and additional charges, including special construction and expedite charges. Last day to request Professional Services--Tuesday, July 1, 2008. Requests for Professional Services placed after July 1, 2008 will be handled on an individual case basis, subject to availability and expedite charges. Last day to submit all other order requests – Friday, August 1, 2008. Requests for installation prior to August 8, 2008, or orders placed after August 1, 2008, will be handled on an individual case basis, subject to availability and additional charges, including special construction and expedite charges. Disconnection of all services – Friday, August 29, 2008, by noon (Mountain Time)

BILLING AND PAYMENT POLICIES
AUTHORIZATION
Your organization must be authorized by the 2008 Democratic National Convention staff to place orders for communications needs and equipment for specific locations during the Convention. Authorization will be verified before service is furnished.

ADVANCE PAYMENTS
Qwest may require advance payment for all equipment and services. The amount will depend upon the type of service or equipment provided and the duration of service.

INVOICE
Qwest customer bills will include local service charges billed for services added and for per-use services. Bill descriptions and information may be more limited than typical practice and the specific amounts billed may be organized or set out differently from typical practice and/or any contract descriptions/quotes. You must review invoices upon receipt and contact Qwest if you need further information or if you believe there is an error. Bills are printed and mailed upon completion of services installed. Payment of all rates and charges are due and payable upon receipt. Qwest provides several options for payments, including credit card, electronic fund transfers, and cashier’s check drawn on a U.S. bank and made out to “Qwest”.

CREDIT CARD TRANSACTIONS
Qwest accepts Visa, MasterCard, Discover, American Express and debit cards for payment. Payments can be made by contacting a Qwest Representative at: 1 888-DNC-2232 (1 888-362-2232).

TAXES, FEES AND OTHER CHARGES
Except as otherwise specifically provided on the following pages for specific services/products, the rates identified do not include applicable taxes, regulatory charges and fees, and other charges and fees, including but not limited to universal service charges, fees for 911 service, fees to recover certain costs such as number portability and the like. All such taxes, fees, and charges will be due from and paid by the customer in addition to all other amounts.

CANCELLATION OF ORDERS
Cancellation charges will apply when orders are cancelled prior to the due date. See page 9 for descriptions of Move, Change and Cancel Fees and product pages for charges applicable to each product. Requests to cancel services on or after the due date will be billed at the full Convention Rate for that service.

DISCONNECTION OF SERVICES
Because services are temporary, request to disconnect services must be submitted at the same time that orders for services are placed. Services are not prorated regardless of disconnect date. Please use the Service Request Form to indicate the date of disconnection Services cannot be guaranteed beyond noon (Mountain Time) on August 29, 2008. Services will be subject to the teardown schedule and activities at the Convention Complex, which are beyond Qwest’s control.

SERVICE CENTER AND REPAIR INFORMATION
SERVICE CENTER
During the time of the convention, Qwest will staff a Service Center to assist in ordering, installation, and repair for Qwest customers located at the Convention Complex.

Repair needs should be reported to Qwest at 1 888-DNC-2232 (1 888-362-2232).

IMPORTANT INFORMATION AND DISCLOSURES
• “Convention Rates” identified in this Product and Rate Guide apply to services provided at the Convention Complex, which is the Pepsi Center building, as well as the adjacent parking areas the Democratic National Convention Committee has designated for the media to be located, between Friday, August 8, 2008, and Thursday, August 28, 2008, in conjunction with the 2008 Democratic National Convention. All rates include any applicable installation, inside wiring to a service interface and network repair. Convention Rate is a flat rate and applies to service ordered or installed between August 8, 2008, and August 28, 2008, and cannot be prorated. Refer to specific product page(s) for applicable rates. Additional charges apply for any special construction and/or professional services requested by the customer. (Please see “Professional Services” on page 49 for details.) Regardless of any order confirmation by Qwest, orders will not be provisioned without Convention Complex specific site assignments provided by the Democratic National Convention Committee. Customer agrees that if it has not provided Qwest the site assignment by the order due date, Qwest may unilaterally change the due date to a later date. If Customer has not provided a site assignment to Qwest by August 18, 2008, orders may be cancelled by Qwest and a Cancel Fee may be applied to each service or product ordered, notwithstanding any order confirmation by Qwest. Move, Change Fees apply to any order that involves moving or changing any service within the Convention Complex within five calendar days prior to the due date or anytime on or after the due date. Special construction charges may apply, depending upon the nature of the move or change requested. Move: Moving service from one location to another within the Convention Complex. Change: Changing from one feature or level of service to another. • A Cancel Fee applies to any order that is cancelled prior to due date. Cancel: Request to cancel services prior to the due date or cancellation for failure to provide a Convention Complex specific site. Cancellation of services on or after the due date will be billed at the full Convention Rate for that service. NOTE: To request Move, Change or Cancel, please include on the Service Request Form found on pages 52, 54 and 56.






Expedite order charges may apply depending upon the services and date ordered. Charges will be assessed for each order by product: $200.00 for voice services and $500.00 for non-voice services and voice and data ISDN BRI and ISDN PRI services. Services are also governed by additional terms and conditions, which will be located at http://www.Qwest.com/DNC.

Some services in this Product and Rate Guide are conditioned upon obtaining any regulatory approval necessary to perform under the terms and conditions set forth herein. Customers subscribing to services described in this guide are not eligible for additional discounts or promotions. The ordering, provisioning and repair processes described in this guide are subject to change. Purchases are subject to approved credit. Voice Long Distance – Qwest Choice™ Unlimited Long Distance service offers nationwide long distance calling for a flat rate. The plan is available only for manual dialed, voice calls; it is not intended to be used for mechanically/autodialed calls, phone bank, call center, data center, broadcast fax or like mass calling activities. This local ISDN service offers the power to create a seamless communication system that speeds and smoothes the flow of information without the expense of dedicated lines, modems or special cabling. ISDN Single Line Service – also known as “Basic Rate Service” or “Basic Rate Interface (BRI) ISDN” is compliant with national ISDN standards. ISDN Single Line Service uses NI-2 technology to provide digital network architecture for both a voice connection and a high- or low-speed data connection simultaneously over standard telephone lines (twisted pairs). DATA AND VOICE: ISDN Primary Rate Interface (PRI)
Local ISDN PRI is a comprehensive digital network configuration known as 23B+D. PRI is a digital four-wire full-duplex transmission path between ISDN-compatible customer premises equipment (CPE) and an ISDN-equipped Central Office. It is a non-switched digital service which utilizes DS-1/T-1 level 1.544 Mbps to deliver 24 individual 64 Kbps clear channels, comprised of 23 64 Kbps B-channels and one 64 Kbps D-channel. The 24 channels/trunks are 2-way. The B-channel services supported include direct inbound dial (DID), direct outbound dial (DOD) and 2-way dial. All call control, such as setup/teardown and synchronization, is provided on the D-channel. Caller ID is available for incoming calls on 2-way trunks. HDTV-Net is a one-way, point-to-point, digital video transport service, provided over standard fiber optic facilities. The service provides the transmission of real-time standard definition and high definition digital video signals. Qwest offers a variety of Private Line Video Solution services including: HDTV-Net 19.4 Mbps, 270 Mbps and 1.48 Gbps. Qwest offers a variety of private line video solution services including Broadcast Video Service (TV-1). Interstate Broadcast Video Service is generally used by broadcasters when transmitting a video signal “real-time” to an uplink or satellite for national viewing. Service provides one-way transmission capability for a standard 525-line/60-field System M - National Television Systems Committee (NTSC) RS-250C color, video signal. Audio signals are sold as options. Audio interface options include:

Site Survey & Design

Qwest network team is able to provide a site survey and design layout of potential network elements that suit customers’ needs.

Project Management

For qualified products, project management will include, scheduling, tracking and status reporting. A Qwest representative is available to review qualified products that meet project management criteria. Other requests will be considered on an individual case basis and, if available, scope of work and pricing will be provided.

Network Security

Network security solutions may include Custom Managed Firewall Services - 24/7 monitoring and management of Qwest provided firewall.

Special Construction

Services including additional inside premises wiring for voice and local area networks that may be required. Qwest also can provide installation of cabling systems related to Qwest services ordered. Custom cabling jobs are also reviewed on an individual case basis.

Pricing is available based on the scope of work (SOW) per request. Special Construction & Installation may be required when facilities are not available to meet an order for service and facility construction is necessary. Special Construction & Installation charges may also apply in situations where the customer is requesting rearrangement of Qwest facilities.

INVESCO Addendum
The 2008 Democratic National Convention Product and Rate Guide (“Product and Rate Guide”) is amended to include services at INVESCO Field at Mile High stadium (“INVESCO”), the Acceptance Speech Venue, on August 28, 2008. The parking areas at INVESCO are not included in the Product and Rate Guide and any requests for service at INVESCO parking areas will be handled on an individual case basis... All products and services identified in the Product and Rate Guide are available at INVESCO, under the same terms and conditions identified in the Product and Rate Guide, except as set forth in this Addendum. Rate Guide Provisions applicable to INVESCO: The first day on which services may be installed at INVESCO is August 26, 2008. However, availability of services on August 26, 2008 depends upon the timing of access to INVESCO and may not be confirmed by Qwest, or if confirmed, may be subject to change. Convention Rates for services at INVESCO are flat rates for services provided between August 26, 2008 and noon on August 29, 2008 and cannot be prorated. Convention Rates at INVESCO include any applicable installation, inside wiring to a service interface and network repair. INVESCO is not part of the Convention Complex as defined in the Product and Rate Guide. All services at INVESCO must be placed as new orders and can not be moved from the Convention Complex. Requests for moves and/or changes of products or service at INVESCO will not be accepted. Requests for service at INVESCO, including requests for video services, must be placed by August 1, 2008, so that Qwest can make every effort to meet each Customer’s service needs. Due to limited facilities, and in order to ensure that all customers have access to services, all service request confirmations are preliminary and subject to change by Qwest. If demand exceeds capacity, every effort will be made to equitably allocate facilities among all requesting Customers. Regardless of any order confirmation by Qwest, orders will not be provisioned without INVESCO specific site assignments provided by the Democratic National Convention Committee. Customer agrees that if it has not provided Qwest the site assignment by the order due date, Qwest may unilaterally change the due date to a later date. If Customer has not provided a site assignment to Qwest by August 18, 2008, orders may be cancelled by Qwest and a Cancel Fee may be applied to each service or product ordered, notwithstanding any order confirmation by Qwest.
