E-Governance of Health Services and Infrastructure in Rural India A Model for Discharge Coordination in a Hospital Copyright: 2006, Shogo Kato, Satoko Tsuru, Yoshinori Iizuka, and Mafuyu Takahashi Author: Kato, Shogo; Tsuru, Satoko; Iizuka, Yoshinori; Takahashi, Mafuyu Organization: The University of Tokyo; Ohme Municipal General Hospital Subject: Healthcare industry,International case study,Japan,Social Service Agency; Series: Annual Quality Congress Proceedings, Vol. 60, No. 0, May 2006, pp. 1-11 Online full text for this article is not available at this time. Abstract: [This abstract is based on the authors' abstract.]In a effort to coordinate conditions related to patient discharge, Japanese hospitals have found it necessary to determine the next facility to which the patient will be transferred, based on the patient's needs and those of his or her family. A model is proposed that enables social workers to conduct discharge coordination efficiently and correctly. The model is presented as three outputs: a list of canvas items to be considered during discharge, a flowchart of the discharge coordination process, and concrete components of the flowchart. The efficiency of the model is demonstrated in application to actual cases. Title: Hospital Reduces Medication Errors Using DMAIC and QFD Copyright: 2007, ASQ Author: Benitez, Yani; Forrester, Leslie; Hurst, Carolyn; Turpin, Debra Organization: BJC Healthcare, St. Louis, MO; Alton Memorial Hospital, Alton, IL Subject: Case study,Design for six sigma (DFSS),Healthcare industry,Process improvement,Process improvement team,Quality function deployment (QFD),Six Sigma,Voice of

A multidisciplinary team sought the help of a Black Belt to help the team use the Six Sigma define, measure, analyze, improve, and control (DMAIC) methodology to reduce the defect rate of the current process and to develop a standardized process. While changes improved the medication order entry process, the issue of the multiple ways the staff process medication orders remained. The team then decided to use a design for Six Sigma methodology, quality function deployment, to link the needs of the customers to the development of the process functions. Audits revealed the percentage of order entry errors improved by 90 percent. Additionally, the new process is simpler, works on all units, and is more accurate. If you have questions about your membership please contact Customer Care at 800-248-1946 or [email protected] Abstract: [This abstract is based on the authors' abstract.]In 2004 ProMedica Health System's ProMedica Laboratories at the Toledo Hospital introduced lean manufacturing techniques as part of its continuous performance improvement efforts. The focus of these efforts was on identifying, labeling, and eliminating waste. A quality expert was retained to train core team members from multiple laboratory departments about lean concepts. This grassroots approach ensures early staff by-in, transfers knowledge to a broader base of in-house staff, and minimizes cost to the organization. ProMedica Labs has not added any additional staff for Lean, but is reinvesting productivity gains in additional release time for staff to engage in training and projects. Upon completion of three projects, individual team members are qualified to facilitate teams and train others to apply the methods to their work areas. Abstract: [This abstract is based on the author's abstract.]Path innovation uses existing approaches and considers a new way of thinking and addressing problems. Subject matter experts are involved early in process design and are responsible for transferring domain knowledge to team members. Organizations using path innovation make better use of their investment in information technology systems to achieve breakthroughs in quality, patient safety, and resource utilization The Use of Control Charts in Health-Care and Public-Health Surveillance (With Discussion and Rejoinder) Copyright: 2006, ASQ Author: Woodall, William H.; Mohammed, Mohammed A.; Lucas, James M.; Watkins, Rochelle; Steiner, Stefan H.; Benneyan, James C.; Grigg, Olivia A.; Spiegelhalter, Avid J.; Burkom, Howard Organization: Virginia Tech; Univ. of Birmingham ; J.M. Risk Models to Improve Long Term Care Medication Safety Copyright: 2004, Agency for Healthcare Research and Quality Author: Carley, Margaret Murphy; Comden, Sharon Conrow Organization: Oregon Health Care Association; Health Systems Planning Subject: Healthcare industry,Modeling,Nursing,Risk analysis,Sociotechnical systems (STS),Error; Series: Annual Quality Congress Proceedings, Vol. 59, No. 0, May 2005, pp. 459-478 Online full text for this article is not available at this time. Abstract: [This abstract is based on the authors' abstract.]A study was undertaken by the Oregon Department of Human Services to determine if sociotechnical probabilistic risk assessment (STPRA) can create risk models identifying combinations of medical delivery system and behavioral elements resulting in medical error in long term care facilities. Multidisciplinary teams created the models with input from pharmacists, community physicians, and state surveyors. Results showed that prescribing and administration errors are the dominant risks. In addition, multiple single failure path errors were identified. State agencies, long term care providers, pharmacies, and medical providers are collaborating on strategies to address the risks identified in these models. Author: Pellicone, Angelo; Martocci, Maude Organization: North Shore - Long Island Jewish Health System, Lake Success, NY; North Shore University Hospital, Manhasset, NY Subject: Customer focus,DMAIC,Healthcare industry, Quality improvement process (QIP),Six Sigma; Series: Quality Progress, Vol. 39, No. 3, March 2006, pp. 31-36 Abstract: In 2004, North Shore University Hospital (NSUH) used Six Sigma to reduce bed assignment delay turnaround time that made it difficult to balance capacity needs. A capstone project revealed incorrect use of the hospital's bed tracking system. The goal was to shift the mean turnaround time and reduce the standard deviation. The team discovered both a communication and technical failure in two key steps in the process that caused great delay. Once the project was turned over to the nursing management team, the process went from a turnaround time of 226 minutes to 90 minutes, and the Six Sigma score went from 1 sigma to 2.3 sigma at the end of the six-month project. The project has served as a pilot for the rest of the hospital. A sidebar article explains Capstone projects and Six Sigma. Engaging Physicians in Lean Six Sigma Copyright: 2005, ASQ Author: Caldwell, Chip; Brexler, Jim; Gillem, Tom Organization: Chip Caldwell Associates, Saint Augustine, FL; Erlanger Health Systems, Chattanooga, TN; m21partners, Nashville, TN Subject: Six Sigma, Strategic planning, Employee involvement (EI),Healthcare industry,Leadership,Professionals; Series: Quality Progress, Vol. 38, No. 11, November 2005, pp. 42-46 Abstract: Healthcare is one of the most difficult industries in which to install a quality system because of the confusing role of physicians, yet few process changes can be fully optimized without engaging physicians. One reason physicians resist change is that change often places additional burden on their processes. Healthcare leaders can gain more effective collaboration and engagement from physicians by seeking to understand physician needs, building trust, and finding projects that will please physicians by improving the efficiency of physician/hospital interfacing processes. If you have questions about your membership please contact Customer Care at 800-248-1946 or [email protected] Abstract: The principles of lean manufacturing are as applicable to healthcare as they are to the automobile industry. However, unlike manufacturing, healthcare management structures are not usually hierarchical, and hospitals generally are not-for-profit. Value adding for patients is different than for customers in manufacturing because the goal of healthcare is prevention and cure. The 18 building blocks of lean healthcare can eliminate waste and prevent the propagation

of errors. Two 5S projects show how staff members were benefited and lean efforts were encouraged throughout the organization when results were achieved quickly.
