Home care providers, unsung heros
W
hat do you do when your elderly parents
insist on living in their home. No matter
how“young at heart” they might be, there
often are times when more supervision
is needed. Preparing
meals, housekeeping,
a trip to the grocery
story and sometimes
even personal care and
upkeep. Their adult
children can feel guilty
and overwhelmed
like they aren’t doing
enough.
More of the elderly
are insisting in living at home. But you need not
worry, home care providers can ease the burden
on the family and provide a friendly smile and
companionship to those who are in need.
Demand is increasing for home care professionals
since the population of older people are living longer
and at home.
“Among caregivers and those who are in need
of the service, its’ hard to find someone who doesn’t
have a story to tell,” said Kris Faupel, marketing
director of Trova Home
Care. “They either have
been through it or know
someone who has.”
Home care providers
offer medical and
nonmedical care. Many
organizations like
franchisees with Trova
Home Care only provide
nonmedical caregivers
with basic companionship, light housekeeping and
personal care. They can be hired for a couple of hours
a day — to make sure a proper breakfast is eaten,
the house is straightened up and perhaps make sure

s our parents or loved ones age, we find
ourselves dealing with unfamiliar issues
and problems. How do I get my parents
to stop driving? What transportation is
available for them? How can I monitor
their medications? When should I

consider? Or financial issues? And what resources
are available to provide help in their home?
A range of public and private agencies are
available to help caregivers throughout Ventura
County. They range from case management and
care planning agencies to specialized service
agencies like adult day care and respite services.
Adult day care offers supervised care for
dependent adults for a portion of the day,
including social and recreational activities.
Respite care and Caregiver support is there
when you need a break from caring for your elderly
loved one. It can provide a few hours or days away
from daily caregiving responsibilities. Enough for
you to recharge.

HICAP Insurance Counseling
Finding your way around health plans for older
adults can be mind boggling. Most seniors choose
between Medicare and a Medicare managed care plan.

The basic level of care which includes
companionship, light housekeeping and meal
preparation etc., requires a minimum of one
year of experience and references. Since they are
not providing medical care, there is no licensing
involved, however, the company does run
background checks on the home care providers
and they are bonded and insured through the
franchise. Faupel said they also want to know
what their interests are which will help place
them with a client.
“The job as a caregiver is all about creating
relationships and matching personalities,” she
said. And In spite of what many might think, the
job is a lot more than simply babysitting,”
They also are expected to monitor and give
feedback to the family on changes in behavior,
appearance, etc. Often families don’t see specific
changes in their elderly parents and caregivers
can offer that perspective.
“We have a spike in caregiver requests after
the holidays,” Faupel said. As family members
spend time with their parents they might notice
a change — the house is not as well taken
care of, their appearance has become more
disheveled, they are not eating well, there’s a
sense of isolation, etc. It’s then the family realizes
they need someone to help.
When someone is requesting home care
assistance, the provider goes to the home and
meets with an adult child to get the history.
Following that, they are introduced to the
elderly client.
First impressions will tell if it is a right match
between both provider and client — how they
feel with one another and how at ease they are.
Creating the relationship between the client
and the home care provider also gives adult
children that security, relieving some of their
guilt. They can be reassured that the client has a
multilevel of coverage.
Professional home care providers also can be
hired by a primary caregiver to provide respite
care, temporary care as a break for them. Respite
care can be provided for a few hours, a day, or
longer.
Faupel said many people think a home care
professional is “babysitting.”“The job is a lot more
than simply babysitting,” And those who have
experienced their services have a lasting respect
for them.

that’s slowing you down?
You’re out walking, or you’re at home, climbing
the stairs. Your legs or feet ache. Maybe they start
to cramp. You get relief when you stop moving
because the pain goes away.
It’s just part of aging, right? Not necessarily.
Discomfort in your legs or feet during the day could
indicate Peripheral Arterial Disease, or P.A.D.
An estimated 8 to 12 million Americans are
currently living with P.A.D., but only 2 million have
been diagnosed. Even more worrisome, more than
50 percent of those with P.A.D. have no signs or
symptoms of the disease.
According to Alan T. Hirsch, M.D., Professor
of Epidemiology and Community Health at the
University of Minnesota School of Public Health,
“P.A.D. is a disease that has huge significance to the
public and to every American family because it is
associated with a very high rate of heart attack and
stroke, and it is the primary cause of amputation.”
The disease occurs when blood flow to the legs
and feet is restricted by a build-up of fatty deposits
in the blood vessels in the legs. It can cause pain,
cramping and discomfort that can make walking
— and life — miserable, or individuals may have no
symptoms at all. Ultimately, untreated blockages
can cut off blood flow entirely and lead to the loss
of a leg or foot to amputation.
The symptoms of P.A.D. can be tiredness,
cramping or pain in the legs that occurs when you
walk but goes away when you rest; foot or toe pain
that disturbs sleep; and/or slow-to-heal wounds or
sores on the feet or toes. Even if you do not have
symptoms, you may have P.A.D. or be at increased
risk for it.
Factors that put you at risk for P.A.D. include:
• Age 50 years and older
• Diabetes
• African-American or Hispanic
• Currently smoke or are a former smoker
• High blood pressure, or hypertension
• High cholesterol
• Heart disease, heart attack or stroke
“Current national guidelines state that
individuals who are 70 years and older, or those
who are 50 years of age and older who have known
risk factors for P.A.D., or individuals who are younger
than 50 years and have diabetes, should be tested
for P.A.D.,” says Dr. Hirsch.
Doctors can determine if you have P.A.D. with
a simple, painless test called an Ankle Brachial

Symptoms
• Discomfort, fatigue, heaviness,
tiredness and cramping in the
leg muscles during activity.
• Pain in the legs and/or feet that
disturbs sleep.
• Sores or wounds on toes, feet or
legs that heal slowly or not at all.
• Color changes in the skin of
the feet, including paleness or
blueness.
• Lower temperature in one leg
compared to the other leg
• Poor nail growth and decreased
hair growth on toes and legs.
Identification
Physicians diagnose P.A.D.
in several ways:
• Physical Exam of the legs, feet
and toes to discover weak or
absent peripheral pulses or other
signs of disease.
• Doppler (Ultrasound)test
performed on the legs to
determine where the blockage is
occurring.
• Ankle•brachial index (ABI)
performed to compare blood
pressure in the arms with blood
pressure in the legs to diagnose
asymptomatic P.A.D.
Treatment options
Mild P.A.D. can usually be
treated with lifestyle changes or
medication.
• Lifestyle changes: Stop
smoking, eat healthy, exercise
regularly.
• Medication: Lower blood
pressure, reduce cholesterol,
lower the risk of blood clots
and/or manage diabetes.
Procedures
When P.A.D. has progressed in
its severity, further treatment is
recommended to restore blood
flow to the leg and foot, thereby
reducing pain and lowering the
risk of losing a leg or foot to
amputation.
• Endovascular procedures
include atherectomy,
angioplasty, stenting and lastly
surgery.

ne of the more difficult conversations is the one
where we talk about death. But it’s necessary,
particularly for family caregivers who are helping an
ill or aging parent.
And often you will find the parent eager to
participate in the conversation.
Talk about plans for funerals so you fully understand their
wishes. That includes how the body will be laid to rest, what
kind of service is desired and how to pay for funeral and burial
arrangements.
Also understand local options for hospice care. This is
available in Ventura County through a number of different
community-based agencies that emphasize dealing with
symptoms of an illness rather than with cures. Hospice care
meets the needs of terminally ill individuals and/or their families,
including in-home options.
Finally, don’t forget yourself. You might want and need help
in coping with the passing of a loved one. Many local support
groups and agencies are there to provide assistance you need.
You don’t have to go through this alone.

ne of the most important functions when helping a parent
or other older adult, is to ask questions. This is particularly
necessary when it comes to their healthcare.
Be sure to ask themhowthey are feeling, and if they are not
feeling well, try to get as specific information as you can.
When you begin to look for a physician, ask friends for
referrals and then —if possible, depending on your care plan —arrange
for a meeting with the physician where you talk with themabout your
expectations —and theirs.
When medical treatment is necessary, you must be willing to step
up and ask all the questions you can so you, and your parent, are fully
informed of the diagnosis and treatment, including any medication.
The California Health Insurance Counseling and Advocacy Program
(HICAP) offers assistance with Medicare and private insurance operations.
Counselors with the programhave regularly scheduled hours at most
senior citizen centers in the county as well as at hospitals. The Social
Security Administration also can provide answers to many of your
Medicare and Social Security questions.
Visit hospitals, rehabilitation centers or nursing centers, if possible.
Ask questions about the operation; be sure to understand if they accept
Medicare or MediCal, or your insurance. Talk about billing procedures.
Check out nursing home facilities. Make sure it is a place where you
and your loved one will be comfortable and be sure to spend time with the
residents as well as the staff.

Keep your brain fit
If you find yourself wondering where you left
your glasses — again — or struggling to recall
a name or a word you’ve used a million times,
you’re not alone. But if you’re worried that your
lapses are destined to be a permanent part of
old age, take heart. It’s never too late to improve
brain function.
Here are some simple exercises you can do to
keep your brain fit.
• Take a guided tour of a museum or another
site of interest. Pay careful attention to what the
guide says. When you get home, reconstruct
the tour by writing an outline that includes
everything you remember. Memory activities
that engage all levels of brain operation, such
as receiving, remembering and thinking, help to
improve the function of the brain.
• Choose a song with lyrics you enjoy but
don’t have memorized. Listen to the song as
many times as necessary to write down all the
lyrics. Then learn to sing along. Once you’ve
mastered one song, move on to another.
Developing better habits of careful listening will
help you in your understanding, thinking and
remembering. Reconstructing the song requires
close, intentional focus and an active memory.

as they age. With a little help, that is sometimes
possible. Area groups listed in this section can help
provide modifications to the home, such as installing
guard rails or wheelchair access that may be enough
to allowthemto stay put.
But sometimes it’s best to downsize, to move into an apartment,
condominiumor smaller house with less space to worry about. And
there are other options, depending on the individual and the level of
assistance and convenience they desire, including senior apartments,
retirement homes, assisted living or skilled nursing facilities. What’s
best depends on your situation. Be aware that this is a very important
decision. Be sure of what is being committed to. Don’t be afraid to
ask for help. There are a number of organizations that can help you
understand the different types of housing and what is offered. Be
ready to ask lots of questions. This is where your loved one is going

Can we still be happy in old age?
Biologist Lewis Wolpert notes in his just-
published book, “You’re Looking Very Well: The
Surprising Nature of Growing Old,” that young
people who hold negative stereotypes about the
old (regarding them as feeble and helpless) are at
a higher risk of suffering heart disease or a stroke
when they grow older themselves.
Nobody dies of just old age in America. In fact
“old age” is no longer permitted as the cause of
death on death certificates.
It is widely assumed that aging is
programmed into our genes, like our
development from infant to adult. But Wolpert
notes that genetic research now persuasively
indicates otherwise. No normal human genes
promote aging.
Rather, we age because random molecular
damage accumulates in our cells over our
lifetime, and our cells’ ability to repair the
damage declines as we grow older. According to
Darwinian natural selection, once we are past the
age of reproduction and caring for our offspring,
we are useless anyway, so our cells’ repair
mechanisms become unnecessary.
Experiments with laboratory animals indicate
that, with treatment, cell repair can be extended,
increasing the human lifespan as much as
fivefold. It’s debatable whether anyone would
want to live that long. Wolpert quotes George
Bernard Shaw that a person would require an
“insane self-conceit” to believe that “an eternity of
himself would be tolerable even to himself.”
The author, 81, who teaches at University
College, London, admits to occasional episodes
of depression. But he notes that the most
common age for depression is just 45.
His formula for successful aging includes
restricting calorie intake and keeping physically
and mentally active. Having a religious faith,
keeping a pet and living in the country all
contribute to healthy, happy aging, he says.
Age management is already a moneymaking
business in America, with private doctors
charging up to $1,275 a month per patient
to monitor medically supervised lifestyle
regimens. Jeffrey Leake, a senior doctor at the
Cenegenics Medical Institute in Las Vegas, told
the London Sunday Times that the steep cost is
a strong incentive to ensure adherence to the
programming.
For those without that kind of money to
spend, Cambridge University researcher Aubrey
de Grey advised Times reporter Lois Rogers
that the short answer to healthy aging is: Don’t
smoke, don’t get fat —- and don’t retire.

POLST directive is simple but powerful legal document, preventing confusion
P
hysician Orders for Life-
Sustaining Treatment
(POLST) is an available
option terminally ill
patients nowhave to direct medical
personnel on the type of treatment
(if any) they would want toward the
end of their lives.
According to Mary Carr,
executive director of the Ventura
County Medical Association (VCMA),
POLST is a medical and legal form
printed on bright “pulsar pink”card
stock paper and is recognized by
emergency roomphysicians and
medical personnel around the
county.
After a discussion between
a patient and treating physician,
this fairly simple but extremely
powerful legal document is signed
by both parties and gives terminally
ill patients control over end-of-life
decisions.
POLST was first developed in
Oregon in 1990 due to concerns that
Do Not Resuscitate (DNR) orders and
advanced healthcare directives did
not insure that the patients’ wishes
were being honored.
It is a much more direct form
and prevents confusion as to how
a terminally ill patient should be
treated by a medical facility. The
heavy card stock paper formis
durable and can travel along with
the patient if they are transferred
fromone care facility to another.
Designed for people who have
a chronic progressive illness, serious
health condition, or are medically
frail, a completed POLST formdetails
specific end-of-life decisions that
a patient wants honored. These
decisions can include the use of
cardiopulmonary resuscitation,
antibiotics, IV fluids, the use
of intubation and mechanical
ventilation, or artificial “tube”
feeding. In addition, POLST outlines
the patients’ wishes regarding
whether or not they want to be
transferred out of their residence to a
facility for medical treatment.
“It (POLST) should go hand-in-
hand with an advanced healthcare
directive,”Carr said. “Patients know
what their desires are and they
understand the importance of
discussing end-of-life issues with
their physician. A patient should
begin talking with their physician if
there is a possible terminal disease in
the next 12 to 24 months.”
Age is not a consideration in
deciding if POLST is right for your
situation. The document can be
used by both terminally ill adult and
pediatric patients, Carr said.
POLST differs froman advanced
healthcare directive because it
requires a physician’s involvement
along with a signature. It can prevent
unwanted or medically ineffective
treatment, reduce suffering by the
patient and family and insure that
patients’ wishes are honored during
an extremely difficult and stressful
time.
Once signed by the patient and
physician, POLST becomes a valid
order in any medical setting and
must be followed by healthcare
providers. The lawalso provides
immunity for any medical steps
taken to implement the orders on
the POLST.

Orders for Life-SustainingTreatment
Coalition coordinator.

Walking through the emotional journey of aging
Aging isn’t easy once you hit a point in life.
Some dread turning 40 and for others it isn’t until
they qualify for the senior menu, that they really
feel the sting.
In my 10 years of working in senior care, I
have never had anyone tell me they were happy
to be in their 70s, 80s or 90s. With the worry
of decreasing physical ability or increase of
forgetfulness comes the biggest fear, which is
depending on others. Since many adult children
don’t live close or have very busy lives, many
seniors face these fears alone. In an attempt to
keep some independence or not to be perceived
as a“burden” on family, the decision is made to
look into assisted living.
The decision to move into an Assisted Living
community is not only a decision of location
and services provided, but that of emotional
comfort. For many seniors their sense of security
and independence is tied to their home, and
with its loss they will have the sense of losing
themselves. This emotional struggle is at the
forefront of every decision to move to Assisted
Living or to try and stay home. The emotions felt
are different based on who is experiencing them.
For the senior, there are stages of emotions
through the process, much like that of grief. First
is the feeling of anxiety and fear of the unknown.
If you have never lived in Assisted Living, you
won’t have much to draw from. The belief that
independence is lost and you are moving to“a
home” is the biggest misconception. Tip: Spend
time with residents of communities you like,
before you make a decision.
Next is the feeling of being overwhelmed
and loss of control. Big decisions of where and
when and what to do with a home and all one’s
belongings is too much for a senior to process at
once. Breaking it into steps and asking for help
from family and friends is a must.
As the family member, usually the adult child,
look for signs that your senior loved one needs
some assistance. Some examples are: is the house
not being kept the way it always has been, is the
person losing weight, are there more leftovers
or half eaten meals in the fridge than usual, or is
your loved one less socially active than before.
Tip: Start conversations sooner than later and
have a plan for when help is needed. Some of the
feelings of loss that are experienced by mom and
dad carry over to the adult child, so it is important
to be empathetic, but still stay positive.
Others can be of help in walking through this
emotional journey of aging. This includes friends,
clergy, respected medical professionals and other
seniors who have walked the road you are on
now. Emotions can sneak up on us when we least
expect it. The best counter attack is to do your
homework early and know what your options are,
and take your friends and family into the aging
battle with you.

Plan for the inevitable; get organized
While you can never plan for the emotional
response from a death, you can ease some of the
anxiety by taking a few steps to help organize
and execute plans once someone has passed.
Here are a few suggestions:
• Discuss everything. While it is impossible
to know for sure, there are reports that nearly
25 percent of life-insurance policies are never
claimed. A big reason for that has to be that loved
ones don’t know what policies were in place. If
you find yourself digging for documents for your
loved one talk with your family about everything.
Store important information (will, policies, etc.) in
one spot and let family members know where it
is. A Life.doc or binder with all of your important
legal, financial and insurance information is a
great way to ensure your loved ones will know
where to turn.
• Help parents put together a living-will
and power-of-attorney documents in advance
of health problems. If dementia sets in at a later
date you’ll be glad you took the time to get
organized since you may be responsible for there
care. Compile all of their medical and insurance
information in one binder that is accessible and
easily update, with the latest information.
• Resist the temptation to control and learn
to lean on others for help with those things
you aren’t very good at. The process of getting
through the first few weeks of a loved one’s death
is much easier to bear.
• Determine divisions. The last thing
that grieving relatives need is fights over who
gets Grandma’s favorite brooch. If your loved
one did not specify how to divide up the
possessions, allow some time to pass and then
get together and figure it out. An example of
this is family members planning a weekend at
the beach to mark the one-year anniversary of
her grandmother’s death in 2007. Not only did
they honor her memory, but they went through
her items and reminisced. Waiting a year gave
everyone time to settle his or her emotions,
and holding the get-together in a place her
grandmother loved gave everyone perspective.
3. Delegate. Whatever the task that is needed
to do, let each family member in on the action. It
helps avoid the inevitable“I am doing everything”
fight and helps bring together family members at
a time when you need each other the most.
