1. After having passed the entrance exam and interview, proceed to the GATC to obtain the following: A. Enrollment Slip B. (for those eligible) Application Form for Honor Student / Sibling discount. 2. Bring the following requirements placed in a long brown envelope: A. NSO Birth Certificate: 2 Original copies and 1 photocopy B. Baptismal Certificate: 2 photocopies (Please bring the original copy for authentication purposes.) C. Form 138 or Transfer Credentials: 1 original copy and 1 photocopy D. 2" x 2" color I.D. Picture: 1 piece 3. Submit all accomplished forms and requirements to the GATC for checking. 4. Proceed to the Registrar’s Office. Submit the documents cited in procedure number 2 endorsed by GATC. Only applicants with complete requirements, checked by the GATC will be entertained.
5. Get the final assessment of school fees and list of books at the Registrar’s Office. 6. Proceed to the Accounting Office for payment. 7. Proceed to the book distribution center to claim the books.
