PART 6 – APPLICATION ATTACHMENTS AND SUPPORTING DOCUMENTS
CHECKLIST – Please attach the following documents along with your completed application form

Business Information  A written detailed description of the project (Part 4 of the application or an attached document)  Business narrative – provide a brief history of the Business of the Borrower  Business plan with projections for five (5) years (provided by the tenant business if Borrower is the property owner/developer)

Project Information, Estimates, and Budget  Architect drawings (if the project will be an entire fit out)  Project budget (total project including soft costs if applicable)  Contractor and/or equipment estimates with detailed descriptions and information detailing on-site labor/installation costs and offsite material and fabrication costs. Two estimates are required. We recommend you secure at least three. All applicants must seek proposals from businesses owned and controlled by minority persons (MBEs), women (WBEs) or disabled persons (DSBEs) as described below. Equipment estimates MUST include the model numbers of the units. Financial Information (from Borrower)  Operating budget with projections for five (5) years (pro forma)  Three (3) years Financial Statements (Tax Returns for three (3) years if audited Financial Statements are unavailable)  Current year Interim Financial Statements (if available)  Personal Financial Statements of Principals (All Principals owning 20% or more)  Current-year Personal Tax Returns of Principals (Financial forms prepared for the Bank are acceptable if not older than six months) Property Information  Proof of ownership (e.g. Agreement of Sale for Project site or a copy of the Lease Agreement)  Letter from property owner (if different from applicant) granting permission for the proposed work.
