The male genital tract includes the testes, the epididymis, the vas, the prostate and urethra. The testes situated in the scrotum are mainly made up of
loops of fine tubes (seminiferous tubules) which produce the sperm. The sperm cells mature as they pass through the epidiymis (a narrow system of
tubes on the surface of the testes). The vas is a hollow tube, which carries the sperm from the epididymis to the urethra.
It takes 3 – 4 months for sperm to develop, during this time sperm production may be affected by febrile illness, exposure to drugs, toxins, radiation,
local trauma or infection.
The primary laboratory test for male fertility is ” semen analysis”. The sample is obtained by masturbation or collected from a special condom
following intercourse. Sterile containers must be used to collect the sample following three days of sexual abstinence.
A normal assessment should show :

» Semen volume – 2-4mls
» Sperm count – more than 20 million per ml
» Sperm motility – more than 50% moving
» Sperm morphology – more than 30% of normal shape
» White blood cells – should be less than 1 million per ml
» Antisperm antibodies test – should be negative

Immunological factors :
Antisperm antibodies may occur following surgery, trauma or infections of the genital tract. Antisperm antibodies impair sperm motility and their
ability to penetrate and fertilise an egg.

Absence of Sperm : (Azoospermia)
May be due to an obstruction at the level of the vas, epididymis, or even the testes, caused by previous infections, trauma or surgery. It may also be
due to a bilateral congenital absence of the vas. Azoospermia could also be due to testicular failure caused by hormonal, chromosomal abnormalities,
previous infection such as mumps or undescended testes.
A single sperm consists of a head, which contains the man’s genetic information and its tip, an acrosome which will
help the sperm penetrate the outer shell of the egg; a midpiece, which supplies the energy needed for movement;
and the tail which propels the sperm forward.

Requirements For Conception to Occur
In both sexes -

» A normal functioning reproductive system
» An adequate sex drive, and full sexual intercourse

At the beginning of the menstrual cycle, the pituitary gland in your brain releases follicle – stimulating hormone (FSH) which stimulates the ovary to

produce follicles. One of these follicles grows faster to become the ” dominant follicle”. It is from this follicle that the egg will be released.
The ovaries also produce many hormones, the most important ones are oestrogen and progesterone. Oestrogen promotes growth of the follicles and
development of the endometrium, while progesterone, which is released after ovulation, is important in preparing the endometrium for pregnancy.
When the egg is released, it is swept into the fallopian tube, then begins to move slowly down the tube to be fertilised in the outer third of the
fallopian tube, and then continues to the uterus to implant in the lining ( endometruim ) resulting in a pregnancy. If the egg is not fertilised, the
endometium is shed as a menstrual period approximately 14 days after ovulation.

Intra Uterine Insemination IUI
IUI may be the right treatment solution for you. UI is used when the woman is fertile, but pregnancy cannot be
achieved through normal intercourse. It is suitable for a number of categories of patient including: -

Couples using Partner Sperm
Patients Suitable for Treatment
IUI is a good initial treatment option for many couples who have been unable to achieve a pregnancy through
normal intercourse within a period of 12 months or more when the probable cause is: -

» Oligospermia – a reduced sperm count of the male partner.
» Poor movement or motility of the male partner’s sperm.
Either fresh or previously frozen partner sperm can be used.
Both partners will need to attend the Initial Consultation, when alternative treatment options may also be considered but, following this, the first
treatment stage is a full investigation of the male partner.

Couples using Donor Sperm
Patients Suitable for Treatment
Male factor problems account for 30-40% of couples presenting with infertility. This may be due to a total of absence of sperm in the ejaculate, or to
low numbers and/or poor quality of sperm.
We do everything possible to make treatment with the male partner’s sperm possible. These steps include the use of advanced sperm recovery
techniques which are successful in many cases and enable the use of IVF treatment with ICSI.
There is, sometimes, no medical or surgical treatment for these conditions and, if no sperm can be provided by the male partner, one way of
achieving pregnancy is by insemination of the female partner with donor sperm.
The decision to go ahead with Donor Insemination (DI) treatment is often a difficult one for the couples involved. There are medical, ethical, religious
and legal aspects that need to be considered. Patients require thorough counselling to discuss the implications of DI and any reservations they may
have about the treatment. Bridge supports prospective DI patients in every way possible and we will do all we humanly can to make you feel
comfortable with your decision and confident about the future.
As the number of babies available for adoption is limited, DI often offers the only chance of becoming a family and we welcome new Blossom Babies
born as a result of IUI / DI on a regular basis.

What does the Treatment Involve?
IUI treatment can be natural (without fertility drugs) or stimulated (with fertility drugs). The most appropriate course will be chosen at your Initial
Consultation.
The IUI procedure involves placing sperm directly into the uterus at the fertile period each month, as close to the day of ovulation as possible,which is
decided by serial sonography at the centre from 11th day of your period.

First, the semen sample from the male partner, or donor, is prepared by spinning it in the laboratory. This is done to separate the sperm from the
seminal plasma (the liquid which carries the sperm) which is known to irritate the uterus. In normal sexual intercourse the seminal plasma does not
enter the uterus, but rather the sperm ‘swim’ away from it, through the uterus and into the fallopian tube.
Next, the embryologist assesses whether the sample is suitable for IUI. This involves counting the sperm to ensure adequate numbers are present,
checking for healthy appearance and assessing motility (the amount of movement they display).
Finally, to minimise the presence of seminal plasma, the embryologist mixes the sperm with a culture medium.

Insemination
The sperm and culture medium are drawn into a very fine plastic catheter, or tube, which is then passed through the cervix (the neck of the womb)
into the uterus, where the sperm are released. The procedure is usually painless and takes only a few minutes. About 10% of women experience
temporary menstrual-like cramping and, occasionally, a little bleeding. It is usual to lie down afterwards, for 15-20 minutes, after which normal
activities can be resumed.

Possible Side-effects
Separating the sperm from the seminal plasma removes most of the bacteria from the ejaculate, but neither the sperm nor the cervix can truly be
sterilised. There is always the slight chance that IUI could produce an infection in the uterus, although experience has shown this to be a very
uncommon occurrence, with a risk of less than 1%.
Occasionally, even after thorough preparation, some seminal plasma may still be present with the sperm and culture medium. This residual seminal
plasma may cause an irritation in the pelvic cavity, resulting in a feeling of discomfort and bloating. Whilst this may feel as if there is an infection
present, it is not accompanied by fever nor raised white blood cell count, which are the usual signs of infection.

Stimulated IUI
The chances of pregnancy may be increased by the use of fertility drugs, which can be used to support more than one phase of the reproductive
cycle.
Firstly, there are drugs which stimulate the ovaries to produce more than one egg each cycle. Vaginal ultrasound monitors progress by identifying the
number and maturity of ovarian follicles (each follicle can produce one egg). The most commonly used drugs for this phase are Clomid tablets, or
Menopur, Puregon or Gonal F injections.
The next step involves testing for the LH surge. LH, or Luteinizing Hormone, stimulates the egg to ripen and triggers ovulation (the release of the
egg.
Testing for the surge is done from Day 9, using an ovulation predictor kit at home, or by daily urine testing at Bridge. Once the LH surge is detected,
an injection of HCG, either Ovitrelle or Pregnyl, may be given (or self-administered) to ensure release of the egg.
Insemination at Bridge is booked for the day following ovulation.
Finally, Cyclogest pessaries (progesterone) may be used after insemination to facilitate implantation (the process of the fertilised egg attaching to the
uterine wall).

After the IUI Treatment – Natural or Stimulated
A urine or blood pregnancy test can be done 14 days after IUI.

Intra Uterine Insemination (IUI)
This is the method by which processed semen is placed directly in the uterus with the help of a catheter. IUI can be done in a natural cycle, however,
to get better results, it is usually preceded by ovulation induction / ovarian stimulation using appropriate fertility enhancing drugs.

IVF Using Donor Oocyte (Eggs)
Women who are unable to produce their own eggs or their eggs failed to fertilise during an IVF cycle due to poor Oocyte quality can be helped by
using other women’s egg (Donor Oocyte). Replacement of donor embryos and surrogacy are other available options to the infertile couple.

Surrogacy
Surrogacy is when a woman, surrogate mother, carries and delivers a child that is not genetically related to her. For medical reasons, the wife of the
intended parents cannot carry a child but produces healthy eggs. If the wife cannot produce healthy eggs an egg donor can be used. The intended
mother or egg donor takes fertility medication to produce multiple eggs. At the right time, these eggs are retrieved from the intended mother/egg
donor and fertilized with the husband’s sperm in a Petri dish through the process of in-vitro fertilization/Embryo Transfer (IVF/ET). The resulting
embryos are then transferred to the surrogate mother. If a pregnancy results the surrogate mother will carry the child(ren) to term for the intended
parents.

PGD – Preimplantation Genetic Diagnosis
PGD, or Preimplantation Genetic Diagnosis, is a new technique, which marries the recent spectacular advances in molecular genetics and assisted
reproductive technology (ART).
Preimplantation genetic diagnosis enables physicians to identify genetic diseases in the embryo, prior to implantation, before the pregnancy is
established.

It’s a sad fact that IVF (In Vitro Fertilization) technology today is still not perfect. Only one of ten embryos we transfer in the uterus implants
successfully in the endometrium to become a baby. Why is the embryo ( Laser Hatching Video ) implantation rate only 10 ? Some doctors believe this
is because the surrounding shell of the embryo (called the zona pellucida) hardens when it is cultured in the laboratory.

Male Infertility Treatment
Male Infertility Treatment – and why it does not work
Male Infertility can be treated in several ways, and you should be aware that there may be side effects to some of them. Treatments vary from
ineffective Vitamin supplements to the highly successful advanced ICSI (Intracytoplasmic Sperm Injection) treatment
Conventional treatment of male infertility in the past used a wide array of medicines to try to improve a low sperm count in the infertile man.

Cryo Preservation of Embryos ( embryo freezing )
Excess (Surplus) embryos can be Cryo-preserved at ultra low temperatures for many years. These frozen embryos can be used subsequently without
the need for ovarian stimulation and egg retrieval. However the pregnancy rate following transfer of frozen embryos is lower than that with fresh
embryos.

Assisted Reproductive Technologies (ART)
Any medical technique that attempts to obtain a pregnancy by means other than by intercourse is defined as ART. These techniques involve the
manipulation of sperm or Oocyte or both, and the gametes or the embryos are transferred into the uterus or fallopian tubes. ART includes
Intrauterine Insemination (IUI), In Vitro Fertilisation-Embryo Transfer (IVF-ET), Intra Cytoplasmic Sperm Injection (ICSI), Assisted Hatching (AH) etc

Steps involved in IVF procedure:
» Ovarian stimulation by hormonal injections to produce multiple eggs.
» Monitoring of the response by ultrasound scans and blood tests.
» Egg retrieval with the help of a needle under local / general anaesthesia.
» Fertilisation of the eggs in the laboratory.
» Transfer of the resulting embryo(s) into the uterus of the woman.
» Blood test performed 15 days after embryo transfer to access the establish Pregnancy.
If the treatment procedure is successful, one or more embryos will implant in the uterus and the pregnancy will result, just as it happens in the
natural process of conception

Assisted Hatching ( AH )
The procedure is based on the fact that an alteration in zona pellucida (outer covering of egg) either by drilling a hole through it or by thinning it, will
promote hatching or implantation of embryos that are otherwise unable to escape intact from the zona pellucida.

Semen /Sperm freezing
Semen /Sperm can be stored frozen at ultra low temperatures for long duration which could be helpful in a variety of circumstances such as:

» Semen freezing prior to chemotherapy.
» Inability of the male partner to be present or to deliver the semen on the day of procedure(however, the frozen semen is used as a
standby and fresh sample is always preferable)

» Testicular biopsy / sperm freezing to avoid repeated biopsies.
» Donor semen, until getting a repeated investigation done for the infectious diseases.

IVF at a Glance
In vitro fertilization is an option for many couples who cannot conceive through conventional therapies. In IVF,
fertilization occurs outside the woman’s body. The woman’s eggs and the man’s sperm are united in the
laboratory. Once fertilization occurs, the early embryo(s) are transferred to the woman’s uterus.

Eligibility Criteria
IVF is an option for couples who have tried unsuccessfully to conceive or for whom other treatments have failed.
IVF was initially developed to help women conceive who had blocked, damaged, or absent fallopian tubes. Other eligible patients are those with
infertility due to a condition not responsive to conventional therapy, including one or more of the following:

» Tubal blockage or failed tubal reversal
» Endometriosis
» Cervical factor
» Pelvic adhesions
» Male factor
» Unexplained infertility/ failed conventional therapy
» Genetic testing (PGD) for inheritable diseases
» Genetic testing (PGD) for possible reasons for multiple miscarriages
To be considered for IVF, a woman must either have a uterus capable of carrying a child, or a compassionate gestational carrier. For optimal results,
the woman should not be more than twenty percent over her ideal body weight. We consider and evaluate women between the ages of 40-43 for IVF
on an individual basis based on previous testing, such as FSH levels and/or responses to gonadotropin therapy.

Cycle Monitoring
To begin the process of IVF, the woman is given fertility drugs to stimulate egg production and control the timing of ovulation. This helps maximize
the number of eggs produced, thereby increasing the chance that at least one will be fertilized, and will implant itself in the uterus. Frequent blood
tests to monitor hormone levels and ultrasounds to monitor follicle development are required. Stimulation protocols may vary depending on multiple
factors.

Egg Retrieval
The eggs are harvested primarily through a transvaginal ultrasound-guided procedure. The ultrasound-guided egg retrieval is performed on an
outpatient basis with monitored I.V. sedation. Recovery from the sedation requires thirty minutes. The patient is discharged from the surgery center
with instructions to rest for the remainder of the day.
Once the eggs are retrieved and the sperm sample collected, they are immediately given to our embryology laboratory for incubation. Our IVF lab is

located adjacent to our operating room. The man’s semen is specially prepared in order to select the most viable sperm. In conventional IVF, the
sperm are then placed together with the eggs in an incubator for 12-18 hours to allow for fertilization. When sperm quality is compromised, the
eggs are inseminated with a single sperm by means of a micromanipulation procedure known as ICSI (intracytoplasmic sperm injection). This may also
be used on half of the eggs to enhance fertilization in some circumstances. Following normal fertilization, early embryonic development is closely
observed on a daily basis.

Embryo Transfer
Within 72 hours after egg retrieval, the embryos are generally ready to be transferred into the woman’s uterus through a thin tube, or catheter, gently
inserted through the cervix. This is a non-surgical procedure performed under ultrasound guidance with no need of sedation. The woman may wish
to rest for the remainder of the day in order to maximize the chance for success. About ten to twelve days following the embryo transfer, the
woman’s blood is drawn and tested to determine if pregnancy has occurred.

Intra Cytoplasmic Sperm Injection (ICSI)
A revolution for male infertility treatment
ICSI involves injecting a single sperm directly into the cytoplasm of a mature egg (oocyte) using a glass needle (pipette)
thus bypassing any barriers to fertilization. ICSI is currently the most successful form of micromanipulative assisted
fertilization available for male factor infertility.

Reasons for ICSI
» Sperm completely absent from the ejaculate (azoospermia)
» Sperm present in low concentrations (oligospermia).
» Poor sperm motility (asthenospermia)
» Poor sperm morphology (teratospermia)
» Sperm retrieved by surgical techniques (for example TESA, TESE)
» Problems with sperm binding to and penetrating the egg
» Antisperm antibodies
» Previous failed or poor fertilization
» Unexplained infertility
» Frozen sperm limited in quantity
» If preimplantation genetic diagnosis (PGD) is being used to screen embryos for a specific genetic disorder

The ICSI Procedure
The process is performed on an inverted microscope at a magnification of 200x. The microscope is equipped with
a warming stage to maintain a temperature of 37ºC and two micromanipulators moved hydraulically by joystick
controllers. Using these tools, precise movements can be made to microtools which hold the oocyte and sperm.
The oocyte is held in place by suction on a holding pipette which is a little smaller in diameter than the oocyte.
Sperm are injected using a small sharp injection pipette with an internal diameter of 5 microns, just a little larger
than the width of a sperm.
A small portion of prepared sperm is added to a solution of polyvinyl pyrollidone which is viscous and slows down
forward progression, making individual sperm easier to visualize and aspirate. Morphologically normal sperm are
selected for injection, and then immobilized by rubbing the tails with the injection pipette. The immobilized sperm are sucked up into the injection
pipette.
Oocytes oriented in such a way as to avoid injection in the area of the nucleus are held in place and a single sperm is injected into each oocyte. Once

this is achieved the oocytes are returned to the incubator. Fertilization is assessed the following morning, seventeen to nineteen hours after ICSI.

IVF Success Rates are not affected by bedrest
IVF success rates depend on many factors. Some of these we know, some we do not. From the earliest days of IVF, bedrest has been recommended in
the hopes that it would increase the likelihood of implantation. It was not unusual for IVF specialists of the time to keep an IVF patient in bed for 24
hours or more.

As time progressed, the duration of recommended bedrest after an IVF embryo transfer became shorter and
shorter. Then actual data became to emerge that IVF success rates may not depend on bedrest.
In 1988, a report was published calling into question the logic of bedrest after IVF. The arguement was that since
the uterus is tilted in the body, that in the upright or standing position, the uterus is almost horizontal. It made
more sence, then to have an IVF patient stand rather than lie down. when this IVF group then encouraged
patients to get up after an embryo transfer, their pregnancy rates actually increased!
Another study used ultrasound to study the issue. During an IVF embryo transfer, there are air bubbles present
in the transfer cather that are visible on ultrasound. After the embryo transfer is completed, these air bubbles can be seen in the uterus. It was found
that when patients are asked to stand immediately after the embryo transfer, the position of the air bubble (and by extension the embryos) do not
move.
In the modern are, there are two times in which bedrest has been recommended:

» Immediately upon performing the embryo transfer
» After the patient is discharged and goes home. Lasting from some specified amount of time from a few days to a few weeks
Two IVF studies from the United Kingdom have addressed this issue. In both studies, no effect of bedrest could be detected on IVF success rates.
This month, another study was published in the journal Fertility and Sterility. In this study, 406 patients were given the option to rest or not after the
IVF embryo transfer. Of these women, 167 decided to get up immediately and 239 decided to stay for 1 hour’s bed rest. The results are not
surprising. Pregnancy rates did not differ between the groups: 41 out of 167 (24.55%) in the group that got up immediately and 51 out of 239
(21.34%) in the bed-rest group.
This should not be surprising to anyone. We have been counseling patients that bedrest after the embryo transfer or later at home is not necessary
and maybe counterproductive! However, when we tried to institute apolicy where the patients got up immediately after the transfer…they revolted. I
compromised and now allow patients to rest for one half hour after the transfer.
I also have been telling patients that they can resume any normal activities once they are released. No bedrest at home is required. Patients can go
back to work if they like. This reduces stress levels and that may have some benfit as well.

Eight Ways to Deal with the Emotions of IVF
IVF is often a long and emotional roller coaster. Here are some useful strategies to help cope with the ups and downs of the journey.
It is incredibly difficult for those who have no experience with IVF to understand it. The IVF road can become very long and lonely in a world that
seems filled with babies and where it appears that every other woman gets pregnant with ease.
Couples and individuals going through IVF will experience stress and a range of emotions along the way; hope, despair, anger, disappointment,
excitement. Strategies to help cope with the more negative emotions are essential.

Support Group
If possible find a support group of people who are also going through IVF, or start one yourself. Meet for dinner once a month and enjoy the
company of other people who are also sharing the journey you are on. The comfort, the camaraderie, the sharing of hurts and also of laughter can
help you feel less lonely.

Trusted Friends
Identify one or two friends whom you can call on for an ear to listen to you, for a hug, or just to be with when you need it. You would be able to
share where you are up to with your IVF with these people, so that they can be there for you when need arises.

Talking
It is so important to keep the lines of communication open between yourself and your partner (if you have one). Sharing feelings and thoughts can
help you both feel closer to each other and more aware of how each other are going along the way. Make time to talk and make time to be together.

Make Other Plans
While you hope that within the next year you will have a baby to occupy your time, it is possible it won’t happen as quickly or easily as you had
hoped. It is very healthy and important to have other plans and things to look forward to as well. Perhaps you can make plans for a holiday, or you
might take up a new hobby, or you might start a course of study. Naturally you will want to make these plans flexible in case your baby does turn up
soon.

Journaling
Journaling is a wonderful way to recognise and move through your thoughts and emotions. You don’t have to share your journal with anyone, but it
can help you in clarifying anything that you might wish to share with someone else. There are no set rules for what type of journal, how often you
should write in it or on how good you are at grammar or spelling. Just write from the heart.

Rituals for Saying Goodbye
In cycles where a pregnancy has not been achieved it is important to give yourself time to grieve and time to say goodbye. It may sound strange that
you are grieving someone that never came to be, but during the cycle you no doubt pictured a little baby that you hoped would come into your life.
With a failed cycle that baby has gone and needs to be grieved for. One way to say goodbye is to do a small ritual in whatever way feels comfortable
for you. You might light a candle, release a balloon, say a few words, or just sit and be. Don’t underestimate the loss you have experienced and be

The Infertility Trip
The Recognition
This is a journey that no one intends to start, and no one can be sure how it will end. The first step is the recognition that there may be a problem.
The formal definition of infertility is failure to conceive after one year of exposure to pregnancy (“unprotected intercourse”). The concept of waiting
one year reflects the reality that usually conception will take place within 6 months of trying and the full year accounts for those cycles where
appropriate exposure may not have taken place due to missing the right day for example.

When to start?
Most couples start on this journey at the office of the wife’s gynecologist. During a routine exam, she may mention that she stopped using
contraception some time ago and nothing is happening.
The initial evaluation will typically be simple. Most gynecologists will initially recommend that the patient should monitor her fertile period days ,which
are between 12 to 16 days of period in regularly menstruating women. These monitoring exercises also serve the very important function of verifying
whether the couple is having intercourse at the right time of the month. After this initial step, there is great variability as to how gynecologists will
manage their patients who are trying to conceive.
Some GYNs will immediately give the patient some treatment, typically in the form of Clomiphene citrate (Siphene or fertyl) tablets which will enhance
or establish ovulation. This may be a waste of time if the cause of the infertility is not related to ovulation but it isn’t completely off base as a starting
point since about 20-25% of the time it is. Other GYNs will carry out some series of diagnostic tests in an attempt to establish the cause of the
infertility. These tests will typically include evaluations of the husband (semen analysis, post coital tests), the fallopian tubes (hysterosalpingograms
or HSG) and occasionally hormonal assessments (blood tests) or even laparoscopy to rule out endometriosis. Some GYNs will immediately refer
patients to infertility specialists once the diagnosis has been made. It is certainly reasonable for a GYN to carry out an initial evaluation and even
initial treatment but typically if there hasn’t been success within 6-12 months, it is appropriate to see a specialist. Unfortunately, some patients may
lose their opportunity to conceive as a consequence of increasing age because they were not treated by a specialist in a timely fashion, especially in
those situations where the woman is over 35 years of age.

The advanced journey
If a couple hasn’t conceived under the care of the GYN, they will generally end up under the care of an Infertility specialist.
In any case, once a patient comes to see an infertility specialist, the first step will be to review that which has been done previously. Further diagnostic
testing may be required. It is striking how often a couple will be treated without a diagnosis having been identified. A thoughtful infertility specialist
will avoid repeating tests which have already been done. The purpose of testing is to arrive at a working diagnosis for the couple’s infertility.
Once a diagnosis is established, directed therapy can be implemented. For example, if the woman is not ovulating, ovulation induction will be
necessary. If the husband has borderline sperm concentrations, sperm preparations, which can concentrate the available good sperm and intrauterine
inseminations may be used. The medical literature has shown time and again that our therapies are surprisingly efficient; typically a couple will be
pregnant within 3 cycles of a given treatment. The caveat is that pregnancy will happen that quickly if it will happen at all. Once again a conscientious
infertility specialist will need to reassess and possibly change therapies if success is not reached within this time frame.
In some cases, all testing is normal and we cannot find the “reason” for the couple’s infertility. This is the case about 15% of the time. These couples
are thought to have Unexplained or Idiopathic Infertility. This diagnosis can be very difficult emotionally because couples are frustrated when a
problem cannot be identified. Clearly there is a problem preventing pregnancy; however we may not have the tools yet to identify what it is. Even
though this may be an emotionally difficult diagnosis to deal with, the good news is that couples with Unexplained Infertility have an excellent
prognosis for success with treatment… The treatment of the infertile couple is dynamic. It is inappropriate to be dogmatic in this field. As time
passes, situations change and we need to be constantly aware of possible new data which will change the diagnosis and therefore treatment. For

example, if a woman seeks out therapy at age 40, hopefully one of the first tests that will be done is some assessment of egg quality. While it may
have been normal when last checked, it is imperative to recheck it periodically if the couple is still not pregnant. Time is always passing! It is very
important that the couple and their doctor work as a team, continuously assessing where they are, where they’ve been and where they are going. The
point is that once a diagnosis is available, optimized therapy should be carried out for a few cycles and if there is no success, to re-assess and
change course.

The optimal options
Unfortunately some couples will not get pregnant with simpler therapy. Yet many of these couples will be successful with more complex therapies.
The “big guns” of infertility treatments fall under the name of the Advanced Reproductive Techniques (ART). There have been many different
techniques described over the years, usually alluded to by their abbreviations such as IVF, GIFT, ICSI, TET, ZIFT etc. Today, the dominant procedure
(and the original one!) is IVF or In Vitro Fertilization.
IVF is a very powerful tool in that it bypasses non-functioning tubes, it can minimize the impact of endometriosis, and it can bypass male factors. The
biggest change in the treatment of infertility in the last 15 years has been the growth of IVF. This is for the very good reason that success rates have
risen steadily. As recently as 10 years ago, the best IVF programs in the country had “take home baby rates” of 20%. The best programs today have
rates that are almost 3 times higher than that.
The future trend is for IVF to be used earlier in the course of treatment than before. This is not only because it is the most successful therapy option
we have available today but also because it will treat just about all problems which may be preventing pregnancy. As the per cycle success rates
continue to rise and as we continue to reduce the likelihood of multiple pregnancy, it is only a matter of time before IVF becomes the procedure of
choice for the treatment of infertility.

Old age old eggs
We know that all women have a “biological clock.” The difficult part is to determine when a given woman has undergone the transition from having
“good” eggs to “bad” eggs. We know that typically this will happen in the decade between ages 35 and 45 but it can actually happen at any time.
Furthermore, this transition is not necessarily related to timing of menopause, so a woman will not have any hints or symptoms that her eggs may be
decreasing in quality. By the time symptoms such as irregular cycles, hot flashes etc. appear, it may be too late. It is imperative that a physician
treating an infertile couple checks for egg quality. If a woman has abnormal egg quality (usually referred to as “abnormal ovarian reserve”) all
therapies which rely on her eggs will have a very poor likelihood of success (less than 5% chance of healthy live born babies, unfortunately).
Furthermore, our treatments, regardless of complexity or simplicity, will not increase the baseline likelihood of success. This raises the question of
ethics in that if the therapy we offer a couple is not going to make pregnancy any more likely, should we carry it out?
If a woman over 35 has normal ovarian reserve, however, she deserves aggressive efficient therapy. Time is of the essence and the couple should
proceed quickly trough the options. Obviously whoever is taking care of the couple shouldn’t waste any time.

Any other routes?
Sometimes the couple won’t be able to establish a pregnancy because of egg quality issues. The traditional options for these couples have been to
remain as they are as a family or to pursue adoption.
Those are still the right options for many couples. For others however, these are not the right choices. By using in vitro fertilization techniques, we
can establish pregnancies using eggs donated by another woman. This is analogous to the situations where the husband is sterile and a sperm donor
is used. In the process of egg donation, healthy eggs are retrieved from an egg donor and by means of IVF, these eggs are then inseminated with the
husband’s sperm and the resulting embryos are transferred back into the uterus of the wife who is the egg recipient. Technically the process is fairly
straightforward and these are highly successful IVF procedures. Emotionally, however, this may not be the right option for all couples. Obviously this
is a very individual decision and all couples should undergo extensive evaluation and counseling to ensure that this is the right path for them on their
journey to create the family they envision.

Road less travelled!
Sometimes our treatments don’t work. The vast majority of couples presenting for the treatment of infertility will be successful and will do so in a
short amount of time. Most don’t need complex, expensive therapy, such as IVF, and will conceive with simpler office based therapy. Unfortunately,
some couples will not conceive. Sometimes we know why, e.g. poor quality eggs as a consequence of age. And sometimes we never find out the why.
Regardless of this, we have to deal with this outcome since human beings are emotional creatures.
After couples have resolved their situation, different choices will be available. If the lack of success is due to poor quality eggs, donor eggs have
enabled these couples to be parents. Adoption is the right choice for some couples. Other couples may choose to remain as they are, remembering
that they married and decided to make a life together because of each other and not because of possible future progeny. While treating infertile
couples, we have found that the process is as important as the outcome. Of course we wish every couple could have a healthy baby, and we’re
ecstatic when that happens. In the cases where it doesn’t happen (and in the ones where it does!) it is most important that after all is said and done
we can look back and be at peace with what took place. It is critical that the couple as well as the physician can look back and conclude that we
followed the right path in that we didn’t do too much and carry out unethical treatments but we als

Special Feature – CLASS 10,000 LABORATORY
This kind of facility is being offered for the first time in South Gujarat. It entails the most modern air handling unit (AHU) which ensures highly sterile
atmosphere for embryos as per international norms.
In IVF, the results largely depend on the atmosphere in which embryos have to stay for 3-5 days out of uterus. Embryos are very sensitive and are
easily insulted by any adverseconditions, which results in their failure to grow. At Blossom we have tried to address this by putting in very

Incubators
This is the place where all embryos live and grow before they go back to their mother’s uterus. Our 2 Heracell (Germany), 2 Labotact (Germany) and 2

Laminar flow provides a workplace which provides super sterile atmosphere while handling the dishes, eggs, sperm and embryos.

Stereo Zoom Microscopes which produce 3D images are used in IVF lab for egg harvesting during ovum pick-up and other detailed procedures.

Advanced treatment for severe male infertility (MESA and TESA)
When a man has little or no sperm in his ejaculate, it may be possible to retrieve sperm from his testicles or epididymis and have a baby through ICSI.

Surrogacy
In case of a woman who has no uterus or severely damaged uterus, or a woman who has had multiple miscarriages, we offer surrogacy which enables

Dr Praful B. Doshi
Dr Praful Doshi is the senior most consultant at Blossom IVF Centre.Thousands of couple are blessed with a child through IVF at
“Me and Mummy IVF Centre”,which inspired him to expand into present venture to reach out to wider cross-section of
society.He has been a distinguished professor of gynecology in medical college of Jamnagar and Surat for 18 years and was
widely recognized for his gift of teaching.Bandwidth of his 30 years of vast experience includes advanced laparoscopic
surgeries,urogynecology and IVF,offering him respect and fame.He obtained “Diploma in advanced endoscopic surgery from
Germany and obtained his IVF training from USA and Australia.he prides himself in being most informed in his subject and
always first in being new technology of the world at your doorstep. Dr Praful also enjoys being outdoor with his family and
finding quite time for singing and reading.

Dr Mitsu P. Doshi
