Who is Take Care Health Center?
Take Care Health Employer Solutions operates the most comprehensive workplace healthcare delivery system in the nation and manage more workplace health and wellness centers and pharmacies than any other provider. Take Care Health Employer Solutions has 364 on-site Medical Centers supporting 46 states.

Who staffs the Take Care Health Center?
TCHCis staffed by a part-time, board-certified Medical Director/Physician, a full-time nurse practitioner, registered nurse, licensed vocational nurse and certified medical assistant. Our medical team works closely together to provide Mustangers with high-quality care.

What is a nurse practitioner, and what services can he or she provide? Do I need an appointment?
For a preventive care/wellness exam yes, an appointment is needed. For regular illness visits appointments are preferred but not required. However, if you come to TCHC without an appointment and there is a long line or a provider will not be able to see you in a timely manner, we may ask that you make an appointment so you receive the care and attention you deserve and expect. Also, a treatment priority list will be posted in the Take Care Health Center that will specifically identify who will be seen first (e.g., emergencies are always treated first, etc.). A nurse practitioner is a registered nurse (RN) who has a master’s degree (MSN) and advanced Centeral Training. Nurse practitioners serve as the regular health care provider during health and illness visits. In order to provide complete health care, nurse practitioners: • • • • • • obtain medical histories and perform physical exams diagnose and treat acute health problems such as infections and injuries diagnose, treat and monitor chronic diseases such as diabetes and high blood pressure order, perform and interpret diagnostic studies such as blood tests and other lab work prescribe medications and other treatments collaborate with physicians and other health professionals as needed

Who can use the Take Care Health Center?
Every Mustang employee, (Direct, LBO, Leased, Independent Contractor), his or her spouse, and dependents ages 2 to 25, regardless of full time student status, may use the Take Care Health Center. Eligible dependents are defined as natural born, step child, legally adopted or can show proof of legal guardianship. While we are pleased we can extend services to Mustangers dependents, we ask that you do not bring sick dependents into the office while waiting for an appointment.

Do I need to be enrolled in the Mustang health plan to use the Take Care Health Center?
No, you don’t need to be enrolled in the Mustang health plans to use the Take Care Health Center.

What services are provided at the Take Care Health Center?
The Mustang community can use the Take Care Health Center for both work-related injury care and personal illness care. You will be able to receive medical treatment without leaving the Mustang campus. • Lacerations that require stitches • Sprains and strains (shoulder, back knee, ankle etc.) • Allergies • Flu and upper respiratory infections • Eye infections • Headaches; and many other medical conditions or illnesses • Occupational services such as work-related physicals • Travel immunizations • And medical treatment for on-the-job injuries • Annual Preventive Care, Well Woman Exams, and Student School Physicals

How will the Take Care Health Center work with my medical plan?
Co-pays incurred at the Take Care Health Center will NOT be coordinated through ANY medical plan. This will not count towards your deductible, annual maximum out-of-pocket, coordination of benefits, whatsoever.

Will my personal medical information be shared with Mustang?
No, TCHC complies with all state and federal laws pertaining to patient confidentiality (e.g., HIPAA, the Health Insurance Portability and Accountability Act). The only information Take Care Health Center will share with Mustang is information related to a work-related injury/illness or information such as FMLA certifications that you personally allow us to provide through a written release.

Is there a co-pay for using the Take Care Health Center?

When can I go to the Take Care Health Center for a personal medical visit?
Mustangers may visit TCHC for personal medical care before or after their workday, during breaks, during lunch or at other times during the day as permitted by your manager. Of course, if your personal medical condition needs immediate attention, you should come to TCHC as soon as possible.

Additional annual preventive exam care services include:
Immunizations – for 11 years of age and older. 1. Tetanus booster (routine) 2. Menactra – meningitis vaccine 3. Gardasil – HPV vaccine for females 4. Pneumovax – adult pneumonia vaccine 5. Zostavax – shingles vaccine Basic Personal Physicals to include physical examination, basic labs (dependent on age) such as CBC, CMP, lipid, HgbA1C, Thyroid (TSH), PSA, urinalysis and EKG. Well Woman Exams – pap smear and breast exams Will refer out age-specific screenings such as mammograms, colonoscopy. Abnormal findings will be referred to follow-up with Primary Care Physician. Medication refills will NOT be a part of these additional services. Please follow-up with your Primary Care Physician for medication refills.

Can I use the Center charges in conjunction with my FSA/HSA accounts?
Yes, you may file your TCHC co-pays manually on the UHC FSA claim form by attaching the Take Care Health Center charge sheet. Unfortunately, your FSA debit card will NOT work for co-pays at the TCHC. If you are a High Deductible Health Plan participant you will be able to use your HSA debit card at the Center.

Can I still see my primary care physician?
TCHC is not intended to replace your primary care physician. Employees are encouraged to maintain their relationship with their physician under their health care plan. The physician and nurse practitioner at TCHC can work with your primary care physician or specialist. If you don’t have a primary care physician, TCHC can assist you in finding a physician within your health plan. Should you need to see a specialist, the TCHC’s staff can help you identify the best specialist for your condition.

Can I fill my written prescriptions at the Take Care Health Center?
No, TCHC will be able to fax your prescription to a pharmacy or you may take the prescription to a pharmacy of your choice and use whatever medical plan is applicable to your individual situation. While the TCHC is not a pharmacy they will have a limited selection of pre-packaged medications available with associated costs (i.e. co-pay & medication co-pay). Typically, pre-packaged medicine is a 10-14 day supply.

What services will not be provided at the TCHC?

What are the Take Care Health Center hours?
Monday through Friday from 7:00AM to 5:00PM. Closed on office closure days.

What visits do not require a co pay?

How will the TCHC know if I or my dependents are eligible for treatment?
Mustang HR will provide a data feed nightly. All those who will use the clinic must present their Mustang I.D. badge. Your spouse and/or dependents should present another form of I.D.

Will TCHC accept personal checks?
No, personal checks; only cash and credit cards
