Please print this form and mail to the NDEB office with your required documents. Documents received without this form will not be processed. To ensure the correct documents are submitted please carefully review the Guidelines for Required Documents on page 2 and 3 prior to submission. The verification process takes approximately 6 weeks from the date of receipt of all required documentation. Therefore, it is recommended that submission of applications and required documents be completed well before the application deadline date. Submission of documents well in advance of the application deadline date will allow sufficient time for the NDEB to verify the documents and if required, for submission of amended documents prior to the application deadline date. All of the required documentation listed below must be received by the NDEB office by the application deadline date of September 27, 2013. Documents received after September 27, 2013 will only be reviewed for participation in the 2015 Equivalency Process. Please note that it is the Applicant’s responsibility to ensure that documents sent by the university are received by the application deadline date. Therefore, be sure to request the documents well in advance of the deadline date.

Guidelines for Required Documents
Please be informed that: NOTARIZED means the photocopied document has been compared to the original by the individual who has signed and sealed the document. All notarized photocopies must bear the original seal and signature of a Notary Public, a Commissioner of Oaths, or an Officer of a Canadian Consulate or Embassy. The seal/notary statement must state that the document is a true copy of the original document seen by the individual who has notarized the document. Original seals must display the credentials of the individual in English or French. ORIGINAL TRANSLATION means translations done by a certified translator, bearing the original seal and signature of the certified translator, and must be attached by the translator to a photocopy of the original document from which it is being translated. A certified translator is an individual who has been evaluated by a translation authority and holds a license, issued by a government recognized translation association, to perform legal translations. A translation attested by a school official such as a Dean or Registrar, or a notarized photocopy of a translation is not acceptable. Original seals must display the credentials of the individual in English or French. Confirmation of Degree Completion Form The Confirmation of Degree Completion Form must be downloaded from your online application. You must fill in the required information in Section 1 and send it to the university that awarded your dental degree/diploma. The university must accurately complete the required information in Section 2, in English or French, and place a stamp or seal on the form. The form must be received by the NDEB office in a sealed envelope directly from the university by September 27th, 2013. Please note that Confirmation of Degree Completion forms completed by an official of an affiliated College will not be accepted. Transcript of marks An original official transcript of marks must be received by the NDEB office directly from the university that issued your dental degree (in a sealed envelope, bearing the university’s stamp) by September 27th, 2013. A transcript of marks is your academic record from all dental schools or faculties attended stating the results obtained for each course, throughout all years of the program. A Transcript of Marks issued by an affiliated college will not be accepted. If the university does not provide a transcript of marks in English or French, an ORIGINAL TRANSLATION of your transcript of marks is required. Government Issued Photo Identification You are required to submit a photocopy of your current government issued photo identification (ie. Passport, driver’s license) issued in English or French. The name used on your online application must exactly match your name as it appears or your current government issued photo identification. Proof of name change/difference in name If your name appears differently on any documents submitted, your government issued photo identification, or on your online application, you must provide a NOTARIZED photocopy of a proof of name change document in the language issued. If your proof of name change is in a language other than English or French, you must also provide an ORIGINAL TRANSLATION into English or French. If you do not have a proof of name change document or did not change your name but your name appears

3 differently on any documents submitted, you must provide a sworn affidavit attesting to the difference in your name. A sample template of a sworn affidavit can be found on the Dashboard of your online application. Dental Diploma/Degree You are required to submit a NOTARIZED photocopy of your dental diploma(s)/degree(s), issued on the date of graduation, in the language issued. If your dental diploma/degree is in a language other than English or French, you must also provide an ORIGINAL TRANSLATION into English or French. Internship Completion Certificate If your dental faculty or school requires the completion of an internship prior to awarding the diploma/degree, you are required to provide a NOTARIZED photocopy of the internship completion certificate or equivalent in the language issued. If your internship completion certificate is in a language other than English or French, you must also provide an ORIGINAL TRANSLATION into English or French.
