Patient Protection and Affordable Care Act:

Title V: Health Care Workforce

Title V

STATE PERSPECTIVE

An Academic Track
• The track will be offered through select universities and will grant advanced degrees that focus on teambased service, public health, epidemiology, and emergency preparedness and response. The plan is to graduate from this track each year:
• • 150 medical students • 100 dental students • 250 nursing students • 100 public health students • 100 behavioral and mental health professional students • 100 physician assistant or nurse practitioner students • 50 pharmacy students

State Responsibilities in Health Reform
• Many of the key elements of health care reform are envisioned to be delivered at the state level. As such, states are charged with establishing some key mechanisms for implementation. For those states that choose not to put those components in place, the Secretary of the U.S. Department of Health and Human Services (HHS) will set up alternative mechanisms, often working through local non-profit organizations

In 2010, the state must:
• Put in place a process to review “unreasonable” insurance premium increases. • Within 90 days of enactment (i.e., by June 23), establish a temporary high risk pool to cover those who were previously excluded from coverage as a result of pre-existing conditions until 2014. • By July 1, establish a mechanism so all residents can identify affordable health insurance options in the state. • Maintain at least existing levels of coverage in Medicaid and CHIP through 2019. • Upon enactment, provide direct payments to freestanding birthing centers. • Within six months of enactment, conduct a needs assessment related to maternal, infant and early childhood home visitation services. • Cover tobacco cessation services for Medicaid covered pregnant women.

By 2014, the state must:
• Establish American Health Benefit Exchanges and a Small Business Health Options Program (SHOP) to offer Qualified Health Plans (QHPs) to provide specified health benefits coverage to individuals and small businesses. Establishment grants are provided to the states in 2011 to begin development of the Exchange. • Contract with one or more reinsurance entities to replace the temporary high risk pool. • Establish secure, electronic transfer of information between Medicaid, CHIP and the Exchanges. • Expand Medicaid coverage to all those up to 133 percent of poverty (states have the option to expand Medicaid coverage starting in 2010. If phased in, they must start with the most needy). • Collect a wide range of data in order to better measure and address health disparities.

Research and Innovation Grants
• The Act includes considerable funding for research to be carried out by academic and other provider organizations. Most notable is funding for the Patient Centered Outcomes Research Institute (PCORI, Sec 6301). The PCORI is funded to do comparative effectiveness research with a focus on both care management and clinical research. Funding for the PCORI is expected to grow from $10 million in 2010 to $500 million by 2013.

Research and Innovation Grants
• • • • • • • • • • • • • Post-partum depression research, screening, and treatment. Local entities, through the Center for Quality Improvement and Patient Safety at AHRQ, to improve safety, focus on health care acquired infections, improve ICU care and reduce readmissions. Implementing and supporting medication management services to improve the quality of care and reduce treatment costs among patients with chronic disease. Four pilot programs to design, implement, and evaluate innovative regional emergency care and trauma systems. Prevention, wellness, and public health activities. Development and operation of school-based health centers. Activities that promote individual and community health and prevent the incidence of chronic disease. Five-year pilot programs to improve the health status of 55-64 year olds. Nurse managed health care clinics. Establishment and operation of new federally qualified health centers. Community mental health centers to coordinate and integrate services through co-location of primary care and specialty care. Research on prevention of and new screening tests for breast cancer in young women. A national diabetes prevention program.

Innovations in the Health Care Workforce
• The purpose of this grant program is to enable state partnerships to complete comprehensive planning and to carry out activities leading to coherent and comprehensive health care workforce development strategies. • State workforce commissions could consider the combined effects of state and federal policies on workforce distribution.

Innovations in the Health Care Workforce
• States must reserve at least 60% of implementation grant funds for competitive grants to encourage regional partnerships to address needs and promote career pathway • Activities Establishing regional partnerships and promoting career pathways should be advantageous for underserviced places.

Academic Grants
• The Secretary may award grants to institutions of higher education to support the recruitment, education, and clinical experience of students in social work and psychology, and to organizations to pay for programs for pre-service or in-service training of paraprofessional child and adolescent mental health workers.

National Health Service Corps
• They will repay up to $60,000 in students loans for those who:
– serve 2 years in a medically underserved area – And are primary care, nursing, dental, or medical health clinicians.

• This year’s investment include $290 million in the primary health care workforce to help those who may not be privy to such services. • For the first time, clinicians may apply online for loan repayment as well as further information

Benefits to Clinicians
• It will expand coverage and consumer protection to offer security • Reduces paperwork to decrease cost of care through simplification of administrative duties • Consumer protection to hold health insurers more accountable for treating patients and providers health care • New implementations for healthier lives and to promote prevention and public health • Improvements to health care delivery will be driven by health care providers • Strengthening the health workforce through growing and investing in community health centers and new clinical settings

• • • • Overcoming language barriers More likely to work in underserved areas Reduce inequities in academic medicine Address difficulties for patients in navigate the health care system • Implementing opportunity grants that focus on areas with African-American, Hispanic, or American-Indian populations

Title V

IMPLEMENTATION

Three Years in…
• Dependent coverage to age 26, no lifetime caps • Prohibition on denying coverage to children with pre-existing conditions • Pre-existing condition insurance plan for current uninsured • Small business tax credits • Premium review and rebates • No cost-sharing for preventive services in new private plans and Medicare as well as for new women’s preventive services

In progress and to come in 2014
• Coverage becomes mandatory • State decisions about health insurance exchanges • State decisions about Medicaid Expansion – states can decide any time • Federal regulations on many aspects of ACA operations, including exchange rules, plans, rules, Medicaid eligibility

INDIVIDUAL PERSPECTIVE

Individual
• It states that parental financial information is not required to determine financial need for an independent student and the determination of financial need will be at the discretion of each school loan officer.
