General Directions to the Hotel: Sheraton Station Square Hotel 300 West Station Square Drive Pittsburgh, PA 15219 412.261.2000 From Pittsburgh International Airport: Take 60 South (Pittsburgh) to Interstate 279 North to Exit 5C. Bear right and continue for approximately one mile. At the second light, turn right onto Carson Street (837). The hotel is approximately 1.5 miles on the left. From East: Take Interstate 376 West to the Grant Street Exit (1C). Turn left at the first light onto Fort Pitt Boulevard. Stay in the left lane and go one block to Smithfield Street. Turn left at the light onto the Smithfield Street Bridge, and the Station Square entrance is at the end of the bridge. From North: Take Interstate 79 South to I-279 South to Fort Pitt Bridge. Stay in the far right lane and exit 5C (just before the tunnel). This will put you on Carson Street (Route 837). Turn right at the first light and follow the access road to the hotel. From South: Take Route 79 North to I-279 North (Pittsburgh). Take Exit 5C (West End) and bear to the right. Continue for approximately one mile and at the second light, turn right onto Carson Street (837). The hotel is approximately 1.5 miles on the left. PAT Bus Service to Station Square: 42S, 46B, 46D, 46F, 46G, 46H, 46K & 47L The Allegheny ONTRACK Initiative: The Allegheny ONTRACK Initiative (AOI) is a five-year Treatment for Homeless grant funded by the Substance and Mental Health Service Administration and awarded to Primary Care Health Services/Health Care for the Homeless. ONTRACK intends to initiate, expand and strengthen integrated mental health and alcohol and other drug (AOD) treatment services for homeless persons in Allegheny County, Pennsylvania. The purpose of ONTRACK is to improve patient and process of care outcomes for homeless persons experiencing AOD disorders, mental disorders or co-occurring disorders in Allegheny County. Through ONTRACK, Health Care for the Homeless has partnered with experts in homeless service delivery and evaluation including Western Psychiatric Institute and Clinic, Operation Safety Net, Community Human Services Corporation, the VA Pittsburgh Healthcare System and other community organizations. ONTRACK will achieve its purpose by attaining two broad goals: 1) to establish model integrated clinics of co-located medical, AOD, and mental health AOD treatment for homeless persons in Allegheny County; and 2) to initiate, promote and utilize evidence-based Screening, Brief Interventions and Referral to Treatment (SBIRT) for AOD disorders within a new integrated clinic system for homeless persons of Allegheny County.
Allegheny ONTRACK Initiative presents a conference on building best practices through integration, collaboration and other evidencedbased practices for persons that are homeless.

Please join the organizations listed below as we work together to enhance recovery in the lives of persons who are homeless. At the conclusion of the Health Care for the Homeless is ONTRACK with Recovery conference, participants will be able to: • identify the health disparities and the unique challenges and barriers that homeless persons encounter on their road to recovery; • explain the concept of recovery and the importance of promoting recovery-oriented practices for persons experiencing homelessness; • describe the evidenced-based and promising practices of Screening, Brief Interventions and Referral to Treatment (SBIRT), integration and how to apply concepts into their own service areas and recovery; and • assist individuals in using their unique skills and strengths in their recovery journey.

Mindy Fullilove, MD, Keynote Speaker Professor of Clinical Psychiatry and Professor of Clinical Sociomedical Sciences. Co-Director, Community Research Group, New York State Psychiatric Institute and Mailman, SPH, Columbia University. Listed as one of the “Best Doctors in New York” in the category of addiction psychiatry, Dr. Fullilove’s current work focuses on the connection between urban function and mental health. She is the author of Root Shock: How Tearing Up City Neighborhoods Hurts America and What We Can Do About It. Adam Gordon, MD, MPH, FACP, FASM Assistant Professor of Medicine and Advisory Dean at the University of Pittsburgh School of Medicine. Dr. Gordon’s specialty is in addiction medicine. He is also an evaluator for the Allegheny ONTRACK Initiative and an expert in SBIRT. David Hall, MD Medical Director for East Liberty Family Healthcare Center and Medical Advisor for Health Care for the Homeless. Dr. Hall has extensive knowledge and is an advocate for the delivery of health care services to under-served populations. Randall Kolb, MD Director, Family Medicine Residency Program at UPMC Shadyside. Dr. Kolb has been a medical provider/volunteer in the Health Care for the Homeless Clinics for over 18 years and is a recipient of the Jefferson Award. Wesley Sowers, MD Medical Director for the Office of Behavioral Health Department of Human Services of Allegheny County; Director, Center for Public Service Psychiatry of WPIC. Dr. Sowers has extensive experience in the provision of treatment to homeless individuals and substance users. Kenneth S. Thompson, MD Medical Director, Center for Mental Health Services at SAMHSA and Associate Professor of Psychiatry and Public Health at the University of Pittsburgh and WPIC. Dr. Thompson has worked for primary care and HIV clinics, state hospitals, disaster response teams, homeless outreach teams and community mental health centers. He advocates for the transformation of the mental health system to support recovery. Paul Wittman, MSW, LSW, MPA Manager of Training, Community Care. Mr. Wittman is an experienced trainer in the areas of recoveryoriented services. Michael Yonas, DrPH Assistant Professor, Department of Family Medicine University of Pittsburgh School of Medicine; Co-director of the Clinical and Translational Science Institute (CTSI) Community Engagement Core Dr. Yonas partnered with Community Human Services Corp. to explore the social and environmental factors associated with living in a housing crisis using a community-based participatory research method called Photovoice.

Registrations must be received by February 26, 2010. Seating is limited, so register early. Registrations are accepted on a first-received, first-registered basis. Upon submission of a registration form, participants are registered unless otherwise notified by the college. There is no registration fee for participants.
