April 2012 Professor of Congenital Cardiac Surgery,
Bristol Heart Institute/Bristol Royal Hospital for Children NHS Foundation Trust; School of

From May 2014 I have been invited to set up the Congenital Heart Surgery Program at the
RUSH University Medical Center in Chicago, where I have been appointed Director of
Congenital Heart Surgery.
This is a full-time clinical academic appointment with a 50:50 clinical: research split. My
clinical interests include neonatal surgery, complex aortic surgery in young adults, minimally
invasive pulmonary valve surgery, mitral and tricuspid valve repair. I perform over 200
complex major cardiac procedures per year. My 30 day mortality rates have been constantly
below 1%

(6) TEACHING AND TRAINING
I am recognised expert in several fields of cardiac surgery and I have given invited lectures on these topics
at national and international meeting, educational seminars and postgraduate courses. This includes being a
faculty member on the Bristol Training Course in Cardiac Surgery, the Human Disease Course, the
Paediatric Ground Round Program and the Bristol Paediatric Simulation Programme. In 2000 I was
appointed to a Visiting Teacher position at the International Heart School “J W Kirklin” in Bergamo, Italy.
This is now recognised as the European School for Cardio-Thoracic Training. I am routinely involved in the
preparation of monthly audits meetings for the Cardiothoracic Services at the Bristol Royal Hospital for
Children and the Bristol heart Institute, and the junior doctors training for the Fellowship Examination. I am
also the supervisor for Research Registrars with projects for Higher Degree thesis.
I have developed a monitoring system for assessing performance new surgical techniques, which greatly
facilitate the auditing and constant monitoring of the results. This work has been widely published and has

become fundamental in monitoring the training process of our specialist registers as well as the consultants
in learning new surgical techniques.
Since January 2000, I have been involved as the chief surgeon, in the development of the Paediatric Cardiac
Unit at the University of West Indies in Trinidad and Tobago, teaching local doctors and nurses, and since
March 2006 I am an honorary surgeon for the International Compassionate Medical Assistance
“CardioStart”, participating in a clinical and educational mission in Peru.
Since September 2009 I have started a video assisted anatomical course on the most common congenital
abnormalities with videos recorded by myself of the most important surgical procedures performed at the
Bristol Royal Hospital for Children. The lectures are run together with a cardiologist (for the pre-operative
investigations) and have been greatly successful. The web site with all these interactive lectures is being

Since 2010 I have also been appointed as surgical adviser for congenital heart surgery at the Bristol Medical
Simulation Centre.
I act as external reviewer to several UK and European Universities for undergraduate and post-graduate
degrees and Unit Assessment.
Since 2013 I am the teaching leader for the congenital heart disease theme for the MSc in Translational
Cardiovascular Medicine at the University of Bristol.
Since April 2014 I have been nominated academic mentor for the CardioThoracic and Vascular Residency
Program at the Rush University Medical Center.

Postgraduate teaching
I have responsibility for supervising postgraduate students aiming to post-graduate higher degrees including
designing the study, getting ethical approval and R&D registration, sourcing funding, and selecting the
student. I also advise students during the conduct of the trial, data collection, writing-up of publications, and
final thesis. Most of my post-graduate students have now reached consultant level in UK and abroad.
As a Clinical Academic I train junior doctors allocated to my firm. Since my appointment I have contributed
to the training/teaching of either a Specialist Registrar, or a Clinical Fellow allocated to my firm on a sixmonths rotation basis. I am co-organiser of the weekly “Paediatric Teaching Programme” for junior doctors.
I am the Principal Lecturer in charge of the Congenital Heart Disease section of the MSC in Translational

(7) RESEARCH
Major Achievements
Since my appointment to a Reader position in 2008, I have achieved a total of 10 successful grants for a
total approximately of £ 3 million. In October 2008 the Bristol Heart Institute has also been granted £7
million for the NIHR Biomedical Research Unit (BRU) for Cardiovascular Disease, with my congenital
projects successfully chosen for funding. A £7 million award has been recently achieved at the University of
Bristol and the University Hospitals Bristol NHS Foundation Trust to fund the renewal of the BRU for
Cardiovascular Disease with congenital heart disease chosen as a major research area under my direct
leadership (approximately £1 million for research in the congenital field). In 2014 I have been awarded the
prestigious Sir Jules Thorn Award for Biochemical Research in stem cell tissue engineering in congenital
heart surgery (£1.5M). At the same time I have published several original articles, review articles, book
chapters and presented videos my innovative surgical techniques at the most prestigious international
meeting and specialised websites.
I have established an international reputation in two major areas of translation research:
i)
Improving clinical outcomes in myocardial revascularization surgery.
ii)
Improving cardiopulmonary bypass (CPB) and cardioprotective strategies for myocardial
preservation in congenital heart surgery
Adult cardiac surgery
I have led the first controlled randomised trial on the use of epidural anaesthesia (BHF support) during off
pump coronary artery bypass surgery, demonstrating a significant decrease in hospital morbidity, intensive
care and hospital stay, and had a significant impact in changing clinical practice around the world. At the
same time I have evaluated the impact of pre-operative use of ACE inhibitors on hospital outcomes in
patients undergoing heart surgery. This study, presented at the American College of Cardiology 2008
received significant media coverage and a TV interview with the Editor of JACC. The study findings were
widely published in the scientific internet press and it has been included in the 2011 ACCF/AHA Guideline
for CABG Surgery (www.medlive.cn/uploadfile/2011/1110/20111110104731867.pdfguidelines). Finally,
my work on the effects of pre-operative renal dysfunction on clinical outcomes in patients undergoing
cardiac surgery, has led to the development of new tools for risk stratification. This study had a significant
international resonance and we were invited to present our findings at the American Heart Association 2010.
Congenital Heart Surgery
I have conducted two major control randomised trials, supported by the BHF and NHIR BRU Bristol,
comparing normothermic vs hypothermic (THERMIC) and hypoxic vs normoxic (OXIC) CPB in paediatric
patients. Furthermore I have started a very new and exciting work on the relationship between chronic
hypoxia and gene expression, and the molecular mechanisms underlying the effects of re-oxygenation injury
on organ dysfunction in cyanotic heart patients undergoing surgical repair (BHF Fellowship). The OXIC
trial has had a significant international resonance and we were invited to present our findings at the
American Heart Association in 2011 and American Association of Thoracic Surgery in 2012 and 2014.
Furthermore the THERMIC Trial has provided the first evidence in a randomised controlled trial that in
infants and children requiring correction of congenital heart disease, normothermic CPB was associated with

reduced oxidative stress, myocardial reperfusion and renal injury, compared with hypothermic CPB,
opening an international debate of whether cooling children undergoing cardiac surgery is really necessary
and potentially detrimental. This trial was presented at the American Association of Thoracic Surgery in
2011 and won the John Parker Medal at the Annual Meeting of the Society for Cardiothoracic Surgery Great
Britain and Ireland.
Overall these last 4 years of my research in adult and congenital heart surgery have been a complete success.
They have provided vital data regarding CPB and myocardial protection strategies in a very vulnerable and
high risk group of patients. They have demonstrated the real possibility of conducting randomised studies in
children despite all the difficulties in enrolling this group of patients and the related lack of such studies
internationally. They have also given the Bristol Heart Institute and the Bristol Royal Hospital for Children
an international reputation in the field of paediatric cardiac surgery launching the base for vital international
collaboration. Finally and more importantly the results of all these studies have changed our clinical practice
and have greatly contributed to the outstanding clinical outcomes (30 day mortality of 1% form the CCAD
data with no mortality for the benchmark operations) that the Bristol Royal Hospital for Children has
achieved in the last 10 years, putting this Unit one of the top three in the country along with Birmingham’s
and Great Ormond Street.
Furthermore, I have recently secured funding (£ 300,000) for a new excited project in collaboration with
professor Anthony Hollander, on stem cell and tissue engineered valves, aiming at testing the in vitro
creation of tissue engineered valves and conduits based on decellularized matrix reseeded with autologous
cord blood and tissue stem cells. Tissue-engineered valves, conduits and vascular patches made of living
tissue could function like a native structure with the potential to grow and remodel, avoiding dangerous reoperations, and improving survival and quality of life in children undergoing heart surgery. This project has
generated a significant interest in the public (Alumni support) and industry (RED) for raising funding and
has been an important part of our recent application “Cell Therapy Technology & Innovation Centre” (TIC)
coordinated by RED. For this project I have been awarded the prestigeous Sir Jules Thorn Award for
Biochemical Research (£1.5M) as Principal Investigator.
My research activity and its translation into the clinical arena has played a vital part in our recent application
for the Safe and Sustainable process aiming at reducing the number of congenital cardiac centres in UK
from 11 to 6/7. Bristol Royal Hospital for Children was selected in all the possible options and will therefore
became the central hub in the South West for congenital cardiac surgery, performing more than 600-700
cases per year. This will give us an incredible opportunity to carry on and expand all our clinical trials and
translational research under the support of the BHF and NIHR BRU Bristol.

Research Grants
A total of 21 successful grants and a Senior Lecturer Fellowship for a total approximately of £4M.
In October 2008 he was the lead paediatric principal investigator for the successful award of the first Bristol
NIHR Biomedical Research Unit (BRU) in cardiovascular diseases (Total funding £7.1M) and for the most
recent renewal 2012-2017 (Total funding £7M). Of this funding approximately £1.5M have been awarded
to congenital heart surgery with Dr Caputo as theme leader and principal investigator. In 2014 he was
awarded the prestigious Sir Jules Thorn Award for Biochemical Research (£1.5M).

I have been responsible for preparing annual reports, calling for committee meetings, and organizing
seminars for basic scientist and clinical doctors in cardiovascular research. I have actively contributed to
visiting research charities and successfully managed a team of researchers and support staff. I have also

I have recently been nominated Medical Director responsible for the organization of the homograft bank
related to the cardiac surgical services within the Bristol Tissue Bank.

(9) CLINICAL DUTIES
My performance in clinical activity has been excellent. My job plan is divided between adult and paediatric
cardiac surgery. Up to date I have performed around 3000 cases from routine adult to a wide range of congenital
heart pathologies and grown up congenital heart diseases (average of 4 sessions a week and on call rota for
emergencies 1:2). My clinical outcomes are excellent with an overall mortality of <1% with 15 deaths among
the congenital cases in more than 1500 operations performed including the very complex and high risk neonates
(TGA, HLHS, TAPVD) and infants (<1%). Assessment of outcome against national standards shows that since
my appointment I am performing well above the standards of the national mortality rate for coronary and aortic
valve surgery (no mortality), and at one third the national mortality rate for all inclusive procedures.
Another major contribution in the clinical service has been in the field of the Grown Up Congenital Heart
(GUCH) Surgery Service. I have been responsible for the development of the complex aortic root reconstructive
surgery in Bristol (Ross and Marfan Syndrome Surgery) and the Ebstein Tricuspid Valve Repair
(www.ebsteins.org). I was the first in UK to perform minimally invasive pulmonary valve replacement (inject
able pulmonary valve) and awarded funding for the first multicentre randomised trial using this technique
(NIHR). I have been routinely invited in several UK (Glasgow, Manchester, Leicester) and European Cardiac
Units (Milan, Naples) to perform and teach these new surgical techniques as leading expert. At the same time,
surgeons from UK and abroad come to Bristol or send their patients to us because of the expertise we have
developed.
As a result of my clinical activity I have received recognition of excellence of clinical skills at national and
international level through regular invitations to courses and meetings. I have been appointed as an Honorary
Surgeon at The University of West Indies (Trinidad and Tobago), the charity organization CardioStart Medical
Missions (Peru), and the Al-Dir Charity Hospital (Homs University, Syria), to establish their congenital cardiac
surgical program (more than 500 paediatric operations performed with UK standard clinical outcomes).
Recently, I have been invited to establish the Paediatric Cardiac Unit at the University Hospital in Bucharest,

Romania. Finally from May 2014 I have been invited as visiting Professor to develop the Congenital Heart
Surgery program at the RUSH University Medical Center in Chicago.
(10) CONCLUSIONS
Overall these last 10 years my research and clinical activity in congenital heart surgery have been a complete
success. They have provided vital data regarding CPB and myocardial protection strategies in a very vulnerable
and high risk group of patients. They have demonstrated the real possibility of conducting randomised studies in
children despite all the difficulties in enrolling this group of patients and the related lack of such studies
internationally. They have also given the Bristol Heart Institute and the Bristol Royal Hospital for Children an
international reputation in the field of paediatric cardiac surgery launching the base for vital international
collaboration. Finally and more importantly the results of all these studies have changed our clinical practice
and have greatly contributed to the outstanding clinical outcomes that the Bristol Royal Hospital for Children
has achieved in the last 5 years, putting this Unit one of the best in the country. These past achievements and
innovative projects have played a major part for securing the £14M award achieved at the University of Bristol
and the University Hospitals Bristol NHS Foundation Trust to fund the Biochemical Research Unit (BRU) for
Cardiovascular Disease with congenital heart disease chosen as a major research area under my direct
leadership.
My research activity and its translation into the clinical arena has played a vital part in our recent application for
the Safe and Sustainable process aiming at reducing the number of congenital cardiac centres in UK from 11 to
7. Bristol Royal Hospital for Children was selected as one of the 7 Units and will therefore became the central
hub in the South West for congenital cardiac surgery, performing around 500-600 cases per year. This will give
me an incredible opportunity to carry on and expand all clinical trials and translational research under the
support of the British Heart Foundation and the Bristol NIHR Cardiovascular BRU.
For all my past and recent achievements I have been granted in 2012 the Chair in Congenital Cardiac Surgery at
the University of Bristol.
