1. Review the patient record for medical diagnosis, current
problems, and physicians’ orders. Check with the nurses,
and consult the patient record and nursing plan of care
to obtain information regarding any transfer techniques
that are part of the plan of care.

2. Assess the patient’s ability to move to plan for patient
participation in the process. Was the patient transferred
before?

3. Assess pulse, respirations, and blood pressure before
transfer.

4. Assess the patient for pain.
5. Assess the patient’s ability to cooperate and follow
directions to allow you to plan for patient participation.

6. Assess the patient’s need for eyeglasses or hearing aid.
7. Identify what equipment available for moving patients
and who is available to assist you.

8. Devise a plan to transfer patient in the safest and most
convenient manner, taking into account proper body
mechanics for you and the patient.

9. Wash or disinfect your hands
10. Obtain any needed supportive devices or assistance
11. Identify the patient using two identifiers
12. Tell the patient about the transfer technique you are
about to use

13. Raise or lower the bed to an appropriate working
position based on the transfer technique you will be
using.

14. Angle the wheelchair or the arm chair at a 45 degree
angle to the bed so

15. that the chair is on the patient’s stronger side.
16. If the footrests are removable, remove them at this time;
otherwise, fold them out of the way.

17. Lock the wheels of the bed and the wheelchair.
18. Begin with the patient in a supine position, close to the
edge of the bed,

19. with the hips placed where the bed will bend as the
head of the bed is raised.

20. Raise the head of the bed slowly until the patient is in a
sitting position.

21. Slide one arm under the patient’s legs and place the
other arm behind the patient’s back.

22. Swing the patient’s legs over the side of the bed while
pivoting the patient’s body,

23. so that the patient ends up sitting on the edge of the
bed with the feet hanging down.


24. Allow the patient to sit for a few minutes.

25. Be sure the patient sees the chair and its position. If the
patient is visually impaired, explain the location of the
wheelchair and of possible, place the patient’s hand on
it.

26. Place the gait or transfer belt around the patient’s waist.
27. Place firm slippers or shoes on patients feet.
28. Position the patient’s feet firmly on the floor and slightly
apart, with the patient’s hands on the bed.

29. Take a wide stance, bend your knees, and grasp the
sides of the belt.

30. Straddle the patient’s weaker leg with your own legs, or
you may want to stabilize the patient’s knees by
supporting them with your own knees.

31. Inform the patient that you will assist him or her to a
standing position on a count of “one, two, three, stand”
and

32. that he or she should lean slightly forward and push up
with the hands on the bed.

33. On the count of “three,” straighten your knees, assisting
patient to a standing position.

34. Stand close to the patient, and pivot to the chair.
35. Instruct the patient to place both hands on the arms of
the chair.

36. Lower the patient to the seat.
37. Be sure that patient’s body is positioned firmly back in
the chair seat and upright.

38. Drape the patient’s lap.
39. Angle the wheelchair or the arm chair at a 45 degree
angle to the bed so

40. that the chair is on the patient’s stronger side.
41. If the footrests are removable, remove them at this time;
otherwise, fold them out of the way.
